Do early procalcitonine levels aid in predicting mortality in burn patients?

Piroglu, Isilay Demir; Tulgar, Serkan; Piroglu, Mustafa Devrim; Thomas, David Terence; Karakilic, Evvah; Gergerli, Ruken; Ates, Nagihan Gozde; Demir, Ahmet

Aim: Burn patients in intensive care units (ICU) are at high risk of mortality. Our aim in this stuy is to evaluate the use of first 48 hour procalcitonin level (PCT), C-reactive protein (CRP) and APACHE II score for the prediction of mortality in burn patients admitted to ICU. Material and method: Files of patients with burns admitted to a tertiary centre's burn unit were retrospectively analysed and those with procalcitonin and C-reactive protein level measurements and APACHE II scores within first 48 hours of admittance were included in this study. Patients with comorbidities that would effect PCT and CRP levels such as chronic renal or liver failure were excluded from the study. Patients PCT, CRP and APACHE II scores were compared with the outcome of patients. Results: Seventy patients were included in this study. While CRP levels were lower in patients that were deceased, PCT levels were significantly higher in these patients. In patients with APACHE II score > 20 and PCT > 2 ng/ml, mortality was statistically very significantly higher compared with other patients. Logistical regression analysis showed that high PCT levels and APACHE II scores were important at predicting mortality in these patients. Conclusion: High PCT levels within 48 hours of burns is a predictor of mortality in these patients. Additional studies are required to correctly determine the cut-off value. A modified scoring system including APACHE II and PCT may be useful for better predicting mortality, although larger multi-centered studies are required.

Permo-Carboniferous granitoids with Jurassic high temperature metamorphism in Central Pontides, Northern Turkey

Gücer, Mehmet Ali; Arslan, Mehmet; Sherlock, Sarah; Heaman, Larry M.

In the northern part of the Central Pontides (N Turkey) there are different metamorphic rocks exposed, notably the Devrekani metamorphic rocks. Here, upper amphibolite-lower granulite facies metamorphic rocks contain predominantly paragneiss, orthogneiss and metacarbonate, and to a lesser extent, amphibolite and quartzite, with cross-cutting aplite, pegmatite and granite veins. This is the first report of these rocks and includes new data on the petrochemistry, geochronology and metamorphic evolution of the Devrekani orthogneisses from the Central Pontides. The orthogneisses show five different mineral parageneses with the characteristic mineral assemblage quartz + K-feldspar + plagioclase + biotite +/- hornblende +/- opaque (+/- ilmenite and +/- magnetite), and accessory minerals (zircon, sphene and apatite). These metamorphic rocks exhibit generally granoblastic, lepidogranoblastic and nematolepidogranoblastic with locally migmatitic and relic micrographic textures. They have well-developed centimeter-spaced gneissic banding and display gneissose structure with symmetric, asymmetric and irregular folds. The petrographic features, mineralogical assemblages and weak migmatization reflect high temperature conditions. Thermometric calculations in the orthogneisses indicate metamorphic temperatures reached 744 +/- 33 A degrees C. Field relations, petrography and petrochemistry suggest that the orthogneisses have predominantly granodioritic and some granitic protoliths, that show features of I-type, medium to high-potassic calc-alkaline volcanic arc granitoids. The orthogneisses have high contents of LILEs and low contents of HFSEs with negative Nb and Ti anomalies, which are typical of subduction-related magmas. The orthogneisses also show significant LREE enrichment relative to HREE with negative Eu anomalies (Eu-N/Eu* = 0.33-1.07) with La-N/Lu-N = 6.98-20.47 values. Based on U-Pb zircon dating data, the protoliths are related to Permo-Carboniferous (316-252 Ma) magmatism. It is likely that peak metamorphism took place during the Jurassic as reflected by the U-Pb zircon ages (199-158 Ma) and also 40Ar/39Ar from hornblende/biotite (163-152 Ma). The four biotite 40Ar/39Ar average ages from the rock samples are ca. 156 Ma, suggesting that the metamorphic rocks cooled to 350-400 A degrees C at ca. 156 Ma. Conclusively, the Devrekani metamorphic rocks can be ascribed as products of Permo-Carboniferous continental arc magmatism overprinted by Jurassic metamorphism in the northern Central Pontides.

