- MakaleYalnız künye2015Scopus SJRQ1Evaluation of the response to treatment of psoriasis vulgaris with reflectance confocal microscopy
Basaran, Y. K.; Gürel, M. S.; Erdemir, A. T.; Turan, E.; Yurt, N.; Bagci, I. S.
Background/objectiveReflectance confocal microscopy (RCM) is a noninvasive, objective imaging technique that provides in vivo, high-resolution skin imaging. We sought to assess epidermal and dermal changes associated with the psoriasis and its treatment with RCM before the treatment and at weeks 4 and 8 of the treatment. Material and methodsThis is an investigator-blinded, internal-controlled, follow-up study. A total of 25 patients with plaque psoriasis were included in the study. The RCM evaluation criteria were defined on the basis of the histopathological diagnostic criteria for psoriasis. The clinical severity of the psoriasis was evaluated using the Psoriasis Area Severity Index (PASI). ResultsThe RCM findings which were correlated with the PASI can be used to follow up the patient's response to treatment have been identified as follows: the acanthosis, the number of spongiotic sites, the number of regular/irregular honeycomb-like sites, the number of epidermal inflammatory cells, the number of focal microabscesses, the total epidermal thickness, the number of nonedge dermal papillae, the length of the papillary dermis, the number of dermal inflammatory cells, and the vascularization in the papillary dermis (P<0.05). ConclusionThis is the first study with a large group of patients to perform a noninvasive assessment with RCM of the response of psoriasis to different treatments: phototherapy, systemic and topical treatment. Micrometric and morphometric changes occurring in the psoriatic tissue during the 8-week treatment period were identified by in vivo RCM in a noninvasive manner. RCM is capable of monitoring of treatment response in psoriasis.

- MakaleYalnız künye2025Scopus SJRQ3Pan-immune-inflammation value predicts sustained ventricular arrhythmias in NSTEMI: a novel inflammatory risk marker
Sahin, Yusuf Bozkurt; Tanik, Veysel Ozan; Koçu, Gokturk Kaan; Özkan, Mehmet Taha; Akdogan, Murat; Seçkin, Ozden; Tunca, Cagatay; Sezgin, Ali; Akbuga, Kursat; Özlek, Bulent
BackgroundSystemic inflammation contributes to arrhythmogenesis in acute coronary syndromes, but its role in NSTEMI-related ventricular arrhythmias remains poorly defined. The pan-immune-inflammation value (PIV) is a novel composite biomarker reflecting immune and thrombotic activity derived from routine blood counts. This study aimed to assess the association between PIV and the risk of sustained VT/VF in NSTEMI.MethodsIn this retrospective cohort study, 1,788 NSTEMI patients who underwent percutaneous coronary intervention were analyzed. The primary endpoint was the occurrence of sustained VT or VF during hospitalization. PIV was calculated as (neutrophil x platelet x monocyte)/lymphocyte. Logistic regression, ROC analysis, Kaplan - Meier curves, reclassification indices (NRI, IDI), and restricted cubic spline modeling were used.ResultsVT/VF occurred in 34 patients (1.9%). Those with VT/VF had significantly higher PIV values (median 1132 vs. 329, p < 0.001). In multivariable analysis, PIV remained an independent predictor (OR: 1.356 per 1000 unit increase; 95% CI: 1.028-1.787; p = 0.031). The risk of VT/VF rose progressively across PIV quartiles (Q4 vs. Q1 OR: 4.0, p for trend < 0.001). Adding PIV to conventional predictors improved risk classification (NRI: +0.089; IDI: 0.007). Kaplan - Meier analysis showed significantly reduced arrhythmia-free survival in high-PIV tertiles (log-rank p < 0.001), and cubic spline modeling revealed a non-linear association with increased arrhythmic risk at higher PIV levels.ConclusionsElevated PIV is independently associated with malignant ventricular arrhythmias in NSTEMI. As an accessible biomarker, PIV may aid early arrhythmic risk stratification and guide clinical surveillance strategies.

