Kozdag, Guliz; Ertas, Gokhan; Aygün, Fatih; Emre, Ender; Kirbas, Ahu; Ural, Dilek; Soran, Ozlem

Objective: Enhanced external counterpulsation (EECP) is a noninvasive treatment that is proven safe and effective in patients with coronary artery disease (CAD) and heart failure (HF). The aims of this study are to investigate the clinical effects of EECP therapy in patients with symptomatic CAD and chronic HF, and to find out an answer to the question: Does EECP therapy have any effect on the prognostic markers of HF? Methods: This study was designed as a prospective cohort study. A total of 68 consecutive patients with symptomatic CAD and chronic HF referred to EECP therapy were enrolled in this study between November 2007 and December 2010; 47 patients (39 males and 8 females, 65 +/- 7, years), have undergone EECP treatment, and 21 patients (20 males and 1 female, 62 +/- 10 years), who did not want to participate in the EECP program comprised the control group. Statistical analysis was performed using t tests for dependent and independent samples, Mann-Whitney U test, Chi-square and Fischer exact tests. Results: EECP therapy resulted in significant improvement in post-intervention New York Heart Association functional class (p<0.001), left ventricular ejection fraction (p<0.001), B-type natriuretic peptide levels (p<0.003), uric acid levels (p<0.05), free-T3/free-T4 ratio (p<0.034) and mitral annular E (p<0.05) velocity, compared with baseline, a finding not evident in the control group. Conclusion: EECP treatment significantly improved clinical and some biochemical parameters, which are mostly prognostic markers in patients with symptomatic CAD and chronic HE (Anadolu Kardiyol Derg 2012; 12:214-21)

Effect of Ptosis Surgery on Tear Oxidative Stress Levels in Patients with Blepharoptosis and Pseudoptosis

Sert, Seda; Arici, Ceyhun; Mergen, Burak; Ekmekci, Ozlem Balci

Objectives: Investigation of the surgical correction effect on tear film functions and tear oxidative stress levels in patients with blepharoptosis and pseudoptosis. Methods: Sixty patients with blepharoptosis or pseudoptosis due to dermatochalasis and 30 healthy controls were included in the study. Forty eyes underwent upper blepharoplasty and 20 eyes underwent levator surgery. The tear oxidative stress levels (8-hydroxydeoxyguanosine [8-OHdG] and 4-hydroxy-2-nonenal [4-HNE]) were determined by enzyme-linked immunosorbent assay and tear film functions were evaluated pre-operatively and at the post-operative 1st and 6th months. Results: 8-OHdG and 4-HNE levels in tears were found higher in patients with dermatochalasis (86.3 +/- 38.2 ng/mL; 29.8 +/- 11.4 ng/mL, respectively) and blepharoptosis (95.3 +/- 43.8 ng/mL; 40.8 +/- 3.8 ng/mL, respectively) compared to healthy controls (52.9 +/- 14.0 ng/mL; 27.8 +/- 6.6 ng/mL, respectively). Both levels decreased 1 month after blepharoplasty surgery. The 8-OHdG level in tears of patients who underwent levator surgery increased 1 month after the surgery (p=0.008). No change was detected in tear function tests findings between visits in any patient group. Conclusion: Dermatochalasis and blepharoptosis may lead to an increase in the tear oxidative stress levels. Contrary to a decrease in these levels after blepharoplasty, they may increase in the early period after levator surgery followed by a return to normal levels at the 6th-month visit.

Bülbül, Kenan; Türkkan, Tugba; Meriç, Cagdas Salih; Yilmaz, Haci Omer

Eating disorders are a problem that negatively affects physical and psychological development and can potentially be life-threatening. In order to successfully combat this disorder, it is important to have a better understanding of the underlying mechanisms. The aim of this study is to examine the mediating role of impulsivity in the relationship between internet addiction and eating disorders in an adolescent sample. The current research was designed in accordance with the correlational model. The study group was formed by convenience sampling method among students who continue their education in secondary schools and high schools in Trabzon. The data of the study were collected with a demographic information form prepared by the researchers, the Young Internet Addiction Test-Short Form, Rezzy Eating Disorders Scale and Barratt Impulsivity Scale-Short Form. As a result of statistical analysis, it was determined that the average eating disorder score of female students was higher than that of male students. It was found that the eating disorder scores of those whose income was less than their expenses were significantly higher than those whose income was more than their expenses. Positive significant relationships have been identified between internet addiction and eating disorders. It has also been determined that impulsive behavior plays a mediating role in the relationship between internet addiction and eating disorders. It is thought that the current findings will contribute to the understanding of the mechanisms underlying eating disorders and the development of effective prevention and intervention strategies.