Cenozoic forearc gabbros from the northern zone of the Eastern Pontides Orogenic Belt, NE Turkey: Implications for slab window magmatism and convergent margin tectonics

Eyuboglu, Yener; Dudas, Francis O.; Santosh, M.; Zhu, Di-Cheng; Yi, Keewook; Chatterjee, Nilanjan; Jeong, Youn-Joong; Akaryali, Enver; Liu, Ze

The Eastern Pontides Orogenic Belt represents one of the best examples of fossil convergent margins in the eastern Mediterranean region. However, the origin and geodynamic setting of the late Mesozoic-Cenozoic magmatism in this belt remain controversial due to lack of systematic geological, geochemical and chronological data. The general consensus is that the late Mesozoic-Cenozoic igneous activity is related to northward subduction of oceanic lithosphere in the late Mesozoic and following collision between Tauride and Pontide blocks in the early Cenozoic Here we present a comprehensive study focusing on the origin and geodynamic setting of gabbro bodies exposed along a narrow zone, parallel to the southeastern coast of the eastern Black Sea basin, in the Northern Zone of the Eastern Pontides Orogenic Belt. The studied gabbro bodies are hosted within late Cretaceous basaltic, andesitic, and dacitic volcanics including pyroclastic rocks and interbedded sedimentary rocks. The gabbro bodies range in size from 0.1 km(2) to 1.5 km(2), and outcrop patterns vary from round or elliptical to markedly elongate with sharp and discordant contact with the host rocks. Their mineral assemblage includes mainly clinopyroxene, plagioclase, minor olivine, amphibole, magnetite and rarely orthopyroxene, biotite, zircon and titanite. The occurrence of sutured grain boundaries on clinopyroxene and plagioclase, and the presence of reverse compositional zoning in clinopyroxene and olivine suggest mixing between magmas of contrasting compositions during mineral growth. Thermobarometric computations indicate that the temperature at the beginning of crystallization was similar to 1250 degrees C and crystallization was polybaric Zircon and titanite U-Pb ages indicate that these small intrusions were emplaced into crustal rocks of the Eastern Pontides Orogenic Belt during Lutetian (45 +/- 2 Ma). The depletion of HFSE is consistent with the involvement of an arc-related source in the petrogenesis of these rocks, and low to moderate enrichment Ce, Rb, Ba, K, Pb, Sr and Th suggests that involvement of subducted oceanic sediment was modest. The low Th content and low Th/Yb indicate that the role of sediment addition was nevertheless limited. The Nd, Sr and Pb isotopic data are consistent with the interpretation that the dominant source component in these gabbros is a depleted, peridotitic mantle, and that crustal contamination is relatively unimportant. We suggest that mafic magmas that produced the gabbroic intrusions were derived from melting of a depleted mantle source under the forearc region of the Eastern Pontides Orogenic Belt during southward subduction of two oceanic plates separated by a mid-ocean ridge, leading to the formation of a slab window. We also infer fractional crystallization and assimilation during both magma storage in the crust-mantle transition zone and transfer into the overlying arc crust. Aydin, Sinan; Akkan, Yasar; Arpaz, Ercan; Koparan, Bulent; Iaman, A; Eskicumali, A