Forage DMY and SY increased over the first 4 yr, but the yields decreased due to the aging of the crested wheatgrass stand in the subsequent year. There were significant differences for lines in all variables, except for SWY and CPC. Particularly, lines AC1, AC23, AC29, AC37, AC39, AC65, AC66, AC107, and AC121 in DMY; AC1, AC37, AC83, AC107, AC113, AC121, and AC125 in SY; and AC23, AC39, and AC113 in CPY showed significant improvement compared with the original parent material. The results of DMY, SY, and CPY show that selection studies were effective. Selected lines will be useful for breeding studies and livestock production.

Background - Mean platelet volume (MPV) is an indicator of platelet activation. The pathophysiology of the primary and secondary Raynaud's Phenomenon (RP) have not been completely established. The aim of this study was to investigate the relationship between MPV and RP. Materials and Methods - Our study was a prospective randomized study carried out from January 2011 to March 2012. The study group consisted of 39 patients: 27 (70%) patients having primary, 12 (30%) patients having secondary RP. An age-, gender-, and body mass index-matched control group consisted of 40 healthy participants. We compared the MPV in patients with RP and control participants statistically. Results - MPV of RP group was 8.79 +/- 1.37 femtoliter (fL) while MPV of control group was 8.39 +/- 1.36 fL. Comparison of MPV of RP group and control group showed no difference (p = 0.274). The mean of MPV was significantly higher among patients with secondary RP (9.76 +/- 1.68 fl) when compared with patients with primary RP (8.37 +/- 0.96 fl) (p = 0.018). Conclusion - The results of our study suggest that MPV may be used as a marker in secondary RP.
- MakaleYalnız künye2016Scopus SJRQ1Abnormal white matter integrity as a structural endophenotype for bipolar disorder
Sariçiçek, A.; Zorlu, N.; Yalin, N.; Hidiroglu, C.; Çavusoglu, B.; Ceylan, D.; Ada, E.; Tunca, Z.; Özerdem, A.
Background. Several lines of evidence suggest that bipolar disorder (BD) is associated with white matter (WM) pathology. Investigation of unaffected first-degree relatives of BD patients may help to distinguish structural biomarkers of genetic risk without the confounding effects of burden of illness, medication or clinical state. In the present study, we applied tract-based spatial statistics to study WM changes in patients with BD, unaffected siblings and controls. Method. A total of 27 euthymic patients with BD type I, 20 unaffected siblings of bipolar patients and 29 healthy controls who did not have any current or past diagnosis of Axis I psychiatric disorders were enrolled in the study. Results. Fractional anisotropy (FA) was significantly lower in BD patients than in the control group in the corpus callosum, fornix, bilateral superior longitudinal fasciculus, inferior longitudinal fasciculus, inferior fronto-occipital fasciculus, anterior thalamic radiation, posterior thalamic radiation, cingulum, uncinate fasciculus, superior corona radiata, anterior corona radiata and left external capsule. In region-of-interest (ROI) analyses, we found that both unaffected siblings and bipolar patients had significantly reduced FA in the left posterior thalamic radiation, the left sagittal stratum, and the fornix compared with healthy controls. Average FA for unaffected siblings was intermediate between the healthy controls and bipolar patients within these ROIs. Conclusions. Decreased FA in the fornix, left posterior thalamic radiation and left sagittal stratum in both bipolar patients and unaffected siblings may represent a potential structural endophenotype or a trait-based marker for BD.

