Pasli, Sinan; Imamoglu, Melih; Çiçek, Mustafa; Yadigaroglu, Metin; Sahin, Aynur; Karaca, Yunus; Yulug, Esin; Tatli, Ozgur

Aim: The purpose of this study was to investigate the protective effects of ethyl pyruvate (EP) and dimethyl sulfoxide (DMSO) on ischemic tissue in brain and cardiac and hepatic tissues in experimentally induced carbon monoxide (CO) intoxication. Material and Methods: Thirty-five mature female Sprague Dawley rats were randomized into five groups of seven animals each. Group 1 received no CO or treatment. Rats in groups 2, 3, 4 and 5 were made to inhale a high-concentration 5000 ppm CO gas mixture for 60 min at 4 L/min. CO levels were then measured from 1 mL tail vein blood. Group 2 received no therapeutic agent, group 3 received 6 mg/kg intraperitoneal (ip) DMSO, group 4 received 50 mg/kg ip EP, group 5 received 50 mg/kg EP and 6 mg/kg DMSO ip. All rats were sacrificed by decapitation. Brain, cardiac and hepatic tissues were removed and histopathological scores were compared. Results: Comparison of group 2 with group 3 and group 2 with group 5 revealed that DMSO alone and EP + DMSO exhibited a reducing effect on degree of cerebral neuronal alteration, degenerative neuron rates, and total cardiac injury score (p=0.005, p=0.002, p=0.004 and p=0.002, p=0.001, p=0.004 respectively). There was no histopathological difference between group 2 and group 4 suggesting EP alone had no therapeutic effect on histopathological injury. Conclusion: Based on our study findings, EP administered alone exhibited no protective effects on the organ injuries investigated, while DMSO exhibited reducing effects on degree of neuron alteration, rates of degenerative neurons, and total cardiac injury scores. An ameliorating effect on cardiac and hepatic injury was more prominent with combined treatment.

Karakoyun, Suleyman; Tanboga, Ibrahim Halil; Gökdeniz, Tayyar; Lazoglu, Zakir; Topçu, Selim; Karal, Huseyin; Aksakal, Enbiya; Sevimli, Serdar

ObjectivesWe aimed to test the left atrial (LA) mechanics and contraction synchrony by 2D strain imaging, in patients with Wolff-Parkinson-White (WPW) syndrome, before and after radiofrequency catheter ablation (RFCA). MethodsStudy population consisted of 25 patients with WPW scheduled for RFCA and 30 healthy controls. The peak LA strain at the end of the ventricular systole (LAs strain) and the LA strain with LA contraction (LAa Strain) were obtained. To assess LA dyssynchrony, septal versus lateral wall time-to-peak strain measurements were measured. ResultsThere was no difference between the patients with WPW and control subjects with regard to peak LAs and LAa strain. Patients with WPW demonstrated higher global time-to-peak LAs and LAa strain values compared with the control group. Peak LAs strain and LAa strain values, measured before and after the RF ablation of the accessory pathway, were comparable (34.33.92 vs. 34.6 +/- 3.2, P=0.816, 14.7 +/- 2.8 vs. 15.3 +/- 2.3, P=0.052, respectively). Global time-to-peak LAs and LAa strain measurements were significantly shorter after the RFCA compared with the values obtained before the RFCA. However, septo-lateral times to peak LA strain differences were found to be comparable in both WPW versus control and pre- versus postablation groups. ConclusionLA mechanical function assessed by 2D strain imaging was comparable between patients with WPW and control subjects. Patients with WPW had more prominent LA dyssynchrony during atrial pump phase as compared with the controls, a condition which could not improve after successful elimination of the accessory pathway by RFCA.