In this study, we describe vocational school students' practices and perceptions of online learning based on interviews and observations. First, we identify student characteristics within online classes. Then, we report student perceptions of online lectures. Finally, we summarize our recommendations to improve the online teaching. The methodology used in this study was qualitative data-collection techniques to obtain vocational school student views on online education and educational processes. For this purpose, two stages of data collection were used in this study these were one-on-one open-ended interviews and think-aloud observation. According to data analysis from this study, communication between students and teachers has a basic effect on shaping students' perceptions and approach to online learning. Also, they have some negative ideas of using and implementations of technology in lectures. Included in this finding is that the meaning assigned by students to any technological material used in lectures is directly connected with the way this material is implemented. Another important finding is that planning of course, assessment and curriculum plays basic role in student's concentrate on lectures and success in online learning. We investigated only a few of the possible relationships between perception of the online environment, the technological materials used in lectures, students' approach to online learning, and students' perceptions of this learning way. Although Web resources and community were mentioned in this study, they may play a more effective role in student online learning than was analyzed in this study. Some additional factors that may effect student perceptions needed to be investigated are the link between perception and outcome, faculty use and knowledge, faculty perceptions and training. DYNAMIC DISPLACEMENT AND STRESS ANALYSES OF CANTILEVER RETAINING WALLS CONSIDERING SOIL-STRUCTURE INTERACTION AND WALL FLEXIBILITY

    Çakir, Tufan; Dag, Serhat

The Mononobe-Okabe (M-O) method based on limit equilibrium principle is widely used in the design of retaining structures against earthquake loads. However, this method does not consider the soil-structure interaction and wave propagation effects. On the other hand, it is known that the interactions of retaining structures with backfill and foundation soils transform the seismic behavior of these structures to a rather complex problem. The main purpose of this study is to evaluate the soil-structure interaction and wall flexibility effects on the seismic behavior of backfill-cantilever retaining wall-foundation/soil systems. In line with this target, three dimensional finite element models were utilized for analyzing the aforementioned systems. Seismic analyses were carried out in the time domain by considering five different foundation soil conditions and two different wall thicknesses. The results were comparatively interpreted in terms of lateral wall displacements and the stresses occurring at critical sections of the wall. It was observed that these responses may vary significantly due to soil-structure interaction and wall flexibility effects.

The Port of Samsun at the Beginning of the 20th Century According to French Archival Documents

    Yilmaz, Ozgur

Due to its geographic location and agricultural potential, Samsun began to develop rapidly and became a new trade center in the middle of the 19th century. A factor was the Crimean War that created new opportunities for the southern ports of the Black Sea. But the real and most important factor was the European demand for Ottoman agricultural products in the 19th century. The rise of Samsun as a gateway for wide region like Merzifon, Amasya, Tokat, Sivas, Yozgat, Sebinkarahisar, Kayseri, Kirsehir and Diyarbakir resulted the steady increase in agricultural production supported by this foreign demand. In this way, at the end of the century Samsun became the primary port in the southern Black Sea. As a reflection of this international interest, foreign consulates established in the city. French consulate, established in 1863 was one of them. Annual commercial and maritime reports in detail prepared by consuls for each year are among the most important sources for the economic structures in the 19th century Ottoman cities. There are very valuable documents on trade in the French archives about Samsun that attracting more attention especially in the second half of the century. It is possible to observe the rise of the city from annual commercial reports prepared by French consular agents. The reports of Henri de Cortanze who served in the city between 1895 and 1906 reflect the situation of Samsun at the end of the 19th and the beginning of the 20th century. In this context, this study aims to investigate Cortanze's detailed report on the port of Samsun in 1903. By using other reports about Cortanze's consular period, the subject is evaluated in a broad sense.

