Can, İbrahim; Bayrakdaroğlu, Serdar; Ertürk, Ceren

High-Risky Sectors in Terms of Work Accidents in Turkey

Ceylan, Hüseyin; Kaplan, Ahmet; Bekar, Murat

Work accidents are serious issues that disrupt harmonious environment in workplace. They cause not only death, injury or disability of employees, but also serious financial loss for businesses and countries. Work accidents are highly preventable incidents, when necessary, precautions are taken. Like the whole world, in Turkey also there are risky sectors where work accidents are intensified. The identification of risky sectors in Turkey, is the first step of being able to fight against work accidents in this sector. Therefore, in this study the riskiest sectors in Turkey were investigated basing on the official accident data from the last ten years. Thus, first of all, the sectoral distribution of work accidents, death and permanent incapacity cases that occurred in Turkey in 2010-2019 was examined. Sectors were evaluated in terms of six different parameters: the number of work accidents, the number of deaths because of work accidents, the number of permanent incapacities because of work accidents, general accident incidence rate, fatal accident incidence rate, and permanent incapacity accident incidence rate. The top ten sectors with intensity have been determined for each parameter. Then, all these data were combined and the ten most risky sectors in Turkey in terms of work accidents were defined. In the last 10 years, the sector of "41-Building Construction" has been found to be the riskiest sector in Turkey, where the most work accidents have occurred, the most employees die and the most employees are permanently disabled. On the other hand, the "05- Mining of Coal and Lignite" sector, in which the general accident incidence rate, fatal accident incidence rate, and permanent incapacity accident incidence rate values are the highest, was the second most risky sector.

Technologized Life as an Imagining of Eternality from Antenatal to Post-Mortem

    Demir, Sertac Timur

In this article, everyday technology, which tends to expand without limits, is discussed in the context of contemporary practices that start from antenatal to death and that correspond to the desire for immortality of modern man. For this purpose, it is examined how technology intervenes in life and death through the episodes of Arkangel, Right Be Back, and San Junipero in Black Mirror, the popular science fiction series. In this study, the current social and human dilemmas that appear in the narratives are discussed, without ignoring the technical analysis of the series. In this respect, gene based medical studies that reformat the body, smart and synthetic technologies that add a new dimension to digital control and surveillance, contemporary medical and technoscientific assumptions about death and immortality are debated. The theoretical framework of this article consists of theoreticians such as Jean Baudrillard, David Le Breton and Byung Chul Han who consider that the change in social structure and relations is mainly driven by technology and critically approach this unavoidable trend. According to the main argument and inference of this research, technologies that can descend to every part of the body are no longer just the comfort and convenience extensions of daily lite, and they affect almost all ontological processes between existence and extinction by being articulated with human destiny. However, this global trend is also the loss of the natural and the real for the modern characters who are deprived of the human conditions such as pain and death that make life precious.

Yilmaz, Mehmet Fatih; Acar, Emrah; Inanir, Mehmet; Karabay, Can Yucel; Izgi, Ibrahim Akin

Introduction Data regarding the possible role of heparanase (HPA) in the occurrence of left atrial/left atrial appendage (LA/LAA) thrombus in patients with atrial fibrillation (AF) is lacking. The goal of the present study was to assess the association between plasma levels of HPA and LA/LAA thrombus in AF. Methods A total of 687 patients with nonvalvular AF (NVAF) without anticoagulation therapy were included from January 2016 to June 2019. Serum HPA analysis was performed with a commercially available human ELISA kit. Logistic regression models were used to test for association. Results Serum HPA levels were significantly higher in patients with LA/LAA thrombus than in those without LA/LAA thrombus (270.8 [193.4 +/- 353.2] pg/mL vs 150.3 [125.2 +/- 208.4] pg/mL; P < 0.001). In multivariate analysis, serum HPA remained a significantly independent predictor of LA/LAA thrombus (odds ratio 1.674, 95% confidence interval [CI] 1.339-2.289, P < 0.001). In the receiver operating characteristic (ROC) curve analysis, HPA showed a predictive value with an area under the curve (AUC) of 0.757 (95% CI 0.652-0.810, P < 0.001). The optimal cutoff level for HPA predicting LA/LAA thrombus was 210.7 pg/mL, with a sensitivity of 74.3% and a specificity of 64.8%. Conclusion An elevated HPA level was associated with the presence of LA/LAA thrombus in patients with AF. HPA might portend the risk for the prothrombotic state in AF patients.

Urologists Are No Longer the Primary Surgeons for Several Urologic Operations: A National Survey Among the Turkish Urologists

Madendere, Serdar; Deger, Muslim Dogan; Demirkiran, Engin Denizhan; Yildiz, Huseyin Alperen

Objective: To investigate the role, interest, and competence of the Turkish urologists for urologic surgeries which are also performed by the other surgeons. Materials and Methods: We conducted a 22-item online survey and invited urologists by e-mail. Questions were asked to evaluate the attitudes, performances, and capabilities of the urologists in surgeries which could be performed by the other surgeons. Results: A total of 305 responses were evaluated. The female incontinence surgery, pediatric urologic surgery, and adrenal surgery are mostly performed by the urologists in their hospitals. Turkish urologists mostly do not perform the following surgeries and interventions themselves and refer patients to the other surgeons for a percutaneous nephrostomy (47.2%), urinary diversions using the intestinal segments (44.9%), kidney transplantation (11.8%), and the transgender surgeries (7.2%). Almost half of the urologists do not perform adrenal surgeries (50.2%). Most urologists agree that they should be the main surgeons for all these surgeries. Conclusion: Urologists are no longer the main surgeons for some surgeries which are included on the urologic curriculum. Many urologists avoid performing the major urologic surgeries. Urology educators should familiarize the new urologists with all the urologic surgeries.