- MakaleYalnız künye2017Scopus SJRQ1Late Jurassic Magmatism and Stratigraphy in the Eastern Sakarya Zone, Turkey: Evidence for the Slab Breakoff of Paleotethyan Oceanic Lithosphere
Dokuz, Abdurrahman; Aydinçakir, Emre; Kandemir, Raif; Karsli, Orhan; Siebel, Wolfgang; Derman, Ahmet Sami; Turan, Mehmet
Middle-Late Jurassic Cimmerian events in Turkey have been actively discussed in the past three decades, but proposed tectonic models associated with magmatism, metamorphism, and stratigraphic features remain controversial. To address this issue, Upper Jurassic mafic lavas are investigated at three locations (Alucra, Gumushane, and Olur) in the eastern Sakarya Zone, northeastern Turkey. These lavas are submarine and form planar flows parallel with the bedding plane in the Upper Jurassic carbonate sequence near the base or just below in the clastic sedimentary rocks. The basaltic lavas show calc-alkaline features and possess Nb-Ta values and Nb/U, Nb/La, and Ce/Pb ratios that are greater than those of island arc basalts. Multielement patterns are almost hump shaped, similar to ocean island basalts, which experience Pb depletion and weak negative Nb-Ta, Zr-Hf, and Ti anomalies. The low initial (Sr-87/Sr-86) ratios (0.70372-0.70554), positive initial epsilon(Nd) values (+2.7 to +4.4), and initial Pb isotope ratios that plot between mid-ocean-ridge and ocean island basalts are consistent with a melt derived from subcontinental lithospheric mantle, metasomatized by earlier fluids from subducted sediments and plume materials from the asthenosphere. Moderate Dy/Yb ratios with an average value of 1.8 imply partial melting in the spinel-garnet transition zone at depths of similar to 70-100 km. Slab breakoff is suggested as a geodynamic mechanism that accounts for these geochemical signatures. This inference is also favored by stratigraphic and sedimentologic evidence from the Upper Jurassic-Lower Cretaceous sedimentary rocks, which is consistent with short-lived vertical (epirogenic) movements in the region. Lower-Middle Jurassic sequences are transgressive, suggesting that subduction-related extension opened a backarc basin (Neotethys) in the south of the Sakarya Zone. Upper Jurassic-Lower Cretaceous carbonates point tectonically to tranquility during carbonate deposition in the Neotethys Ocean, which seems to have been achieved by complete closure of the Paleotethys in the north. About 15-20 m.yr. later (Kimmeridgian), after first carbonate deposition, intraplate-typemafic lavas ascended up to the shelf surface of the Neotethys. This was followed by formation of disconformity surfaces and then accumulation of coarse clastic sediments. All this points to a short-lived epirogenic movement that we ascribe to the breakoff of the southward-subducting Paleotethyan oceanic lithosphere in the Late Jurassic.

Arik, Osman Ziya; Özkan, Bugra; Kutlu, Rasim; Karal, Huseyin; Sahin, Durmus Yildiray; Kaypakli, Onur; Özel, Deniz; Çayil, Murat
Atrial fibrillation (AF) is the most common sustained arrhythmia and associated with adverse outcomes and increased risk for thromboembolic events. Warfarin is still the most extensively prescribed oral anticoagulant in AF to prevent ischemic complications. We aimed to determine the differences at platelet indices with warfarin usage layered by International Normalized Ratio (INR). A total of 250 patients with permanent non-valvular AF (mean age 70.2 +/- 9.1; 153 female) were divided into two groups. Group 1 included 125 patients whose INR is between 2.0 and 3.0 (called as effective'') and Group 2 included 125 patients whose INR is <2.0 (called as ineffective''). Also 123 age- and sex-matched individuals in sinus rhythm enrolled as control group (Group 3). After physical and echocardiographic examination, complete blood counts and INR were studied. There was no statistically significant difference in age, sex, co-morbidities and medications, also hemoglobin, white blood cell and platelet counts among the groups. The CHA(2)DS(2)-VASc scores were similar between Groups 1 and 2. The mean platelet volume (MPV), platelet distribution width (PDW) and plateletcrit (PCT) were significantly higher in Group 2 than Groups 1 and 3 and similar between Groups 1 and 3. MPV was positively correlated with PDW and PCT and also inversely correlated with INR value and platelet count. On multivariate logistic regression analysis, MPV, PDW and PCT were independent predictors of ineffectiveness of INR. The results of this study showed that MPV, PDW and PCT are increased in patients with non-valvular AF without effective warfarin treatment. Warfarin usage adjusted by INR is associated with lower values of these platelet indices, even lower as the values of subjects in sinus rhythm. MPV, PDW and PCT are independent predictors of INR ineffectiveness and seem to be useful parameters for monitoring the effectiveness of warfarin treatment.