Oguz, Ahmet Burak; Polat, Onur; Eneyli, Muge Gunalp; Gulunay, Behnan; Eksioglu, Merve; Gurler, Serdar

Study objective: Our aim was to determine the efficiency of ultrasound (US) scanning in patients with wrist trauma admitted to the emergency department and to compare US diagnostic usagewith other radiological imagingmethods. Methods: Patients who presented to the emergency department with wrist injury and who met the inclusion criteria and exclusion criteria were eligible. For all patients, US evaluation of thewhole wristwas performed by an emergency physician before other radiological imagingmethods (radiographies, computed tomography (CT) and magnetic resonance (MR) imaging). All of the patients included in the study underwent US, radiography, CT, and MR. Results: During the study, 122 patients were admitted with a wrist injury. After filtering for the exclusion criteria, 80 patients were included in the study. The sensitivity of US scanning in detecting fractures was 95.31% (95% confidence interval [CI]: 87.1-98.39), the specificity was 93.75% (95% CI: 71.67-98.89), and the positive predictive value was 98.39% (95% CI: 91.72-99.85), and the negative predictive value was 83.33% (95% CI: 72.98-90.41). The sensitivity of US scanning in detecting tendon and ligamentous structural injury was 66.67% (95% CI: 41.71-84.82), the specificity was 100% (95% CI: 94.42-100), the positive predictive valuewas 100% (95% CI: 94.29-99.89), and the negative predictive was 92.86% (95% CI: 84.25-97.14). Conclusion: US scanning is an effective method that can be applied in the emergency department to adult patients to diagnose distal forearm and carpal bones fractures. In soft tissue injuries, US and MR examinations produce similar results. (C) 2017 Elsevier Inc. All rights reserved.

Piroglu, Isilay; Tulgar, Serkan; Thomas, David Terence; Cakiroglu, Basri; Piroglu, Mustafa Devrim; Bozkurt, Yasin; Gergerli, Ruken; Ates, Nagihan Gozde

Background: Mechanical bowel preparation before colorectal surgery is commonly performed, but its benefits are controversial. The aim of this study was to compare the effects of mechanical bowel preparation on healing of colonic anastomosis and tissue strength. Material/Methods: After institutional review board approval, 20 adult Wistar albino rats were randomly divided into 2 groups of 10 animals each. Mechanical bowel preparation including sodium phosphate was performed on the experimental group via a feeding tube, whereas no bowel preparation procedures were performed on the control group. Transverse colon resection and anastomosis were performed on all rats under general anaesthesia. On postoperative day 5, re-laparotomy was performed and the anastomotic areas were resected. Animals were killed, after which bursting pressure and tissue hydroxyproline concentrations were measured, histopathological examination was performed, and we evaluated and compared the results. Results: There were no differences between control and experimental groups in bursting pressure, tissue hydroxyproline concentrations, or histopathological examination results (P>0.05). Conclusions: Our study demonstrated no significant difference between bursting pressures, tissue hydroxyproline levels, or modified wound healing score at postoperative day 5 between rats undergoing and not undergoing mechanical bowel preparation. Mechanical bowel preparation is not essential for healing or strength of colonic anastomosis in rats.

Artificial Neural Network-Based prediction of production cross sections of the medical radioisotopes 67Ga and 89Zr

Okutan, Murat; Üncü, Yigit Ali; Sevim, Gencay; Sekerci, Mert; Kiliçarslan, Tolga; Demir, Bayram; Ozdogan, Hasan

In this study, the excitation functions of the 67Zn(p,n)67Ga, 68Zn(p,2n)67Ga, and 89Y(p,n)89Zr reactions were investigated. Nuclear reaction simulations were performed using the TALYS 1.95 and EMPIRE 3.2 codes, by using Two Component Exciton Model, Geometry Dependent Hybrid Model, Exciton Model, and Hybrid Monte Carlo Simulation. In parallel, a feedforward artificial neural network (ANN) trained with the Levenberg-Marquardt (LM) algorithm was employed to predict the same reaction cross-sections using experimental data. Comparative analysis revealed a strong consistency between TALYS and EMPIRE calculations, ANN predictions, and experimental results, with the ANN providing enhanced predictive performance in energy regions where experimental data are sparse or uncertain. Furthermore, activation and yield calculations confirmed the feasibility of producing clinically relevant quantities of 67Ga and 89Zr under realistic irradiation conditions. In conclusion, the use of physics-based nuclear reaction codes with data-driven ANN models has been demonstrated to be useful in medical radionuclide production studies by providing a complementary framework for cross-section estimation.

