- BildiriYalnız künye2011Hydroxyapatite production on ultrafine-grained pure titanium by micro-arc oxidation and hydrothermal treatment
Alsaran, A.; Purcek, G.; Hacisalihoglu, I.; Vangolu, Y.; Bayrak, O.; Karaman, I.; Celik, A.
In this study, pure-Ti (Grade-2) was severely plastically deformed by equal-channel angular extrusion (ECAE) to improve its strength, and then subjected to micro arc oxidation (MAO) and hydrothermal treatment (HT) to achieve improved surface behavior with hydroxyapatite (HA)-containing titania (TiO(2)) coating. Microstructure, tensile properties and wear behavior of UFG Ti were investigated systematically before and after surface modification. ECAE considerably enhanced the strength of coarse-grained (CG) Ti by forming an ultra-fine grained (UFG) microstructure with high dislocation density. The application of MAO with or without HT did not cause remarkable decrease in strength and ductility of CG and UFG Ti, but resulted in a considerable increase in its wear resistance and osseointegration due to the formation of hard and adhered HA-containing TiO2 coating on the surface. It can be concluded that the use of ECAE plus MAO/HA coating provides a simple and effective procedure to obtain pure Ti with high strength and high wear resistance. The improved bulk and surface properties can make this biocompatible material a good potential candidate for biomedical applications instead of alloys including toxic elements. (C) 2011 Elsevier B.V. Memetoglu, Mehmet Erdem; Kurtcan, Serpil; Kalkan, Asim; Özel, Deniz
Objective: This study aimed at evaluating the efficiency of applying tumescent anesthesia before an ablation procedure and continuously during endovenous laser treatment (EVLT) (combined tumescent technique) of saphenous vein insufficiency to reduce the patients pain and discomfort. Methods: Twenty-five patients with 31 duplex-confirmed great saphenous vein insufficiency underwent endovenous laser (940 nm) varicose vein treatment between December 2009 and April 2010. The patients were randomized in two groups. In group A (15 patients) tumescent anesthesia was used before the ablation procedure and in group B (10 patients) tumescent anesthesia was used before and continuously during the procedure. Patients were scheduled for a three-day examination after EVLT to assess the level of pain experienced. Results: According to the statistic analysis we observed that the mean level of pain score for the patients given classical tumescent anesthesia showed a higher level (P=0.003) compared with the patients given tumescent anesthesia in the combined procedure. Conclusion: Combination technique of administering tumescent anesthesia before ablation and continuously during the EVLT procedure may be an alternative way to reduce the patient's peroperative pain and discomfort. - MakaleAçık Erişim2010The Relationship Between Knee Osteoarthritis and Osteoporosis
Sezer, Ilhan; Illeez, Ozge G.; Tuna, Serpil D.; Balci, Nilufer
Objective: The aim of this study was to investigate the association between knee osteoarthritis (OA) and bone mineral density (BMD) in the femur and lumbar vertebrae. Materials and Methods: A total of 74 female patients (mean age 61.9 +/- 9.1 years, mean body mass index 27.09 +/- 4.24) diagnosed with knee OA were included in this study. To assess knee OA, bilateral weight-bearing antero-posterior knee radiographs were taken and graded from 0 to 4 according to Kellgren-Lawrence criteria. The BMD of the subjects was measured using dual-energy X-ray absorptiometry (DEXA). BMD measurements of those with OA were compared with those without OA. Results: While there was no correlation between BMD and the grade of knee OA, a significant negative correlation was found between age and femur BMD. Body mass index was positively correlated with OA and negatively correlated with OP. Conclusion: Further investigations are needed to demonstrate the association between knee OA and BMD.

Background: The prognostic significance of perineural invasion (PNI) in gastric cancer has been previously investigated in a few studies, but had not reached a consensus. The aim of this study was to determine the prognostic value of PNI in patients with gastric cancer who underwent curative resection. Materials and Methods: We retrospectively analyzed 238 patients who had undergone curative gastrectomy. Paraffin sections of surgical specimens from all patients were stained with hematoxylin and eosin. PNI was defined when carcinoma cells infiltrated into the perineurium or neural fascicles. PNI and the other prognostic factors were evaluated by univariate and multivariate analysis. Results: PNI was detected as positive in 180 of the 238 patients (75.6%). pT stage, tumor size, lymph node metastasis, clinical stage, tumor differentiation, Borrmann classification, histological type, lymphatic vessel invasion, and blood vessel invasion were closely associated with the presence of PNI. The PNI-positive tumors had significantly larger size and more lymph node metastasis than the PNI-negative tumors (P =.001 and P <.001, respectively). The median survival of the PNI-positive patients was significantly worse than that of the PNI-negative patients (28.1 vs. 64.9 months, P =.001). Multivariate analysis indicated that the positivity of PNI was an independent prognostic factor (P =.02, hazard ratio [HR]: 2.75; 95% confidence interval [95% CI]:1.12-3.13) as were classical clinicopathological features. Conclusion: Our results showed that the frequency of PNI was high in patients with gastric cancer who underwent curative gastrectomy and the proportion of PNI positivity increased with progression and clinical stage of disease. PNI may be useful in detecting patients who had poor prognosis after curative resection in gastric cancer.