Investigation of the Relationship among Fault Types, Focal Depths, and Ionospheric TEC Anomalies before Large Earthquakes between 2000 and 2020

Ulukavak, Mustafa; Yalçinkaya, Mualla; Kayikçi, Emine Tanir; Öztürk, Serkan; Kandemir, Raif; Karsli, Hakan

In this study, the relationships between possible ionospheric total electron content (TEC) anomalies and earthquake focal depths and fault types were investigated using 3,099 global earthquakes with Mw >= 6.0 between 2000 and 2020. The relationship between the ionospheric TEC changes and earthquakes has been previously discussed in the literature. However, very few studies have examined the relationship between the focal depths and the fault types of these earthquakes. In general, three indices, namely the disturbance storm-time index (Dst), the planetary geomagnetic activity index (Kp), and the solar flux (F10.7), are used to characterize the space weather conditions (SWC) that affect TEC anomalies and to identify the active/quiet days for future earthquakes. In this study, 13 different indices were used: indices that measured solar activity included proton flux (Pf) at six different energy levels, F10.7, and extreme ultraviolet (EUV0.1-50 nm and EUV26-34 nm). Meanwhile, geomagnetic storm indices included Dst, Kp, the z-component of the magnetic field index (Bz), and proton density (P-d). By using the ionospheric TEC values derived from global ionosphere maps (GIM-TEC), both negative and positive ionospheric TEC anomalies were observed for 15 days before and 4 days after the earthquakes using a statistical analysis technique that involved a 15-day moving median. The earthquakes were grouped according to their fault types (normal, thrust, and strike-slip) and focal depths (shallow, intermediate, and deep). Their relationships with the negative and positive TEC anomalies in the quiet days before the earthquake were examined. Negative and positive anomalies were observed approximately 3-12 days before earthquakes that occurred on normal faults, approximately 5-8 days before earthquakes that occurred on thrust faults, and approximately 1-12 days before earthquakes that occurred on strike-slip faults. The average TEC anomaly changes were calculated to be 43.4% total electron content unit (TECU) for normal faults, 44.8% TECU for thrust faults, and 41.3% TECU for strike-slip faults. Positive anomaly values were exhibited by larger earthquakes on normal and thrust faults, whereas negative anomaly values tended to occur on strike-slip faults. Negative and positive TEC anomalies were detected around 3-12 days before shallow earthquakes, around 1-13 days before intermediate-depth earthquakes, and around 1-12 days before deep earthquakes. The average TEC anomaly changes were calculated to be 44.4% TECU for shallow earthquakes, 43.3% TECU for intermediate-depth earthquakes, and 42.6% TECU for deep earthquakes. Positive anomaly values were also found to be higher than the negative anomaly values in each group. We propose that there is a relationship between the ionospheric TEC anomalies that occur before large earthquakes and their fault types and focal depths. EFFECT OF TRAPEZIUS MUSCLE THICKNESS ON THE ONE-REPETITION MAXIMUM (1RM) STRENGTH

Yilmaz, Coskun; Ceylan, Tulay; Altunbas, Adem; Soylu, Hakan Huseyin; Kavuran, Korhan; Söyler, Mehmet