Shading Effect of Transparent Photovoltaic Panels on Crops Underneath Agrivoltaic Systems

Esmaeilzad, Nasim Seyedpour; Dino, Ipek Gursel; Güney, Dilara; Yildirim, Yusuf Ersoy; Turan, Rasit; Özden, Talat

Agrivoltaic systems combine soil-grown crops with photovoltaic (PV) panels erected several meters above the ground. Combining solar panels and food crops on the same land can maximize land utilization. Under the PV panels, however, microclimate factors like solar radiation, air temperature, humidity, and soil temperature change. An agrivoltaic system must optimize sunlight sharing between solar panels and crops to maximize food energy production. It has been challenging to improve and analyze the performance of agrivoltaic systems due to the lack of a defined crop-specific parameter. In this work, we present a practical option to partially replace bifacial modules with semi-transparent ones, providing comparable levels of crop protection and greater climate change resilience while generating green energy and increasing land-use efficiency. The agrivoltaic system must be tailored to satisfy the needs of crops. For this purpose, a simulation model was conducted, which examined the impact of module transparency and cell layout based on light availability.

Aydinyilmaz, Faruk; Guliyev, Ilkin; Özbeyaz, Nail B.; Algül, Engin; Aker, Mert; Sahan, Haluk F.; Erzurum, Muhammed; Felekoglu, Mehmet A.; Kalkan, Kamuran

Aim: To appraise the prediction of tricuspid annular plane systolic excursion (TAPSE)/systolic pulmonary artery pressure (SPAP) with regard to hospitalization and the effect of spironolactone use. Materials & methods: A total of 245 patients were evaluated for the study. Patients were followed for 1 year and cardiovascular outcomes were determined. Results: It was determined that TAPSE/SPAP was an independent predictor of hospitalization. A 0.1-mmHg decrease in TAPSE/SPAP was associated with a 9% increase in relative risk. No event was observed above the 0.47 level. Negative correlation with TAPSE (uncoupling) began in the spironolactone group when SPAP was >= 43 and in nonusers when SPAP was 38 (Pearson's correlation coefficient: -,731 vs -,383; p < 0.001 vs p = 0.037). Conclusion: TAPSE/SPAP measurement may be useful in predicting 1-year hospitalization in asymptomatic heart failure patients. This ratio was also found to be higher in patients who used spironolactone.

Air pollution, a possible risk factor for multiple sclerosis

Börü, Ulku Turk; Bölük, Cem; Tasdemir, Mustafa; Gezer, Tugce; Serim, Vedat Ataman

Objective Studies focusing attention on the effects of environmental pollution on the etiology of multiple sclerosis (MS) are on the increase. The aim of this study was to determine MS prevalence in a city home to an iron and steel factory which causes air pollution. Methods The study was designed as a cross-sectional, population-based, descriptive epidemiologic study. Eregli city, which has an iron and steel factory and proven air pollution, was screened. Additionally, Devrek city, which is a rural and clean city, located 40 km away from Eregli was assigned and results were compared. A validated questionnaire was used for screening. McDonald 2010 criteria were used to diagnose cases. Results 32 261 people were screened in Eregli, and 21 963 people were screened in Devrek. In total, 41 patients were diagnosed with clinical definite MS. Crude prevalence was found to be 96.1/100 000 in Eregli and 45.5/100 000 in Devrek. The mean age of patients was 39.8, and the female/male ratio was 1.9. Conclusion The results of this study indicate a more than double MS prevalence rate in the area home to an iron and steel factory when compared to the rural city. This supports the hypothesis that air pollution may be a possible etiological factor in MS.

The COPD-augmented R2CHA2DS2-VA score: development and internal assessment for predicting one-year outcomes after transcatheter aortic valve implantation

Boyaci, Faruk; Sahin, Mustafa Kursat; Akcay, Murat; Yanik, Ahmet; Yenercag, Mustafa; Seker, Onur Osman; Ucar, Melisa; Kokcu, Halil Ibrahim; Ozturk, Berkant; Kaya, Enes