Finite Element Based Investigation of Backfill Effects on Seismic Behavior of a Cantilever Wall

Cakir, Tufan; Drusa, M; Marschalko, M; Yilmaz, I; Segallini, A; Ferrero, AM; Bednarik, M

A series of finite element analyses were conducted to investigate the backfill effects on seismic behavior of a cantilever retaining wall incorporating backfill and subsoil interactions. Considering four different backfill soil conditions, the dynamic analyses of backfill-cantilever wall-soil/foundation system were carried out in time domain through ANSYS general purpose finite element computer program. The cohesionless soil was modelled as an elasto-plastic medium with viscous boundary applied on the artificial boundary of soil to simulate the wave energy absorption. Backfill-cantilever wall interface behavior was taken into account by using interface elements between the wall and soil to allow for de-bonding. The magnitudes of lateral displacements and stresses were determined. The obtained results show that the earthquake behavior of cantilever retaining walls can change considerably due to the backfill interaction. Thus, the backfill interaction should be considered in design criteria of cantilever walls. (C) 2015 The Authors. Published by Elsevier B.V.

The Possibility of a Democracy Based on the Constituent Power: A Political Approach

    Akkanat, Salih

The article aims to bring a political dimension to the problem of constituent power, which is usually discussed in terms of constitution techniques. It argues that some approaches reducing the problem of constituent power to the methodological pre-commitments or the articulations of abstract democratic principals presuppose de-ontological and apolitical premise. In other words, it focuses on the apologetic attitudes of these approaches isolating the modern content of constituent power if we consider its meaning as the creation of new political forms. In conclusion, the article points to the fact that any understanding of constituent power, which ignores its modern particularizy, may result in a poor understanding of democracy

Background: Bipolar disorder (BD) is a highly heritable mental illness which is associated with neuroanatomical abnormalities. Investigating healthy individuals at high genetic risk for bipolar disorder may help to identify neuroanatomical markers of risk and resilience without the confounding effects of burden of illness or medication. Methods: Structural magnetic resonance imaging scans were acquired from 30 euthymic patients with BD-1 (BP), 28 healthy first degree relatives of BD-I patients (HR), and 30 healthy controls (HC). Data was analyzed using DARTEL for voxel based morphometry in SPM8. Results: Whole-brain analysis revealed a significant main effect of group in the gray matter volume in bilateral inferior frontal gyrus, left parahippocampal gyrus, left lingual gyrus and cerebellum, posterior cingulate gyrus, and supramarginal gyrus (alphasim corrected (<= 0.05 FWE)). Post-hoc t-tests showed that inferior frontal gyrus volumes were bilaterally larger both in BP and HR than in HC. BP and HR also had smaller cerebellar volume compared with HC. In addition, BP had smaller left lingual gyms volume, whereas HR had larger left parahippocampal and supramarginal gyms volume compared with HC. Limitations: This study was cross-sectional and the sample size was not large. All bipolar patients were on medication, therefore we were not able to exclude medication effects in bipolar group in this study. Conclusions: Our findings suggest that increased inferior frontal gyms and decreased cerebellar volumes might be associated with genetic predisposition for bipolar disorder. Longitudinal studies are needed to better understand the predictive and prognostic value of structural changes in these regions. (C) 2015 Elsevier B.V. All rights reserved.

Evaluation of the response to treatment of psoriasis vulgaris with reflectance confocal microscopy

    Basaran, Y. K.; Gürel, M. S.; Erdemir, A. T.; Turan, E.; Yurt, N.; Bagci, I. S.

Background/objectiveReflectance confocal microscopy (RCM) is a noninvasive, objective imaging technique that provides in vivo, high-resolution skin imaging. We sought to assess epidermal and dermal changes associated with the psoriasis and its treatment with RCM before the treatment and at weeks 4 and 8 of the treatment. Material and methodsThis is an investigator-blinded, internal-controlled, follow-up study. A total of 25 patients with plaque psoriasis were included in the study. The RCM evaluation criteria were defined on the basis of the histopathological diagnostic criteria for psoriasis. The clinical severity of the psoriasis was evaluated using the Psoriasis Area Severity Index (PASI). ResultsThe RCM findings which were correlated with the PASI can be used to follow up the patient's response to treatment have been identified as follows: the acanthosis, the number of spongiotic sites, the number of regular/irregular honeycomb-like sites, the number of epidermal inflammatory cells, the number of focal microabscesses, the total epidermal thickness, the number of nonedge dermal papillae, the length of the papillary dermis, the number of dermal inflammatory cells, and the vascularization in the papillary dermis (P<0.05). ConclusionThis is the first study with a large group of patients to perform a noninvasive assessment with RCM of the response of psoriasis to different treatments: phototherapy, systemic and topical treatment. Micrometric and morphometric changes occurring in the psoriatic tissue during the 8-week treatment period were identified by in vivo RCM in a noninvasive manner. RCM is capable of monitoring of treatment response in psoriasis.

