From Turkey: First Report of KPC-3-and CTX-M-27-Producing Multidrug-Resistant Klebsiella pneumoniae ST147 Clone Carrying OmpK36 and Ompk37 Porin Mutations

    Özad Düzgün, Azer

Aim: The aim of this study was to identify antimicrobial resistance genes, virulence factor genes, and porin loss or mutations exhibited by the multidrug-resistant Klebsiella pneumoniae strain. Materials and Methods: Whole-genome sequencing was done via the Illumina NovaSeq 6000 platform. Strain identification and antibiotic susceptibility testing of strains were performed by the Vitek 2 automated system. Multilocus sequence typing analysis was carried out using seven conserved housekeeping genes. Results: The strain was resistant to penicillins, cephalosporins, carbapenems, aminoglycosides, fluoroquinolones, fosfomycin, and trimethoprim/sulfamethoxazole. The isolate was found to carry KPC-3, CTX-M-27, SHV-11, SHV-67, and TEM-1 β-lactamases. The clonal subtype of the isolate was ST147, and it possessed wzi64 and wzc38 alleles. Fifteen different point mutations (N49S, L59V, R146H, V178P, G189T, F198Y, V202L, F207Y, A217S, T222L, D223G, H235N, A280V, N304E, and S346N) were detected in the OmpK36 porin. A frame shift was observed in OmpK35 and two different point mutations (I70M and I128M) were found in the OmpK37 porin, in addition to seven mutations observed on the AcrR. Conclusions: This study demonstrated for the first time that the ST147 clone produced CTX-M-27 as well as KPC-3. In addition, new mutations were detected in the outer membrane proteins. These mutations together with the production of extended-spectrum β-lactamase and carbapenemase were found to contribute to the resistance of the ST147 clone to carbapenem and other antibiotics.

Acet, Tuba; Özcan, Kadriye; Sefalı, Abdurrahman

Psephellus appendicigerus, is a plant endemic to Turkey, belonging to the Asteraceae family, which is traditionally used in folk medicine in Turkey. The aim of this study is to reveal for the first time the biological activities of this plant and evaluate its potential use as a natural product. For this reason, different extracts (ethanol, methanol, ethyl acetate, and hexane) of the plant’s aerial parts were used in determining the antioxidant (2,20-azino-bis(3-ethylbenzothiazoline-6-sulphonic acid and 1,1-diphenyl-2-picrylhydrazyl assays), antimicrobial (disc diffusion and microdilution methods), and enzyme inhibition activity (calorimetric methods) related to diabetes (α-amylase and α-glucosidase). Moreover, the phenolic constituents were analysed by high performance liquid chromatography. Methanol extract of the plant exhibited significant biological activity compared to other extracts. This extract had the highest amount of chlorogenic acid as phenolic compound. In addition, all extracts showed inhibition against at least one test microorganism. According to the results, P. appendicigerus has the potential to be used as a natural antioxidant, antimicrobial and enzyme inhibitor.

Topal, Fevzi; Aksu, Kadir; Gulcin, Ilhami; Tümer, Ferhan; Goksu, Süleyman

In this work, the inhibitory effect of some symmetric sulfamides derived from phenethylamines were determined against human carbonic anhydrase (hCA) I, and II isoenzymes, and compared with standard compound acetazolamide. IC50 values were obtained from the Enzyme activity (%)-[Symmetric sulfamides] graphs. Also, Ki values were calculated from the Lineweaver-Burk graphs. Some symmetric sulfamides compounds (11–18) demonstrated excellent inhibition effects against hCA I, and II isoenzymes. These compounds demonstrated effective inhibitory profiles with IC50 values in ranging from 21.66–28.88 nM against hCA I, 14.44–30.13 nM against hCA II. Among these compounds, the best Ki value for hCA I (Ki: 8.34±1.60 nM) and hCA II (Ki: 16.40±1.00 nM) is compound number 11. Besides, the IC50 value of acetazolamide used as a standard was determined as hCA I, hCA II 57.75 nM, 49.50 nM, respectively. Moreover, in silico ADME-Tox study showed that all synthesized compounds (11–18) had good oral bioavailability in light of Jorgensen's rule of three, and of Lipinski's rule of five.