- MakaleYalnız künye2020Scopus SJRQ3Right Ventricular Early Inflow-Outflow Index-A new method for echocardiographic evaluation of right ventricle dysfunction in acute pulmonary embolism
Acar, Emrah; Izci, Servet; Inanir, Mehmet; Yilmaz, Mehmet Fatih; Izgi, Ibrahim Akin; Kirma, Cevat
Introduction Acute pulmonary embolism (APE) is a cardiothoracic thromboembolic emergency at risk of life-threatening. Several risk graduation algorithms may be applied to delineate short-term mortality in patients with APE. In this study, we aim to depict the relationship between the right ventricular early inflow-outflow (RVEIO) index, which is a Doppler-based parameter, and the pulmonary embolism severity index(PESI) in acute pulmonary embolism. Methods In the presented study, a total of 160 patients who were diagnosed with APE using pulmonary computed tomography angiography or ventilation/perfusion scintigraphy were comprised. Patients were separated to 2 groups based on the simplified PESI (sPESI): sPESI < 1 (n = 88) and sPESI >= 1 (n = 72). Echocardiographic parameters, including the RVEIO index, were measured. Results There were no significant differences between the groups in age and gender distribution, or the presence of diabetes mellitus, hypertension, smoking, and history of coronary artery disease. There was a positive correlation with the mortality rate and RVEIO index; the mortality was higher in patients with a higher RVEIO index(<0.001). In receiver operating characteristic (ROC) curve analysis using a cutoff level of 14.39, RVEIO index predicted mortality with a sensitivity of 80.4% and specificity of 57.6%(ROC area under curve:0.694; 95%CI, 0.581-0.814; P < .001). RVEIO index was higher in the sPESI >= 1(n:72) than in the patients with sPESI < 1(14.27 +/- 2.13 vs 10.63 +/- 2.09; P < .001). There was a positive correlation between RVEIO index and sPESI score(+0.428; P < .001). Conclusion As well as predicting the degree of tricuspid regurgitation (TR), the RVEIO index is well-correlated with sPESI score and is associated with mortality in patients with APE. This easily measurable parameter may be used to predict short-term mortality in APE patients.