RESONANT VIBRATIONS OF TWO-SPAN RAILWAY BRIDGES UNDER HIGH-SPEED TRAINS

Kahya, Volkan; Araz, Onur; Turan, Muhittin

In this study, resonant vibrations of two-span railway bridges subjected to high-speed trains (HSTs) are studied. The continuous beam is modelled by using Bernoulli-Euler beam theory, and the train is considered as a series of moving concentrated loads. The dynamic response is obtained analytically by using the assumed mode method. Effects of the speed parameter and the span to car length ratio on the response are examined. Numerical results show that the above mentioned parameters play very important role on the dynamic behavior of two-span bridges.

Nutritive Value of Sunflower Silages Ensiled with Corn or Alfalfa at Different Rate

Tan, Mustafa; Yolcu, Halil; Dumlu Gul, Zeynep

Sunflower (Helianthus annuus L.) having shorter growing season, better cold tolerant and more drought resistant is important alternative silage plant in highlands. But it has poor silage quality. To improve the quality of sunflower silage can be mixed an appropriate amount of corn (Zea mays L.) and alfalfa (Medicago sativa L.). This research was carried out to evaluate quality characteristics of nine silage types: Sunflower, corn and alfalfa silages; plus their some mixtures silages (75% sunflower + 25% corn, 50% sunflower + 50% corn, 25% sunflower + 75% corn, 75% sunflower + 25% alfalfa, 50% sunflower + 50% alfalfa and 25% sunflower + 75% alfalfa); at two harvest stages: Beginning of flowering for sunflower and alfalfa and milk-dough stage for corn (early stage), and end of flowering stage for sunflower and alfalfa, and dough stage for corn (late stage). The experimental design was in a factorial arrangement of randomized complete block with replicated four times. Dry matter content (DM), crude protein content (CP), neutral detergent fiber content (NDF), acid detergent fiber content (ADF), silage pH and physical characteristics (PC) of silages were determined in this study. Mixing of alfalfa in NDF, ADF and CP and corn in DM, pH and PC had positively effect in sunflower silages. Late stage had positively affected DM, PC and pH for silage. Consequently, sunflower silages mixed with corn, with the condition of not being less than 50% at late stage, are recommended for farmers.

Improvement of Palmprint Coding Techniques with Feature Descriptors

    Bingöl, Ozkan; Ekinci, Murat

In recent years, biometric palmprint recognition system is one of the preferred research areas due to reach high accuracy even with the low resolution cameras. In recent studies conducted in this area, especially due to hygiene problems, images are taken in contactless and unrestricted environment. Despite a very successful improving methods have been developed for the images obtained in the restricted environment, shifts or projective distortions that occur in the palm pattern obtained from images taken in unconstrained environments negatively affect the success of this method. In this study, coding techniques that are commonly used palmprint recognition systems (Competitive Code, Ordinal Code, Derivative of Gaussian Filters Code) are strengthened with feature determinants (SIFT, SURF) and a new approach has been proposed. Scope of the study, a new data set consists of low-resolution images taken in unconstrained environments created and palmprint patterns obtained with the help of Active Appearance Model. First, performance rate of these coding techniques on the patterns were analyzed. Then, the advantages of 6 different approaches obtained by combining each feature determinant with each coding technique, is disclosed to the system.