Öz, Mehmet; Fidan, Muhammed Said; Baltaci, Cemalettin; Ücüncü, Osman; Karatas, Seyda Merve

In this study; the essential oils were obtained from the flowers and leaves of Atropa belladonna L. plants through the hydrodistillation method in a Clevenger type device. The chemical composition of volatile oils was determined by analyzing with the GC-MS/MS instrument. The antioxidant activity of the obtained volatile oils was determined according to total antioxidant content (TAC), free radical scavenging (ABTS) capacity, free radical scavenging (DPPH) amounts, total flavonoid content (TFC), total phenolic content (TPC), and ferric reducing antioxidant power (FRAP) capacity methods. Antimicrobial activities of essential oils were determined through the agar-disc diffusion method. As for the results; volatile oil yields of the A. belladonna were determined as 0.37 % in leaves and 1.57 % in flowers. The highest percentage of chemical compounds in essential oils of the A. belladonna plant were determined as (43.75 %) aldehydes in leaves and as (39.96 %) fatty acids in flowers. In addition, the main constituents found in volatile oils were eicosane (35.92 %) in the leaves and hexadecenoic acid (18.84 %) in the flowers of the A. belladonna. The highest amounts of antioxidant for the test specimens were found as 1.37-1.48 mg QE/100 mL in TFC, as 13.25-21.91 mg AA/100 mL in ABTS, as 51.49-62.87 mg GA/100 mL in TPC, and as 4.71-7.97 mg FeSO4/100 mL in FRAP capacity. As the results of the antimicrobial activities, it was determined that only the leaves of A. belladonna show antimicrobial activity against the mold-yeast and gram-positive and gram-negative bacteria microorganisms among the 17 bacteria and 6 yeast-mold types used in the study.

Effect of far infrared heating process on surface decontamination and quality attributes of whole yellow and white onions

Coskun, Eda; Ozturk, Samet; Akpinar, Merve; Halkman, A. Kadir; Erdogdu, Ferruh

This study aimed to develop an effective decontamination process for whole yellow and white onions with maintained quality attributes. For this purpose, a far infrared radiation (FIR) process was evaluated to determine its effects on microbial safety and quality of the onions. Prior to the FIR process, samples were inoculated with Salmonella culture to about 7–8 log10 CFU/cm2 and stored for 1–48 h at room temperature. Inoculated samples were then processed up to 120 s at 400 °C infrared source temperature. Temperature chance of the onions was recorded by using T-type thermocouples, and surface temperature distribution was obtained with an infrared thermal camera. Quality attributes (color changes, mass losses and textural changes) were also evaluated following the FIR process. The results indicated that 3.3 and 2.4 log10 CFU/cm2 reductions in the Salmonella counts were obtained for the whole yellow and white onions, stored for 1 h at room temperature after 120 s FIR processing. The background microbial population on the outer skin of the onions was under the detection limit (<1 log10 CFU/g). It was observed that the effectiveness of FIR treatment on Salmonella decontamination decreased as the storage time increased from 1 to 48 h at room temperature. FIR process did not significantly affect the quality attributes of the onions, and the results confirmed the potential use of FIR process for surface decontamination of the onions. Further studies were suggested to design an effective FIR system for industrial scale applications.