Purpose: The objective of this study was to examine the impact of trapezius muscle thickness measurements on bench press single repetition maximum (1RM) power output. Material and Methods: The study included 13 male bodybuilders who participated in national and international professional natural bodybuilding competitions and 13 healthy sedentary male subjects. Body composition, 1RM values and ultrasonographic measurements of trapezius muscle thickness were obtained from the participants. Results: The findings revealed that both the skin-to-superficial muscle tissue (R1, L1) and skin-to-deep fascia (R2, L2) distances were significantly lower in sedentary individuals compared to bodybuilders (p<0.001). Additionally, 1RM strength output was significantly higher in bodybuilders than in sedentary individuals. In sedentary individuals, a moderate negative correlation was observed between the distance between superficial muscle and 1RM, and a low positive correlation was observed between the distance between deep fascia and skin surface. In contrast, a low negative correlation was found in both parameters in bodybuilders. Conclusion: Our findings indicate that trapezius muscle thickness is significantly lower in sedentary individuals compared to bodybuilders, both in superficial and deep tissue layers. We believe that the skin surface imaging method used in our study could be an effective tool for predicting injuries in athletes. Additionally, we believe that regular imaging follow-ups in athletes could provide important information about adaptations in muscle structures resulting from applied training stimuli.

Investigation of lsepamicin, Chloramphenicol and Minocycline Sensitivity in Carbapenem-Resistant Enterobacteriaceae

Karakullukçu, Asiye; Borsa, Baris Ata; Kuskucu, Mert Ahmet; Bakir, Esra; Taner, Zeynep; Özalp, Veli Cengiz; Aygün, Gokhan

Objective: In this study, we aimed to determine the resistance mechanisms of carbapenem-resistant Enterobacteriaceae (CRE), and evaluate the prospective alternative treatment options. Methods: Between October 2014 and July 2015, rectal swabs routinely obtained from hospitalized patients were included in the study. The isolated Gram-negative enteric bacteria were identified to the species level by using standard conventional microbiological methods. Determined carbapenem resistance in enteric bacteria was confirmed by using meropenem Etest (R) (bioMerieux, Marcy l'Etoile, France), and the resistance mechanisms were investigated with MAST-ID (TM) Discs (Mast Diagnostics Ltd, Bootle, Merseyside, UK). Antibiotic susceptibilities of the CRE were determined by disk diffusion method using isepamicin, chloramphenicol, and minocycline disks (Oxoid, Basingstoke, Hampshire, UK). Results: Of the 3,323 rectal swabs sent to the laboratory during the study period, totally 84 (2.5%) CRE were isolated with one strain from each patient. Of the 84 CRE, 74 (88%) were Klebsiella pneumoniae and 10 (12%) were Escherichia coli. Among the 84 CRE, 66 (78.5%) with OXA-48, 11 (13.1%) with OXA48 + metallo-beta-lactamase (MBL), 1 (1.2%) with only MBL and 1 (1.2%) with AmpC + porin loss were detected, none of the strains were positive for KPC. In 5 (6%) strains, none of the resistance mechanisms were found. Of the 84 CRE, 80 (95.2%), 77 (91.6%) and 14 (16.6%) were found to be susceptible to isepamicin, chloramphenicol and minocycline, respectively. Conclusions: The OXA-48 type, endemic in Turkey, was also the most commonly detected carbapenemase in the study. It has been observed that isepamicin and chloramphenicol can be the potential antibiotics in the treatment of KDE-related infections.

Urologists' Role and Attitude in the Systemic Treatment of Urologic Cancers

Madendere, Serdar; Deger, Muslim Dogan; Demirkiran, Engin Denizhan; Yildiz, Huseyin Alperen

Objective: The goal of this study was to investigate Turkish urologists' role and attitude toward the systemic treatment of urologic cancers. Materials and Methods: An 18-item survey was e-mailed to 2305 certified Turkish urologists. The survey included questions designed to evaluate urologists' knowledge of chemotherapy procedures and their ability to manage complications, as well as how many of them perform systemic therapies and believe urologists should perform these therapies for urologic cancers. Results: We evaluated 305 responses. Most urologists (89.6%) perform hormonotherapy for prostate cancers. Further, the majority refer patients to medical oncologists for chemotherapy. Moreover, 65.9%, 70.8%, and 72.1% of the urologists believe that only medical oncologists should deliver chemotherapy for prostate, bladder, and testicular cancers, respectively. Only 23.9% of urologists believe they are competent to perform chemotherapy, while 16.7% believe they are capable of dealing with chemotherapy complications. Conclusion: Turkish urologists have a lack of performance and interest in administering systemic therapies in urologic cancers. We want to emphasize that urology is more than just surgery. Urologists should be involved in all stages of cancer treatment.