Background and study aims: Esophageal burns due to ingestion of corrosive substances are frequently seen in both children and adults. However, there is no standard method of treatment to prevent associated mortality and morbidity. Therefore, this study aimed to evaluate the effects of known antioxidants, namely N-acetyl cysteine and ethyl pyruvate, on esophageal damage due to sodium hydroxide-induced corrosive burns. Materials and methods: Thirty-five female rats were randomly assigned to five equal groups. Group 1 was the sham group, while Group 2 was the control group. Group 3 received N-acetyl cysteine, Group 4 received ethyl pyruvate, and Group 5 received both N-acetyl cysteine and ethyl pyruvate. Rats in the burn groups were gavage-fed with 0.2mL of 25% NaOH. All esophagi were extracted on day 4 for histopathological evaluation. Results: Total histopathological damage scores were evaluated at the end of the study. Groups 3 and 5 were significantly different from the control group in terms of total histopathological scores (p = 0.001), while no significant difference was seen with Group 4. Stenosis index results in groups 3 and 5 were similar to those seen with total histopathological scores (p = 0.004). Conclusion: N-acetyl cysteine, alone or in combination with ethyl pyruvate, may be useful in the treatment of esophageal damage associated with corrosive substances and in achieving histopathological improvement in an experimental setting. Purpose To evaluate the potential toxicity of operation microscopes with halogen and light emitting diode (LED) light source on the rabbit eyes. Materials and methods Thirty-two eyes of 16 male New Zealand pigmented rabbits were involved in the study. The rabbits were divided into two groups according to the type of light source applied. Only one eye of each rabbit was exposed to illumination light, unexposed fellow eyes served as the control group. Experimental groups included group 1 exposed to halogen light for 2 h and evaluated 1 day and 1 week after the illumination, group 2 exposed to LED light for two hours and evaluated 1 day and 1 week after the illumination. On the first and seventh days after exposing the light, we evaluated the rabbit corneas using in vivo confocal microscopy (IVCM). At the end of the seventh day, the Hematoxylin-eosin staining and TUNEL staining were performed to investigate the presence of apoptosis in the retina and retina pigment epithelium. Results Early IVCM findings revealed corneal epithelial cell ovalization and indistinct intercellular borders in the halogen light group. We also observed more increase in the keratocyte density index (23.7% vs 14.1%, p = 0.001, respectively) and the Bowman reflectivity index (12.4% vs 4.1%, p = 0.001, respectively) at first day of the light exposure in halogen light group compared to LED light group. However, late IVCM indicated that these findings disappeared one week later. No apoptosis was observed in the corneal and retinal layers in early and late examination groups. Conclusion The present experimental study demonstrated that both halogen and LED lights, which were commonly used for microscopic eye surgery, had no sustained adverse effect on the cornea and retina of the rabbits; however, halogen light had a temporary adverse effect on corneal epithelium and stroma, which resolved within 1 week.

Background: This study aims to evaluate the characteristics of patients with traumatic rupture of diaphragm (TDR), the diagnostic and therapeutic approaches used in these cases and and the outcomes of the patients. Methods: Forty-one patients (31 males, 10 females; mean age 41.9 years; range 7 to 71 years) with thoracoabdominal trauma and diagnosis of TDR who were treated in our clinic between January 2000 and January 2010 were analyzed retrospectively. The patients were statistically compared in terms of age, gender, the time of diagnosis (early or late), the diagnostic procedures that were performed, whether the diagnosis of rupture was made during or after the operation, the localization of the rupture, the diameter of the rupture, accompanying injuries, the surgery performed, mortality and morbidity and the trauma injury severity score. Results: The etiology for the diaphragmatic rupture was blunt trauma in 43.9% and penetrating trauma in 56.10% of the patients. 85.36% of the patients were diagnosed in early stage and 14.64% in late stage. Herniation dignosis was made radiologically in 118 (43.9%) patients and diaphragmatic rupture was detected during the operation in 23 patients (56.1%). Twenty-nine (70.73%) patients had thoracotomy, seven (17.02%) patients had laparatomy and five (12.19%) patients had thoracotomy plus laparatomy. The overall mortality rate was 14.63% (n=6). Conclusion: Traumatic diaphragmatic ruptures may have a fatal course depending on the strangulation of the herniating abdominal viscera. Traumatic diaphgram rupture should be suspected in all multitraumatic patients and these cases shuld be seriously evaluated for the definitive diagnosis and treatment plan.

Günaydin, Murat; Adanur, Suleyman; Altunisik, Ahmet Can; Sevim, Baris
In this study, it is aim to perform the construction stage analysis of suspension bridges using time dependent material properties. Fatih Sultan Mehmet Suspension Bridge connecting the Europe and Asia in Istanbul is selected as an example. Finite element models of the bridge are modelled using SAP2000 program considering project drawing. Geometric nonlinearities are taken into consideration in the analysis using P-Delta large displacement criterion. The time dependent material strength variations and geometric variations are included in the analysis. Because of the fact that the bridge has steel structural system, only prestressing steel relaxation is considered as time dependent material properties. The structural behaviour of the bridge at different construction stages has been examined. Two different finite element analyses with and without construction stages are carried out and results are compared with each other. As analyses result, variation of the displacement and internal forces such as bending moment, axial forces and shear forces for bridge deck and towers are given with detail. It is seen that construction stage analysis has remarkable effect on the structural behaviour of the bridge.