Alici, A.; Kement, M.; Gezen, C.; Akın, T.; Vural, S.; Okkabaz, N.; Basturk, E.; Yegenoglu, A.; Oncel, M.
Background: What level of arterial ligation is best in left-sided colon cancer and rectal cancer remains controversial. This study aims to assess the necessity and risk of high ligation from an oncological and technical perspective. Methods: The lymph nodes at the origin of the inferior mesenteric artery (IMA) were separated as apical nodes in all patients operated for distal colorectal cancer in our department. The number and status of the nodes were prospectively assessed, and demographic and tumor-related variables were evaluated as risk factors for apical tumor invasion. Anastomotic leaks were also evaluated. Results: A hundred and three patients (52 [50.5%] males, 60.3 ± 12.9 years old) were included. The number of non-apical lymph nodes harvested was 14.5 ± 7.1 with an additional 4.4 ± 3.2 apical nodes at the high ligation site. Tumor invasion of apical nodes was observed in 6 (5.8%) patients. Two of these (1.9%) had no other positive nodes (skip metastases). Although none of the variables evaluated was found significant for predicting apical node positivity, tumor invasion was detected in 8.5 and 22.2% of patients with pT3 and pN2 cancers, respectively. Among patients, who had an anastomosis (n = 84, 81.6%), anastomotic leak was observed in 7(8.3%) and 1 (1.2%) of these patients required emergency relaparotomy. There was no mortality related to high ligation. Conclusions: High ligation of IMA may be routinely performed in patients with distal colorectal cancer, since tumor invasion of apical lymph nodes is neither rare (>5%) nor predictable, and skip metastases may also occur. This is especially true in case of an advanced disease for which apical node positivity peaks. The anastomotic leak rate is less than 10%, and mortality is low after high ligation of IMA.

Background: Axillary lymph nodes (ALNs) are the most important prognostic factor for survival in breast cancer. Pathological evaluation can affect the number of involved lymph nodes. In the current study, we evaluated whether the metastatic lymph node ratio (n ratio) is important in predicting disease-free survival (DFS) for breast cancer patients. Material and Methods: From 802 breast cancer cases, 427 patients with ALN metastasis were analyzed retrospectively. The n ratio was categorized as n ratio 1 (1-10%), n ratio 2 (10.01-50%) and n ratio 3 (>50%). DFS was established according to the Kaplan-Meier method. Predicting risk factors for relapse were analyzed using the Cox proportional hazards model. Results: The n ratio was significantly higher in breast cancer patients with advanced pathologic pT, pN and clinical stage, undifferentiated histology, lymphovascular and extracapsular invasion, more resected ALNs and positive progesterone receptor. In the univariate analysis, multicentricity, necrosis, grade, pN stage, estrogen receptor and progesterone receptor positivity, trastuzumab and neoadjuvant chemotherapy usage, the presence of inflammatory breast cancer and n ratio were found to be important factors in predicting DFS. Multivariate analysis indicated that multicentricity, neoadjuvant chemotherapy, trastuzumab usage and n ratio were significantly associated with prognosis. Conclusions: The n ratio is inexpensive, easily available and a simple prognostic factor for breast cancer patients with positive ALNs.

Gokhan, Nezihe; Sozmen, Mahmut; Ozba, Burhan; Gungor, Emine
Objective: To describe the clinical and pathomorphological features of meibomian carcinoma (MC) diagnosed in a Simmental cow. Material and methods: A 7-year-old Simmental cow was admitted to the Veterinary Surgery Clinics of Faculty of Veterinary Medicine, University of Kafkas, Kars, Turkey. The signalment and anatomical distribution were summarized and focused on pathomorphological description. Results: Macroscopically, a papillary mass, 5 × 3.5 × 7 cm in size, arising from the left lower eyelid with local spread to the upper eyelid and covering the entire globe was observed. The mass was ulcerated and hemorrhagic and applied pressure to the globe. Following sedation and local anesthesia, the mass and globe were excised. Histopathological examination revealed an MC. Conclusions: The diagnosis of MC is infrequent in veterinary literature. Complete surgical excision could be curative and histopathology is crucial for the diagnosis of MC occurring in animals.