Retrofitting Effect on the Dynamic Properties of Model-Arch Dam with and without Reservoir Water Using Ambient-Vibration Test Methods

Altunisik, Ahmet Can; Günaydin, Murat; Sevim, Baris; Bayraktar, Alemdar; Adanur, Suleyman

Continuous health monitoring and regular condition assessment is of vital importance for engineering structures in general and especially for arch dams. These structures can be exposed to many different loads such as earthquake, blast, wind, ice, and water pressure, which may cause deterioration and loss of structural integrity. Such development may have severe economic consequence and, more importantly, may present a serious risk to public safety. Therefore, the structural behavior of arch dams must be monitored at regular intervals during their service life, although the overall failure rate of these dams is around 1%. Experimental measurements by ambient vibration test methods are among the most commonly preferred inspection techniques for the evaluation of structural performance and safety. The aim of this research is to examine the retrofitting effect on the dynamic properties of a damaged arch dam model, with and without reservoir water, after retrofitting with high-strength structural repair mortar and carbon fiber reinforced polymer composite material. To this end, an arch dam-reservoir-foundation model was built. In the study, ambient vibration tests were carried out for three test-cases to determine changes in dynamic properties, such as natural frequency, mode shape, and damping ratio before and after retrofitting. The three cases were (1)a severely damaged dam with full reservoir (Case-A), (2)a repaired dam with full reservoir (Case-B), and (3)a strengthened dam with full reservoir (Case-C). In addition, all the measurement tests were repeated with an empty reservoir to extract the effect of water. Data for the ambient vibration tests were gathered from the dam body during vibrations by natural excitations. These excitations were provided through small impacts, and the response signals were measured by sensitivity accelerometers placed at the crest points. To obtain the experimental dynamic properties, the enhanced frequency domain decomposition method in the frequency domain was used. Comparisons of experimentally identified dynamic properties for the damaged and the retrofitted dam showed that the retrofitting application affected the structural behavior of the arch dam considerably. It was very effective in restoring the structure's original dynamic properties.

Finite Modeling Updating Effects on the Dynamic Response of Building Models

    Günaydin, M.; Adanur, S.; Altunisik, A. C.; Sevim, B.; Bayraktar, A.

An improved finite element model was used to obtain more accurate modeling of engineering structures. The main goal of this paper was to determine how this improved modeling procedure affects the dynamic response of buildings based on experimental modal parameters such as natural frequencies, mode shapes and damping ratios. For experimental assessment, three small, one-story, two-bay, reinforced concrete buildings with raft foundation were constructed under laboratory conditions. The initial three-dimensional finite element models, built with the ANSYS software, were used to analytically identify modal parameters, including natural frequencies and mode shapes. Following the analytical study, the ambient vibration tests were performed to obtain modal parameters, experimentally. The enhanced frequency domain decomposition method and the stochastic subspace identification method were used to identify the modal parameter. The analytical and experimental modal parameters were compared and then initial finite element models were updated to minimize the differences by changing of some uncertain parameters such as materials properties. With model updating, the maximum difference between the measured and initially obtained frequency was reduced from 28.47 to 4.88%. To show the model updating effect on dynamic response of building models, dynamic analysis with the experimental computed damping ratios and 5% damping ratio were carried out using 1992 Erzincan earthquake ground motion record. For the experimental damping ratios, the maximum differences in the displacements and stresses between the initial and updated models were obtained as 23 and 32%, respectively. These differences were calculated as 43 and 15 % for the 5 % damping ratio. Comparisons of dynamic analyses results for the initial and updated model show that the finite element model updating affects the dynamic response of the buildings considerably.

The effect of different therapeutic treatments on the frequency of postoperative hypocalcemia in patients with thyroidectomy