Background Existing CHA(2)DS(2)-VASc-based scores incompletely capture the prognostic impact of chronic obstructive pulmonary disease (COPD) and of graded renal dysfunction in patients undergoing transcatheter aortic valve implantation (TAVI). We acknowledge that the score's development in a high-risk, high-comorbidity cohort may produce optimistic performance estimates that require attenuation through external validation. We aimed to develop a novel COPD-augmented R(2)CHA(2)DS(2)-VA (COPD-R(2)CHA(2)DS(2)-VA) score that systematically integrates quantitative renal function and COPD, and to internally assess its discriminative performance for one-year all-cause mortality and major adverse cardiovascular events (MACE). Methods In this single-center, retrospective cohort study, 622 consecutive patients undergoing TAVI for severe aortic stenosis (2018-2024) were analyzed. The COPD-R(2)CHA(2)DS(2)-VA score was constructed as a prespecified clinical tool by augmenting the R(2)CHA(2)DS(2)-VA framework with COPD (1 point) and refining renal dysfunction into two graded estimated glomerular filtration rate strata (eGFR 30-59 mL/min/1.73 m & sup2;: 1 point; eGFR < 30: 2 points). Discriminative performance was evaluated using receiver operating characteristic curve analysis with bootstrap internal validation (2,000 resamples) and compared with established CHA(2)DS(2)-VASc-based scores. Multivariable logistic regression assessed the independent prognostic value of the score treated as a continuous variable. Results The COPD-R(2)CHA(2)DS(2)-VA score demonstrated encouraging discriminative ability for one-year all-cause mortality (apparent area under the curve [AUC] 0.946, 95% CI 0.926-0.971). As a prespecified tool with fixed component weights, bootstrap internal validation confirmed negligible optimism (mean optimism - 0.00014), with bias-corrected AUC identical to the apparent value. The addition of COPD significantly improved mortality discrimination over the R(2)CHA(2)DS(2)-VA score (Delta AUC + 0.033, p = 0.0001). For one-year MACE, the apparent AUC was 0.806 (95% CI 0.765-0.852); the addition of COPD did not significantly enhance MACE discrimination (Delta AUC + 0.001, p = 0.853). In multivariable analysis, each one-point increase in the COPD-R(2)CHA(2)DS(2)-VA score was independently associated with 3.52-fold higher odds of one-year mortality (95% CI 2.45-5.06, p < 0.001) and 2.02-fold higher odds of MACE (95% CI 1.67-2.46, p < 0.001). The bias-corrected AUC of the full multivariable model was 0.973 (95% CI 0.962-0.984) for mortality and 0.795 (95% CI 0.752-0.841) for MACE. Conclusions The COPD-R(2)CHA(2)DS(2)-VA score demonstrated encouraging discriminative ability for one-year all-cause mortality after TAVI in this single-center development cohort (apparent AUC 0.946). However, this high discriminative performance is likely influenced by the elevated comorbidity burden in our cohort and the single-center design, and should be interpreted as an upper-bound estimate. The addition of COPD provides a statistically significant but clinically modest incremental improvement for mortality (Delta AUC + 0.033, p = 0.0001), with no significant enhancement for MACE (Delta AUC + 0.001, p = 0.853). These findings are preliminary and hypothesis-generating; urgent external validation in diverse, prospective cohorts is essential before any clinical application can be considered. At present, this score should be regarded as a development-phase, hypothesis-generating tool that complements rather than replaces guideline-recommended instruments such as STS-PROM and EuroSCORE II. The score's clinical utility remains unproven.

Analyzing the linkage between Islamic financial literacy and Islamic banking services adoption: evidence from Turkey

Pala, Fahrettin; Erdogdu, Aylin; Ali, Muhammad; Alnori, Faisal; Barut, Abdulkadir

PurposeThe purpose of this study is twofold. First, this research explores the level of Islamic financial literacy of customers in the context of Islamic banking. Second, this study examines the determinants of customer adoption of Islamic banking in Turkey. Design/methodology/approachThis study gathered sample data from 409 participants determined using the purposive sampling method. In the study, first, the reflective measurement model is used to examine the reliability, validity and multicollinearity problems of the variables. Then, AMOS structural equation model (SEM) is used to reveal the relationship between Islamic financial literacy and Islamic banking services. Additionally, this study performed both descriptive and inferential analysis to understand customer literacy about Islamic banking and their adoption behavior of Islamic banking. FindingsThe results obtained from descriptive assessment indicate that Turkish customers of Islamic banking possess sufficient literacy about Islamic banking. Moreover, the results from SEM indicate that the adoption of Islamic banking by customers is significantly predicted by the role of Sharia Board management, Islamic banking and purpose of financial institution, religious factor and legitimacy of Islamic financial system. Research limitations/implicationsThis study focuses only on the level of knowledge and perceptions of customers who have accounts in Islamic banks or financial institutions in Turkey. It does not focus on the level of knowledge and perception of Muslims who do not have accounts in Islamic banks and financial institutions. Originality/valuePrevious studies on Islamic banking are mostly studies that investigate customers' perceptions of the Islamic banking system and why individuals prefer Islamic banks. In particular, studies examining the relationship between individuals' Islamic financial literacy level and Islamic banking preferences are limited. This study is considered to be an original study as it investigates the relationship between the Islamic financial literacy level of individuals and their adoption of Islamic banking services in Turkey.