The effects of noise levels on pain, anxiety, and sleep in patients

Aydın Sayılan, Aylin; Kulakaç, Nurşen; Sayılan, Samet

Background: Intensive care is a noisy environment for patients and one that affects pain, anxiety levels, and sleep quality. Aims and objectives: To determine the relationship between noise levels and pain, anxiety, and sleep levels in patients in intensive care units. Design: A descriptive and observational study design was used. Methods: This study was conducted between June and December 2018 in a public hospital and included 111 patients admitted to surgical critical care for at least 24 hours. Three Benetech Gm1351 manual sound level metres were used to measure noise. A Patient Information Form, a pain Visual Analog Scale (VAS), the Spielberger State-Trait Anxiety Inventory, and the Richards Campbell Sleep Questionnaire (RCSQ) were used for data collection. Results: The mean age of the patients was 57.29 years. The mean noise level detected in the intensive care unit was 66.52 dB (dB). Patients' mean pain VAS score was 3.79 ± 1.72, the mean State Anxiety Inventory score was 39.74 ± 2.98, and the mean total RCSQ score was 25.10 ± 13.17. Our findings show that patients in the intensive care unit are exposed to high noise levels and that, while this has no effect on pain, it significantly impacts anxiety and quality of sleep. Conclusions: Noise levels in intensive care units significantly exceed recommended thresholds, and this adversely affects patients' anxiety levels and sleep quality. It is important for suitably restful conditions to be provided for patients, to be aware of the potential for anxiety, and for these factors to be borne in mind when planning nursing interventions. Relevance to clinical practice: Further studies on the effects of noise levels on pain, anxiety, and sleep levels in patients admitted to intensive care units are needed.

Automatic Classification of COVID-19 using CT-Scan Images

    Reis, Hatice Catal

Medicine and engineering sciences have been working in close contact for common purposes. Machine learning algorithms are used in the medical field for early diagnosis prediction. The major aim of this study is to evaluate machine learning algorithms and deep learning algorithms using computed tomography scan (CT-scan) images for automated detection of the coronavirus disease 2019 (COVID-19) patients. We obtained seven hundred and fifty-seven (757) CT-scan images from a public platform. We applied four automated traditional classification methods to predict COVID-19 using deep learning and machine learning. These algorithms are SVM, AdaBoost, NASNetMobile, and InceptionV3. Comparative analyses are presented among the four models by considering metric performance factors to find the best model. The results show that the InceptionV3 model achieves better performance in terms of accuracy, precision, recall, Cohen’s kappa, F1-score, root mean squared error (RMSE), and receiver operating characteristic-area under the curve (ROC-AUC), in comparison with the other Covid-19 classifiers. Accordingly, the InceptionV3 approach is recommended for the automatic diagnosis of Covid-19 and assessments. This research can present a second point of view to medical experts and it can save time for researchers as the performance of standard machine learning methods in detecting COVID-19 is evaluated.

Meric, Çağdaş Salih; Ayhan, Nurcan Yabanci; Yilmaz, Hacı Ömer

Today, increased intake of sugar and sugar-sweetened beverages is seen as an important factor in the growing prevalence of chronic diseases, such as obesity, obesity-related diabetes, and coronary heart diseases. This study involved 214 Ankara University students from the Department of Nutrition and Dietetics, which was intended to evaluate the consumption of sugar and sugar-sweetened beverages. The frequency of student consumption of beverages and the quantities and amounts of sugar taken with beverages were questioned. The average total amount of sugar added to drinks by the students was 4.69 ± 6.35 gram, while the average total amount of sugar taken with sugar-sweetened beverages was 11.34 ± 15.32 gram. Female students relative to male students, students in grade 4 compared to grades 2 and 3, and students who had daily breakfast compared to those who did not had lower average sugar consumption (p-value < 0.05). As class grades increase, the amount of sugar added to beverages and fruit juice and sweetened soft drinks and carbonated beverages decreased (p-value < 0.05). The consumption of herbal teas and dietary beverages increased and the eating habits of students generally changed positively (p-value > 0.05). It is important to educate university students about the reduction in sugar intake and sugar-containing food in order to avoid many chronic diseases that may be seen in older ages.

Exopolysaccharide of Anoxybacillus pushchinoensis G11 has antitumor and antibiofilm activities