Piyade, Hamdi Burak; Basoglu, Mahmut; Gürsoy, Ersan

Objective: Thyroid gland surgery and its surgical complications are situations that a surgeon frequently encounters in his daily practice. In our study, it was aimed to examine the effect of different treatment methods given to patients who underwent thyroidectomy on hypocalcemia. Material and Methods: Three hundred and seventy-one patients who underwent thyroidectomy at Ondokuz Mayis University Medical Faculty General Surgery clinic between December 2016 and January 2021 were retrospectively included in the study. Parameters such as surgery indications, fine needle aspiration biopsy results, preoperative serum calcium values, type of surgery, serum calcium values at postoperative 1st day and 1st month, postoperative hospital stay, drugs prescribed at discharge, histopathological diagnosis of the patient, and whether there was incidental parathyroidectomy or not were included. Results: Mean age of 371 patients who underwent thyroidectomy was 49 (19-82) years. Total thyroidectomy was the most common type of thyroidectomy with 61% (n= 225) of the patients. There was a significant decrease in pre- op and post-op calcium values in all three types of surgery performed on the patients, and there was no significant difference between the different types of surgery. Post-operative day one and month one serum calcium values were significantly increased in all groups (p=.000). The increase in post-op serum calcium level was most common in the group using calcium carbonate + cholecalciferol + calcitriol. Conclusion: The use of post-op calcitriol in patients undergoing thyroidectomy seems to be quite effective in preventing the development of hypocalcemia.

EZH2 expression in colorectal carcinoma: an evaluation of clinicopathological and prognostic value

Cagatay, Diren Vuslat; Balci, Mecdi Gurhan; Issin, Gizem; Demir, Fatih

BackgroundEnhancer of zeste homolog 2 (EZH2), the catalytic subunit of Polycomb Repressive Complex 2, catalyzes trimethylation of histone H3 lysine 27 and has been implicated in tumor progression.AimOur study aims to assess the relationship between EZH2 expression and clinicopathologic data, and survival in colorectal carcinomas (CRC).Materials and methodsEZH2 immunohistochemistry was performed on tumor blocks from 124 CRC patients. Based on H-scores, cases were stratified into low- and high-expression groups. EZH2 status was correlated with demographic, clinical, and pathologic features, and overall survival was analyzed with the Kaplan-Meier method and log-rank test.ResultsA total of 124 cases of CRC; 12 (9.7%) cases were classified as low EZH2 expression, and 112 (90.3%) cases were classified as high EZH2 expression. A significant association was found between EZH2 expression and microsatellite stability. High EZH2 expression was significantly enriched in microsatellite-stable tumors (p = 0.007) and in left-sided lesions (p = 0.049). No significant associations were detected with sex, stage, grade, lymph-node status, or vascular invasion. Kaplan-Meier analysis revealed no difference in overall survival between low- and high-EZH2 groups (log-rank p = 0.47; hazard ratio = 1.13, 95% CI 0.45-2.85). EZH2 staining was uniformly strong in normal mucosa and adenomas, mirroring the high expression observed in most CRCs.ConclusionsEZH2 is highly expressed across the normal-adenoma-carcinoma sequence and, in our cohort, was not linked to overall survival or to most clinicopathologic parameters in CRC. The lower expression observed in a subset of MSI-H tumors warrants further investigation; present evidence remains insufficient to establish EZH2 as an independent prognostic biomarker.

Dynamic Characteristics of an Arch Dam Model before and after Strengthening with Consideration of Reservoir Water

Altunisik, Ahmet Can; Günaydin, Murat; Sevim, Baris; Bayraktar, Alemdar; Adanur, Suleyman

This technical note presents an investigation about the changing of dynamic characteristics of an arch dam model before and after strengthening considering reservoir water. To this end, an arch dam-reservoir-foundation model has been constructed in the laboratory. Ambient vibration tests have been performed on the damaged and strengthened arch dam models for empty and full reservoir to determine the effect of water on the dynamics characteristics. Enhanced frequency domain decomposition method has been used to extract the dynamic characteristics. Finally, the dynamic characteristics obtained from the damaged and strengthened dam models have been compared to with each other. Also, the natural frequencies for damaged and strengthened models are compared to detect the effect of strengthening application on the frequencies. Results indicate that the natural frequencies and damping ratios are changed with the full reservoir. However, mode shapes are not changed. Results also show that the natural frequencies of damaged dam considerably increase after strengthening applications.

Evaluation of acute kidney injury (AKI) with RIFLE, AKIN, CK, and KDIGO in critically ill trauma patients

    Ülger, F.; Küçuk, M. Pehlivanlar; Kucuk, A. O.; Ilkaya, N. K.; Murat, N.; Bilgiç, B.; Abanoz, H.