Lakse, Emre; Gunduz, Berrin; Erhan, Belgin; Celik, Evrim Coskun
Objective: The aim of this study was to evaluate the effects of corticosteroid injections on hemiplegic shoulder pain and range of motion. Design: Fifty-two stroke patients with shoulder pain were evaluated. Thirty-eight patients who fulfilled the criteria were assigned randomly into either injection or control groups. Transcutaneous electrical nerve stimulation and therapeutic exercise program were applied to both groups for 15 sessions. The patients in the injection group were administered either intra-articular or subacromial injections of corticosteroid and local anesthetic according to their pathology. The evaluations were performed before treatment and at the first and fourth weeks of treatment. Primary follow-up parameters were passive shoulder range of motion and verbal analog scale for shoulder pain; secondary follow-up parameters were modified Barthel index, Brunnstrom upper-extremity score, and modified Ashworth scale. Results: In both groups, shoulder range of motion and shoulder pain scores showed significant improvement. When the two groups were compared, the improvement was more significant in the injection group. Barthel scores improved in both groups, and no difference was found between the groups. Brunnstrom upper-extremity and modified Ashworth scores revealed nonsignificant changes in both groups. Conclusions: Adding corticosteroid injection to conventional treatment in hemiplegic shoulder pain improved shoulder range of motion and decreased pain scores before treatment to the first and fourth weeks of treatment. Injection in hemiplegic shoulder pain is recommended in appropriate patients.

Merdan, Mehmet
In this article, homotopy perturbation method is implemented to give approximate and analytical solutions of nonlinear ordinary differential equation systems such as human T-cell lymphotropic virus I (HTLV-I) infection of CD4+ T-cells model. The proposed scheme is based on homotopy perturbation method (HPM), Laplace transform and Padé approximants. The results to get the homotopy perturbation method (HPM) is applied Padé approximants. Our proposed approach showed results to analytical solutions of nonlinear ordinary differential equation systems. Some plots arepresented to show the reliability and simplicity of the methods.
- MakaleYalnız künye2005The protective role of erdosteine on testicular tissue after testicular torsion and detorsion
Koc, A; Narci, A; Duru, M; Gergerlioglu, HS; Akaydin, Y; Sogut, S
Testicular torsion and detorsion are important clinical problems for infertile man and oxidative stress may have a role in this clinical situation. The aim of this study was to investigate the protective role of erdosteine, an antioxidant, on unilateral testicular reperfusion injury in rats. The rats were divided into four groups including seven rats in each group: control, torsion, torsion/detorsion and torsion/detorsion+erdosteine. Rats, except the sham operation group, were subjected to left unilateral torsion (720 degrees rotation in the clockwise direction) without including the epididymis. The experiments were finished after sham operation time for control, 120 min torsion for torsion group and 120 min torsion and 240 min detorsion for torsion/detorsion groups. Bilateral orchiectomy was performed for all groups of rats. The ipsilateral and controlateral testis were divided into two pieces to analyse biochemical parameters and to investigate the light microscopic view. Malondialdehyde level of ipsilateral testis was increased in torsion and torsion/detorsion groups in comparison with the other groups (p < 0.05). Erdosteine treatment ameliorated lipid peroxidation after torsion/detorsion in ipsilateral testis (p < 0.05). Also, xanthine oxidase activity of ipsilateral testis was increased in torsion/detorsion group in comparison with the others (p < 0.05). Nitric oxide (NO) level of ipsilateral testis was higher in all experimental groups than sham operated control group (p < 0.05). Also, NO level of torsion group was increased in comparison with detorsion groups (p < 0.05). Erdosteine treatment caused increased glutathione peroxidase activity in comparison with torsion and torsion/detorsion groups and catalase activity in comparison with the other groups in ipsilateral testis (p < 0.05). Superoxide dismutase activity of ipsilateral testis was higher in torsion/detorsion and torsion/detorsion+erdosteine groups than control and torsion groups (p < 0.05). The biochemical parameters were not affected in controlateral testis in all groups. Torsion, torsion/detorsion and torsion/detorsion+erdosteine groups showed ipsilateral testicular damage in the histological examination, but the specimens from torsion/detorsion had a significantly greater histological injury than those from the other groups (p < 0.05). Control rats showed normal seminiferous tubule morphology. Rats in torsion group had slight-to-moderate disruption of the seminiferous epithelium. Rats in torsion/detorsion group displayed moderate-to-severe disruption of the seminiferous epithelium. In all animals from torsion/detorsion+erdosteine group, the testicular tissues were affected with slight-to-moderate degenerative changes of the seminiferous epithelium. Administration of erdosteine resulted in a significantly reduced histological damage associated with torsion of the spermatic cord compared with torsion/detorsion. In all groups, the contralateral testes were histologically normal. In conclusion, the results clearly displayed that erdosteine treatment may have a protective role on testicular torsion/detorsion injury.