Purely endoscopic endonasal resection of extracranial trigeminal schwannomas: a descriptive systematic review and institutional case series

Yilmaz, Eren; Emengen, Atakan; Uzuner, Ayse; Ozsoy, Bedrettin; Gokbel, Aykut; Anik, Ihsan; Ceylan, Savas

Background: Trigeminal schwannomas (TSs) are uncommon, benign tumors that arise from the trigeminal nerve. They can spread to the middle cranial fossa, posterior fossa, and extracranially (pterygopalatine fossa (PPF), infratemporal fossa, orbita). Among these, extracranial involvement is the least frequent. Before the advent of endoscopic techniques, these tumors were primarily removed using conventional microsurgical approaches. Due to both the rarity of extracranial localization and the increasing use of endoscopic techniques in suitable cases, reports of TSs with extracranial involvement treated via a purely endoscopic endonasal approach (EEA) remain limited, typically appearing as case reports or a small subset within larger case series. This study systematically reviews the literature on extracranial TSs managed exclusively with EEA and presents two illustrative cases from our institution. Methods: A systematic search of the PubMed database was performed in accordance with PRISMA guidelines to identify studies that utilized a purely EEA for extracranial TS cases. Additionally, a retrospective review of 6118 EEA procedures conducted at our institution identified two patients with histopathologically confirmed extracranial TSs. Clinical presentation, surgical approach, outcomes, and complications were analyzed. Results: A total of 147 records were identified through database searches and reference lists. After removing duplicate records, 144 full-text articles were assessed for eligibility. Of these, 124 studies were excluded with justification, resulting in 23 studies being included in the review. When combined with our institutional cases, data from a total of 52 patients were analyzed. Facial numbness was the most common presenting symptom (70.5 %). The mean tumor diameter was 38.6 +/- 13.3 mm, and the gross total resection rate was 93.1 %. In our cases, both patients successfully underwent tumor resection via EEA without postoperative neurological deficits. Conclusions: EEA provides a direct, minimally invasive route for managing extracranial TSs while reducing the morbidity associated with transcranial approaches. Although optimal for purely extracranial tumors, lesions extending into the middle or posterior fossa may require a combined approach. Therefore, individualized surgical planning is essential for achieving optimal outcomes in TS management.

Finite element model calibration effects on the earthquake response of masonry arch bridges

Sevim, Baris; Bayraktar, Alemdar; Altunisik, Ahmet Can; Atamtürktür, Sezer; Birinci, Fatma

The focus of this paper is to illustrate the importance of model calibration and in situ vibration testing by comparing the finite element model predictions of the earthquake response of the two historical arch bridges, Osmanli and Senyuva, before and after model calibration. The three-dimensional finite element models of these two arch bridges, built in the ANSYS finite element program, are used to predict bridge dynamic characteristics, such as natural frequencies and mode shapes. Following the analytical study, ambient vibration tests were conducted to experimentally obtain dynamic characteristics of these two bridges. During ambient vibration tests, accelerometers were placed at several points on the bridge to collect the vibration response due to natural and operational excitation sources. Enhanced frequency domain decomposition and stochastic subspace identification techniques were used to extract the experimental natural frequencies, mode shapes and damping ratios. Finite element models of the two arch bridges were adjusted such that the model predictions reproduce the ambient vibration test results with increased fidelity. The behavior of the masonry arch bridges under earthquake excitation recorded during the Erzincan Earthquake in 1992 is simulated by both the initial and adjusted finite element models. The findings of this study emphasize the importance of model calibration and ambient vibration testing. (C) 2010 Elsevier B.V. All rights reserved.