The aim of our study was to evaluate the effects of AKI development on mortality with four different classification systems (RIFLE, AKIN, CK, KDIGO) in critically ill trauma patients followed in the intensive care unit. A retrospective review of 2034 patients in our intensive care unit was conducted between July 2010 and August 2013. A total of 198 patients with primary trauma were included in the study to evaluate the development of AKI. When the presence of AKI was investigated according to the four criteria (RIFLE, AKIN, CK, and KDIGO), the highest incidence of AKI was found according to the KDIGO classification (74.2%), followed by AKIN (72.2%), RIFLE (69.7%), and CK (59.1%). It was observed that more AKI developed according to KDIGO in patients with multiple trauma and thoracic trauma (p = 0.031, p = 0.029). Sixty-two (31%) of the 198 trauma patients monitored in the intensive care unit died; mortality was frequently found high in AKI stage 2 and 3 patients. According to the CK classification, there was a significant increase in mortality in patients with AKI on the first day (p = 0.045). AKI classifications by RIFLE, AKIN, CK, and KDIGO were independently associated with the risk of in-hospital death. In this study, the presence of AKI was found to be an independent risk factor in the development of in-hospital mortality according to all classification systems (RIFLE, AKIN, CK, and KDIGO) in critically traumatic patients followed in ICU, and the compatibility between RIFLE, AKIN, and KDIGO was the highest among the classification systems.

Phytochemical Analysis of Dorycnium pentaphyllum and Its Antiproliferative Effect on Cervix Cancer Cells

Aliyazicioglu, Yuksel; Demir, Selim; Yaman, Scrap Ozer; Sener, Sila Ozlem; Demir, Elif Ayazoglu; Aliyazicioglu, Rezzan; Turan, Ibrahim

The purpose of this study was to investigate the phytochemical analysis of D. pentaphyllum and its cytotoxic activity on human cervical cancer (HeLa) cells with possible mechanisms. Total phenolic contents (TPC) and phytochemical analysis of the extract were evaluated using spectrophotometric method and RP-HPLC, respectively. The cytotoxic effect of the extract was evaluated using the MTT assay. The mechanism of the cytotoxic effect of the extract was also evaluated in terms of apoptosis, cell cycle, and mitochondrial membrane potential (MMP) using the fluorometric methods. The TPC of the extract was calculated as 68.9 +/- 2.3 mg gallic acid equivalent per g sample and quercetin was determined as the major phenolic in the extract. The extract showed a selective cytotoxic effect (6.5-fold) on HeLa cells compared to normal fibroblast cells. The cytotoxic effect of the extract was found to be due to the arresting of the cell cycle in the S phase and increasing apoptosis through decreased MMP. Further studies are needed for the comprehensive effects of the extract on cell signaling.

Yilmaz, Mehmet Fatih; Acar, Emrah; Inanir, Mehmet; Karabay, Can Yucel; Izgi, Ibrahim Akin

Introduction Data regarding the possible role of heparanase (HPA) in the occurrence of left atrial/left atrial appendage (LA/LAA) thrombus in patients with atrial fibrillation (AF) is lacking. The goal of the present study was to assess the association between plasma levels of HPA and LA/LAA thrombus in AF. Methods A total of 687 patients with nonvalvular AF (NVAF) without anticoagulation therapy were included from January 2016 to June 2019. Serum HPA analysis was performed with a commercially available human ELISA kit. Logistic regression models were used to test for association. Results Serum HPA levels were significantly higher in patients with LA/LAA thrombus than in those without LA/LAA thrombus (270.8 [193.4 +/- 353.2] pg/mL vs 150.3 [125.2 +/- 208.4] pg/mL; P < 0.001). In multivariate analysis, serum HPA remained a significantly independent predictor of LA/LAA thrombus (odds ratio 1.674, 95% confidence interval [CI] 1.339-2.289, P < 0.001). In the receiver operating characteristic (ROC) curve analysis, HPA showed a predictive value with an area under the curve (AUC) of 0.757 (95% CI 0.652-0.810, P < 0.001). The optimal cutoff level for HPA predicting LA/LAA thrombus was 210.7 pg/mL, with a sensitivity of 74.3% and a specificity of 64.8%. Conclusion An elevated HPA level was associated with the presence of LA/LAA thrombus in patients with AF. HPA might portend the risk for the prothrombotic state in AF patients.