Segmentation of Pectoral Muscle Region in MLO Mammography Images by Backboned U-Net

Dogan, Ramazan Ozgur; Türe, Hayati; Kayikçioglu, Temel

The pectoral muscle region on MLO mammography images appears prominently similar to suspicious areas. For this reason, Computer-Aided Detection (CAD) systems remove this region to reduce false-positive rates in the mass detection process. In some cases, the pectoral muscle region is exposed to distortions due to the superposition effects caused by the mammography technique. As a result, segmentation error rates of the pectoral muscle region, whose characteristic features are deteriorated, appear. In this study, a method to identify impaired pectoral muscle regions with MobileNetV2 backboned U-Net Deep Learning method is proposed. The proposed method was tested on 84 and 201 mammography images taken from both MIAS and InBreast databases and segmented with 1.81% and 1.92% false-negative (FN) and 0.25% and 0.37% false positive (FP) rates, respectively. Particularly for distorted pectoral muscle regions, the proposed method has been shown to outperform some pioneering studies in this area.

COVID-19 diagnosis with deep learning; [Diagnóstico de COVID-19 con Deep Learning]

    Reis, Hatice Catal

The coronavirus disease 2019 (COVID-19) is fatal and spreading rapidly. Early detection and diagnosis of the COVID-19 infection will prevent rapid spread. This study aims to automatically detect COVID-19 through a chest computed tomography (CT) dataset. The standard models for automatic COVID-19 detection using raw chest CT images are presented. This study uses convolutional neural network (CNN), Zeiler and Fergus network (ZFNet), and dense convolutional network-121 (DenseNet121) architectures of deep convolutional neural network models. The proposed models are presented to provide accurate diagnosis for binary classification. The datasets were obtained from a public database. This retrospective study included 757 chest CT images (360 confirmed COVID-19 and 397 non-COVID-19 chest CT images). The algorithms were coded using the Python programming language. The performance metrics used were accuracy, precision, recall, F1-score, and ROC-AUC. Comparative analyses are presented between the three models by considering hyper-parameter factors to find the best model. We obtained the best performance, with an accuracy of 94,7%, a recall of 90%, a precision of 100%, and an F1-score of 94,7% from the CNN model. As a result, the CNN algorithm is more accurate and precise than the ZFNet and DenseNet121 models. This study can present a second point of view to medical staff.

Methods: A total of 120 elderly were included in this cross sectional study. Elderly were visited in their homes and nutritional status of them was evaluated by Mini Nutritional Assessment-Short Form (MNA-SF) scale. In questionnaire; general information about elderly, presence of chronic disease, pressure sores, anthropometric measurement, nutritional status of elderly were asked. In addition, current biochemical parameters which were routinely evaluated in elderly were recorded by the researcher. Results: The mean age of elderly (40 males and 80 females) was 81.3±8.3 (65-105) years, and 97.5% of elderly were diagnosed with at least one disease. According to the MNA-SF score, 60.8% of elderly were classified as malnourished, 34.2% at risk for malnutrition and 5.0% as normal nutritional status. Currently, 49.2% of malnourished elderly have pressure sores and 54.8% had at least once before; these rates were higher than elderly without malnutrition (p <0.001). In addition, total MNA-SF score of elderly is strongly positive with hemoglobin, hematocrit and calf circumference; it was found that there was a strong negative correlation with C-reactive protein (CRP). Conclusion: In this study, it was found that only 5.0% of elderly who received home health care were in good nutritional status and pressure sore was higher in elderly who have risk of malnutrition or malnourished. Nutritional status of elderly receiving home health care should be evaluated by dietitians and more comprehensive scientific studies are needed.

Ozcan, Handan; Elkoca, Ayse; Yigitbas, Cagla; Kartal, Yasemin Aydin

Background: This study intends to address the COVID-19 pandemic process with specific regard to pregnant women, aiming to determine their state-trait anxiety levels. Subjects and methods: The study has a quantitative design. Ethical permissions were obtained, data were collected digitally, and power analysis was performed for sample size. The sample size was calculated as 656 pregnant women. In the collection of the data, the Personal Information Form, and the State-Trait Anxiety Inventory (STAI) were used. Results: In our study, the majority of pregnant women (86.3%) were concerned about the pandemic process, with more than half thinking that their pregnancy (62.9%) and delivery processes would be affected (66.2%). In the meantime, 60.2% of the pregnant women postponed their health checks and they indicated being most concerned about their babies (57.1%). In 51.2% of the pregnant women, the state anxiety levels were low, and 89.8% of the trait anxiety levels were moderate. Conclusions: In crisis periods like the pandemic, pregnant women are among the priority groups. Screening, prevention, early diagnosis, and treatment of affective disorders in the prenatal period are important for maternal and fetal health.