Objectives: To detect the causes of shoulder pain with shoulder ultrasonography in patients with carpal tunnel syndrome (CTS) and compare the findings of the patients with CTS to healthy controls. Patients and Methods: Patients with CTS and healthy controls were evaluated clinically, electrophysiologically. and ultrasonographically by three masked researchers. Electrophysiological assessment consisted median and ulnar nerve conduction studies (NCS). In ultrasonographic assessment, cross-sectional areas of the median nerve were measured at hamatum hook, psiform bone, radio-ulnar joint levels. Shoulder joint, axial and longitudinal biceps tendon, subscapularis tendon, supraspinatus tendon, infraspinatus tendon, rotator cuff, acromioclavicular joint were evaluated by a standardized protocol. Results: The study was completed with 30 patients with CTS and 30 healthy subjects. The presence of shoulder pain in the last week was 53.3% in the CTS group and 30% in the control group but there was no statistically significant difference between the groups (p=0.16; p>0.05). Subdeltoid bursitis was significantly more common in CTS group when compared to the control group (p=0.03) in ultrasonographic assessment, and there was no significant difference between the two groups for other parameters. Conclusion: Shoulder pain and subdeltoid bursitis were common in patients with CTS however, shoulder pain was not directly related to shoulder problems alone. Central sensitization may play a role for shoulder pain rather than shoulder problems.

- MakaleAçık Erişim2026Mercury Removal from Glycine Leach Solutions Using Sodium Sulfide (Na2S) Precipitation
Kantarci, Sadiye; Alp, Ibrahim; Youpoungam, Abdou Azizi; Sivircik, Salih
Amino acid leaching has been considered an environmentally friendly alternative for gold recovery; however, the dissolution of mercury (Hg) during the leaching of amalgamation tailings remains a major challenge. This study is a novelty study that focuses on Hg removal by passing the Hg-containing tailings, previously processed by amalgamation, into solution via glycine (NH(2)CH2COOH) leaching. In this regard, a precipitation method using sodium sulfide (Na2S) was developed to remove Hg from solutions obtained after glycine leaching. The effects of precipitation time, pH, Na2S dosage, and temperature were investigated. The effects of pH, precipitation time, and temperature were found to be limited within the investigated ranges. The results showed that Na2S dosage was the most effective parameter in mercury removal. The initial Hg concentration of 12,160 ppb was reduced to 223 ppb at a Na2S:Hg molar ratio of 60, corresponding to approximately 98% removal efficiency under the selected operating conditions. Increasing the Na2S:Hg ratio to 80 resulted in a slightly higher removal efficiency (similar to 99%) with a final Hg concentration of 144 ppb; however, a ratio of 60 was considered optimal in terms of reagent consumption and process efficiency. The outcomes indicate that Hg removal with Na2S is a rapid and effective process under alkaline conditions, while Au remains in solution and Cu and Ag are partially removed, indicating limited selectivity and the need for further optimization. Therefore, this study provides a basis for developing more environmentally friendly processes for amalgamation tailings.

This study is concerned with encoding types such as value encoding and binary encoding for continuous and discrete optimization with a GA, which is coded in FORTRAN and considers stress and displacement constraints, in view of weight minimization of truss structures. Moreover, when continuous optimization is performed, the challenge of huge search space due to the effort of considering continuous set of design variables is overcame by a mechanism introduced as restricted range approach (RRA) in this study. In comparison with the literature, it is concluded that the program developed in this study can be effectively used in the weight minimization of truss structures. It is also came to the conclusion that value encoding overcomes the adverse effects of Hamming-cliff, and that value encoding requires less computer memory and time, never destroys the fit chromosome.
