Sen Demirdogen, Ezgi; Algedik, Pinar; Warikoo, Nishchint; Bahadir, Erdi; Akbiyik, Meral; Büyüktarakçi, Seda; Kadak, Muhammed Tayyip; Yavuz, Mesut

Healthcare workers (HCWs) providing medical support while facing one of the highest levels of adverse and potentially fatal outcomes due to COVID-19 are put in a vulnerable position leading to the development of mental health problems. The development of any prevention and intervention programs to reduce this risk is possible with better understanding and knowledge of possible vulnerability factors. The aim of the present study is to investigate psychological effect of working directly with patients infected with COVID-19 (WD) and possible individual vulnerability factors for the development of psychological problems in HCWs. The data used in this cross-sectional study were collected using online self-reported questionnaires from 290 HCWs aged 21-61 years old. The mean score of the scales of 145 HCWs-WD and 145 HCWs not WD (HCWs-NWD) were compared by independent sample t test. Associations between childhood traumas, metacognitive dysfunctional beliefs and internalizing symptoms were analyzed using structural equation modelling (SEM). The depression, anxiety, stress symptoms levels, somatization and sleep problem levels were found to be higher in HCWs-WD compared to HCWs-NWD. SEM revealed that childhood trauma levels was associated with the increased risk of internalizing problems, and metacognitive dysfunctional beliefs had a partial mediator role between childhood traumas and internalizing symptoms in HCWs-WD. Improving metacognitive abilities may hence need to be considered in prevention programs for the HCWs. The findings can also be used to set up further research on the specific interventions on the HCWs who are at a risk as their profession entails them being in such traumatic situations.

Upper extremity arterial thromboembolism in a patient with severe COVID-19 pneumonia: A case report

Bilge, Aysegul; Karasoy, Ismail; Neziroglu, Elif; Güner, Yesim

Although novel coronavirus-2019 (COVID-19) primarily affects the respiratory system, it can affect multiple organ systems, leading to serious complications, such as acute respiratory distress syndrome (ARDS) and multiple organ failure. Nearly 20 to 55% of patients with COVID-19 experience coagulation disorders that cause high mortality in line with the severity of the clinical picture. Thromboembolism can be observed in both venous and arterial systems. The vast majority of thromboembolic events are associated with the venous system and are often observed as pulmonary embolism. Arterial thromboembolisms often involve the arteries in the lower extremities, followed by those in the upper extremities. Herein, we report a rare case of COVID-19 pneumonia whose left arm was amputated at the forearm level after arterial thromboembolism in the left upper extremity. This case report is valuable, as it is the first reported case of upper extremity arterial thromboembolism in Turkey, as well as the only case in the literature in which the patient underwent four surgical interventions and is still alive.

Diagnosis of COVID-19 and non-COVID-19 patients by classifying only a single cough sound

    Melek, Mesut

In the last month of 2019, a new virus emerged in China, spreading rapidly and affecting the whole world. This virus, which is called corona, is the most contagious type of virus that humanity has ever encountered. The virus has caused a huge crisis worldwide as it leads to severe infections and eventually death in humans. On March 11, 2020, it was announced by the World Health Organization that a COVID-19 outbreak has occurred. Computer-aided digital technologies, which eliminate many problems and provide convenience in people's lives, did not leave humanity alone in this regard and rushed to provide a solution for this unfortunate event. One of the important aspects in which computer-aided digital technologies can be effective is the diagnosis of the disease. Reverse transcription-polymerase chain reaction (RT-PCR), which is a standard and precise technique for diagnosing the disease, is an expensive and time-consuming method. Moreover, its availability is not the same all over the world. For this reason, it can be very attractive and important to distinguish the COVID-19 disease from a cold or flu through a cough sound analysis via smartphones which have entered into the lives of many people in recent years. In this study, we proposed a machine learning-based system to distinguish patients with COVID-19 from non-COVID-19 patients by analyzing only a single cough sound. Two different data sets were used, one accessible for the public and the other available on request. After combining the data sets, the features were obtained from the cough sounds using the mel-frequency cepstral coefficients (MFCCs) method, and then, they were classified with seven different machine learning classifiers. To determine the optimum values of hyperparameters for MFCCs and classifiers, the leave-one-out cross-validation (LOO-CV) strategy was implemented. Based on the results, the k-nearest neighbors classifier based on the Euclidean distance (kNN Euclidean) with the accuracy rate, sensitivity of COVID-19, sensitivity of non-COVID-19, F-measure, and area under the ROC curve (AUC) of 0.9833, 1.0000, 0.9720, 0.9799, and 0.9860, respectively, is more successful than other classifiers. Finally, the best and most effective features were determined for each classifier using the sequential forward selection (SFS) method. According to the results, the proposed system is excellent compared with similar studies in the literature and can be easily used in smartphones and facilitate the diagnosis of COVID-19 patients. In addition, since the used data set includes reflex and unconscious coughs, the results showed that conscious or unconscious coughing has no effect on the diagnosis of COVID-19 patients based on the cough sound.

    Ünlü, Ramazan; Namli, Ersin

From late 2019 to the present day, the coronavirus outbreak tragically affected the whole world and killed tens of thousands of people. Many countries have taken very stringent measures to alleviate the effects of the coronavirus disease 2019 (COVID-19) and are still being implemented. In this study, various machine learning techniques are implemented to predict possible confirmed cases and mortality numbers for the future. According to these models, we have tried to shed light on the future in terms of possible measures to be taken or updating the current measures. Support Vector Machines (SVM), Holt-Winters, Prophet, and Long-Short Term Memory (LSTM) forecasting models are applied to the novel COVID-19 dataset. According to the results, the Prophet model gives the lowest Root Mean Squared Error (RMSE) score compared to the other three models. Besides, according to this model, a projection for the future COVID-19 predictions of Turkey has been drawn and aimed to shape the current measures against the coronavirus.

The economic and fiscal impacts of the Covid-19 pandemic in Turkey and around the world

    Şahin, Muhammet

Humanity has a long history of struggle against pandemics. Covid-19 is the most recent example of such kind of pandemics. Since its first outbreak appeared in China on December 31, 2019, the Covid-19 pandemic has spread rapidly across almost all over the globe within a few months. The Covid-19 has been a type of pandemic which requires compliance with social distancing rules, thereby it has caused both demand-side and supply-side slowdown in economic activities. It is estimated that as a result of the pandemics there will be a very sharp economic shrinkage, an increase in unemployment to the highest levels in the history and a significant decrease in global trade. Governments and international organizations have adopted various measures and incentive packages in order to overcome this problem. These packages mainly include monetary and fiscal tools; however, they have the risk of yielding an increase in government debts thus disrupting public fiscal equilibrium. This chapter aims to examine the economic and fiscal impacts of the pandemic and to examine practices both in Turkey and around the world accordingly.