Rationally decreasing the number of drugs seems to be a useful therapeutic approach in older adults: 6-month follow-up study

Idil, Eral; Aydin, Ali Ekrem; Bulut, Esra Ates; Isik, Ahmet Turan

Aims: Older adults are at risk for polypharmacy, which has multidimensional safety, clinical and economic implications. Therefore, the optimization of drug therapy is one of the critical components of geriatrics clinical practice. This study is aimed to investigate the effect of drug rationalization on comprehensive geriatric assessment(CGA) parameters. Materials and Methods: The study was a retrospective and longitudinal study examining the effect of change in drug number on functionality and physical performance. A total of 515 patients were included in the study. Detailed medication history, laboratory findings, CGA parameters were recorded in the first admission. Polypharmacy was accepted as concurrent usage of five or more drugs. Medications of each patient were reviewed with the guidance of CGA and Beers Criteria. Results: The mean age of patients was 74.13 +/- 7.29 years, and 68.7% were female. The baseline mean number of drugs was 5.11 +/- 3.34. The polypharmacy group(269 patients) had a higher rate of geriatric syndromes and lower CGA scores in the first admission. After optimization of medications, the mean drug number decreased to 4.76 +/- 2.72. Depression, mobility and nutritional scores improved at the end of six months in the patients whose total number of drugs decreased, while global cognition, activities of daily living scores, and gait speed were preserved. Increasing the number of drugs was associated with lower mobility and functionality. Conclusion: Drug rationalization guided with CGA improves the nutritional, physical, and psychosocial status of geriatric patients. Thus, medication reviews have key importance in the management of older patients.

Osteoclastic activity in chronic otitis media with cholesteatoma-related bone destruction

    Özgur, A.; Yemis, T.; Basbulut, E.; Turgut, N. F.; Özdemir, D.; Akgul, G.; Mehel, D. M.; Celebi, M.

Background Cholesteatoma-related bone destruction is the cause of many complications due to chronic otitis media. This study aimed to evaluate osteoclastic activity in cholesteatoma-related bone destruction using tartrate-resistant acid phosphatase 5b, an enzyme specific to osteoclastic activity. Method Seventy-two patients diagnosed with chronic otitis media were included in this study and were divided into two groups: with and without bone destruction. The blood serum and tissue tartrate-resistant acid phosphatase 5b levels from both groups were compared. Results There were no significant differences in the level of serum enzymes between both groups. However, in tissue samples, tartrate-resistant acid phosphatase 5b levels were significantly lower in the bone destruction group than the group without bone destruction. Conclusion This study determined that the level of tartrate-resistant acid phosphatase 5b, a specific enzyme for osteoclastic activity in cholesteatoma-related bone destruction, is locally decreased. This data suggests that osteoclastic activity may decrease in cholesteatoma-related bone destruction. However, further experimental and clinical studies are required to clarify this highly complex mechanism.

Financial Inclusion, Energy Intensity, and Natural Resources: Environmental Insights From BRICS and MINT Economies

In developing economies, rapid economic growth intensifies environmental pressures, thereby increasing the importance of financial inclusion, energy intensity, and natural resource dependence for environmental sustainability. This situation necessitates a holistic examination of the determinants of environmental degradation in emerging markets, which motivates this study. In this context, the study examines the effects of financial inclusion, energy intensity, and natural resource rents on carbon dioxide (CO2) emissions in BRICS and MINT countries and empirically tests the validity of the Environmental Kuznets Curve (EKC) hypothesis. Accordingly, annual panel data covering the period 2000-2020 are analysed using a machine learning (ML)-based estimation framework, the Augmented Mean Group (AMG) estimator, and Panel Quantile Regression (PQR). The findings indicate that natural resource rents increase CO2 emissions in BRICS, whereas they reduce emissions in MINT. Moreover, energy intensity and income per capita are found to increase CO2 emissions across all quantiles in both country groups. The environmental impact of financial inclusion is heterogeneous, reducing CO2 emissions in BRICS countries while increasing emissions in MINT countries. These results underscore the critical importance of explicitly incorporating country-specific heterogeneity into environmental sustainability policy design.