Genc, Berna; Taskin, Mesut; Adiguzel, Ahmet

Exopolysaccharides (EPS/EPSs) possess several various applications in the food and pharmaceutical industries. This study was performed to investigate the biological (antibiofilm and antitumor), rheological (temperature, shear rate, and density) and chemical (solubility, carbohydrate and protein content, composition, molecular weight, functional group analysis, thermal analysis, X-ray diffraction pattern and scanning electron microscopy) properties of the EPS, which was purified from the locally isolated thermophilic bacterium Anoxybacillus pushchinoensis G11 (MN720646). EPS was found to have antibiofilm and antitumor [lung (A-549) and colon (Caco-2 and HT-29) cancer] activities. The viscosity of EPS showing Newtonian flow was temperature dependent. As chemical properties, the EPS was found to be a heteropolysaccharide containing arabinose (57%), fructose (26%), glucose (12%), and galactose (5%). EPS contained 93% carbohydrates and 1.08% protein. The molecular weight of EPS was determined as 75.5 kDa. The FTIR analysis confirmed the presence of sulfate ester (band at 1217 cm−1), an indication of the antitumor effect. The EPS was semi-crystalline. It could maintain 36% of its weight at 800 °C and crystallization and melting temperatures were 221 and 255.6 °C. This is the first report on the EPS production potential and the biological activity of A. pushchinoensis.

Cebi, Mehmet; Ceviker, Abdulkerim; Ozlu, Kurtulus; Elioz, Murat; Yamak, Bade; Kusan, Osman; Agaoglu, Seydi Ahmet; Dogan, Erol; Ceylan, Levent

Study Objectives: In this study, it was aimed to evaluate the relationship between vitamin D and lipid profiles of athletes playing football professionally during the season and some metabolic parameters and maximal rate. Methods: 52 male elite soccer players performing in Super League, TFF 2nd League, and TFF 3rd League whose mean age 22.86±3.55 years, participated in our study. The sport age of the soccer players was 12.2±1.70 years. The participants attended in at least five 80-minute training periods and performed at least one official competition weekly (through 11 months). In November, blood samplings of the athletes and other parameters were obtained between 09:00 and 10:00 a. m. leading a 12-hour fasting period. The Maximal Aerobic Speed (MAS) measurement developed by Buchheit (2008) to evaluate aerobic power and capacity was used to carry out the study. In order to determine the relationship between the variables of individuals, Pearson Correlation Test was used as the data showed normal distribution. Results: Albumin, Alp, Alt, Ast, B12 vitamin, Ca, Phosphorus, Creatinine, Mg, Free T3, Sodium, Tsh, Ferritin, Folic Acid, Potassium Triglyceride, Glucose, Total Cholesterol, and Free T4 values were found within normal limits. It can be said that athletes had low vitamin D values. It was observed that the participants had low LDL cholesterol values. Variables don’t have any significant effects on running time, peak velocity, true vift and VO2max of biochemical parameters (p>0,05). Conclusion: As a result, it can be concluded that despite the positive relationship between the maximal aerobic velocity test parameters of the footballers and their pre-test biochemical results, it wasn’t statistically a meaningful relationship.

An automatic EEG-based sleep staging system with introducing NAoSP and NAoGP as new metrics for sleep staging systems

Melek, Mesut; Manshouri, Negin; Kayikcioglu, Temel

Different biological signals are recorded in sleep labs during sleep for the diagnosis and treatment of human sleep problems. Classification of sleep stages with electroencephalography (EEG) is preferred to other biological signals due to its advantages such as providing clinical information, cost-effectiveness, comfort, and ease of use. The evaluation of EEG signals taken during sleep by clinicians is a tiring, time-consuming, and error-prone method. Therefore, it is clinically mandatory to determine sleep stages by using software-supported systems. Like all classification problems, the accuracy rate is used to compare the performance of studies in this domain, but this metric can be accurate when the number of observations is equal in classes. However, since there is not an equal number of observations in sleep stages, this metric is insufficient in the evaluation of such systems. For this purpose, in recent years, Cohen’s kappa coefficient and even the sensitivity of NREM1 have been used for comparing the performance of these systems. Still, none of them examine the system from all dimensions. Therefore, in this study, two new metrics based on the polygon area metric, called the normalized area of sensitivity polygon and normalized area of the general polygon, are proposed for the performance evaluation of sleep staging systems. In addition, a new sleep staging system is introduced using the applications offered by the MATLAB program. The existing systems discussed in the literature were examined with the proposed metrics, and the best systems were compared with the proposed sleep staging system. According to the results, the proposed system excels in comparison with the most advanced machine learning methods. The single-channel method introduced based on the proposed metrics can be used for robust and reliable sleep stage classification from all dimensions required for real-time applications.

Assessment of the concentration and environmental impacts of potential hazardous elements in coalfields in Northeastern Turkey

    Saydam Eker, Ç.; Sipahi, F.; Akpınar, I.

To investigate the impact of potential hazardous element (PHE) contamination on environmental and human health, the coaly shale, coal and shale coal outcrops of four sites, namely Ozyurt, Tarhanas, Kayadibi (Gümüşhane), and Manas (Bayburt), were selected as sampling areas for this study. The samples were analyzed using inductively coupled plasma (ICP) emission spectrometry and inductively coupled plasma-mass spectrometry (ICP-MS) and a Rock–Eval 6 analyzer with a total organic carbon module. The enrichment factor (EF), potential ecological risk index (RI), and potential health risk index (HI) assessment models were used to evaluate the environmental and health risks in the vicinity of the coals. Results indicated that the mean concentrations of PHEs extensively changed and even exceeded the upper continental crust and world average values of low-rank coal concentrations. The assessment of the EF, RI and HI models revealed that the contamination levels of As, Hg, and Cd were higher than those of Pb, Co, Ni, V, and Mo. The evaluation of the EF revealed that the largest and lowest contamination levels of As, Hg, and Cd were observed in Ozyurt (29) and Kayadibi (1.8), Manas (9.6) and Kayadibi (1), and Kayadibi (5.6) and Tarhanas (1.9), respectively. A high-potential ecological risk was present in Ozyurt and Manas, whereas a considerable risk exists in Kayadibi and Tarhanas. The potential health risk values of As for children in Ozyurt, Tarhanas, and Manas and adults in Ozyurt were excessively high. This finding suggested the possibility of transferring the contamination from the coal, shale coal, and coaly shale outcrops

An Investigation of the Effect of COVID-19 on OCD in Youth in the Context of Emotional Reactivity, Experiential Avoidance, Depression and Anxiety

    Seçer, İsmail; Ulaş, Sümeyye

In addition to the serious physical and medical effects on individuals, the COVID-19 pandemic is likely to have short- and long-term psycho-social consequences, especially for young people. Nowadays, with psychological problems becoming more widely recognized in adolescents, it is possible that the fear and anxiety caused by the pandemic will trigger various anxiety disorders, OCD and similar negative outcomes. Considering that psychological qualities such as emotional reactivity and experiential avoidance observed in adolescents may increase the risk of such psycho-social disorders, in this study the relationship between fear of COVID-19 and OCD was investigated in a Turkish sample of 598 adolescents, and the mediating role of emotional reactivity, experiential avoidance and depression-anxiety in this relationship was examined. The data collection was performed online rather than in person because of the COVID-19 threat. A structural equation model was used to determine the direct and indirect predictive effects between variables in data analysis. The results of the study show that the effect of COVID-19 fear on OCD is mediated by emotional reactivity, experiential avoidance and depression-anxiety.

Introduction: The worldwide spread of coronavirus over the last 20 years has resulted in its becoming a major threat to health. The virus is thought to be a zoonotic disease spread by means of direct and contact transmission. The purpose of this study was to investigate the symptoms, anxiety-depression levels, and quality of life of patients with Covid-19. Material and methods: This descriptive, cross-sectional research was conducted in an education and research hospital between August and November 2020. An information form, the Hospital Anxiety and Depression Scale, and the Quality of Life Scale were employed as data collection tools. The study was completed with 103 individuals. Results: The mean age of the patients enrolled in the study was 37.77±10.92 years, and 67% were women. Higher anxiety levels were determined in women compared to men (p=0.001) and in married participants compared to unmarried individuals (p=0.010). Significant negative correlation was determined between anxiety and the Quality of Life Scale sub-parameters of general health, physical health, psychological health, and environment. Positive correlation was found between Covid-19 severity and anxiety, and negative correlation between Covid-19 severity and physical health (p<0.001). Conclusion: Higher anxiety was determined among women and married patients in this study, and quality of life (in the four sub-parameters) decreased as anxiety increased. Physical health decreased, while anxiety increased, with the severity of Covid.
