Assessment of Hazards in Confined Areas Studies Using a Multi-Criteria Decision-Making Method: An Example Application

    Doğan, Onur

Confined spaces are places with only one entrance and exit, not designed for long-term work (reaction tanks, manholes, sewers, tunnels, silos, cellars, pipelines, warehouses, etc.). Confined spaces can contain many risks such as gas, dust, noise, vibration, swallowing, crushing, dent, chemical and biological. In some cases, these risks can occur in an integrated manner and cause serious material or moral losses. In this respect, it is vitally important for occupational safety and employee health to comprehensively analyze risks before indoor work. In this study, the risks in confined spaces were evaluated with the Multi-Criteria Decision Making (MCDM) method. The main criteria and sub-criteria used in the study were prepared by considering the opinions and suggestions of experts in the field (with indoor working experience) and literature research (Aykaş, 2018; Güzel, 2013; Uysal, 2020). In the analysis, employee-related hazards, chemical and biological hazards, ergonomic hazards and physical hazards and a total of twelve sub-criteria affecting these criteria were formed. In the analysis of the study, Analytical Hierarchy Process (AHP) method, one of the Multi-Criteria Decision Making (MCDM) methods, was preferred. The AHP method is an advantageous method compared to other MCDM methods in terms of making the best choice, making easy and fast decisions, and combining objective and subjective decisions. Microsoft Excel was used to solve the analysis and as a result of the analysis made among the main criteria, the consistency index was found to be 0.1 > 0.0780. According to this result, the criterion with the highest weight was 0.3372 (chemical (oxidizing, corrosive, allergic, mutagen, etc.) and biological (bacteria, virus, infectious diseases, etc.) risks.) This was followed by 0.2683 ergonomic risks, 0.2208 employee-related risks, and 0.1737 physical hazards, respectively. In the study, a different method was applied rather than standard risk assessment methods and checklists. With the AHP method, the risks are ranked from the highest to the lowest. Thus, both the identification and prioritization of risks were made.

Background: Human and technology factors in healthcare can lead to errors that threaten human life. A strong patient safety culture in hospitals may be linked to better patient outcomes. Because of the potential for mistakes in surgical departments, surgical nurses are critical to establishing and maintaining a patient safety culture. Aim: The aim of this study was to explore the patient safety culture perceptions of surgical nurses. Methods: This descriptive study was performed with 206 nurses working in the surgical clinics and operating rooms of 1 university hospital, 2 public hospitals, and 1 private hospital. A “Questionnaire” and the “Hospital Survey on Patient Safety Culture” were used for data collection. The data were analyzed using mean, number, and percentage values, 1-way analysis of variance, Tukey Honestly Significant Difference (HSD), and chi-square tests. Results: The study revealed that surgical nurses’ patient safety culture perception was at a medium level. The highest mean rate of positive responses in The Hospital Survey on Patient Safety Culture came from “Teamwork within units” and the lowest mean rate of positive answers came from the “Non-punitive response to errors” subdimension. In addition, 85.7% of the surgical nurses did not report any errors in the previous 1 year, and nurses who had been working for ≥11 years submitted more error reports (P = .001). Conclusion: Nurses’ patient safety culture perception was at a medium level. Recommendations for improving patient safety culture should include on-the-job training programs and improvements in employees’ working conditions.

Bostan, Sedat; Nenni Unal, Ferhan; Unal, Cihan; Metin, Fatma Elçin

Enlightened consent is generally regarded as the process by which open information communication is available to a patient or an experimental individual to decide whether a particular treatment should be performed or to participate in a particular experiment. Informing the patient about the disease, the physician develops a sense of trust among the patient and increases the well-being of the patient. This is of moral importance in terms of showing that the patient's decision is respected and that the patient can make a decision freely. The aim of this study; The aim of this study is to investigate the awareness levels of patients about enlıghtened consent and how they define enlıghtened consent. For this purpose, 400 individuals living in Erzurum were surveyed. In the study, Enlightened Consent Attitude and Awareness Scale developed by Bostan was used. The quantitative data were analyzed by frequency and difference analysis. According to the findings of the research; A significant difference was found between the groups of gender variable, the number of hospital visits variable groups, and the profession variable groups. It was found that there was a significant difference between age variable groups, education level variable groups, income level variable groups in the answers given to the questions within the awareness factor. According to the results of the research; Although the awareness level of the participants is high, it can be said that they do not have enough opportunities to use these rights in hospitals and health institutions.

Public Hospital Union Construction and Changing Patient Rights Board Effects on Board Decisions

    Bostan, Sedat

The concept of patient rights, 1980s went into the literature, developed in 1990, and began to be implemented in health institutions in the 2000s. Thus, a legal infrastructure has gained, institutionalized in 2010 years. The purpose of this study is to investigate patient rights violations through the decisions of the patient rights board, taking into account the change in board structure and public hospital union structuring. The survey was conducted on board decisions taken in 2012- 2015. The data were processed with cross-table technique and the difference between the years was tried to be monitored by Pearson Chi-square test. Public hospital union structure has reduced written complaints of patients and strengthened on-site solutions. When the causes of patient com- plaints are examined, half is concerned with the right to benefit from the service, and issues of re- spect, medical care and information are at the forefront. Approximately half of the complaints are consisted of physicians and one in ten nurses. Significant reductions have been observed in com- plaints of medical secretaries, cleaners, security personnel and administrative staff when there has been a significant increase in complaints of dentists in the last year. According to findings; the in- tegration of patient rights boards within the health directorates and the change of member struc- ture contributed to making decisions more professionally and objectively. As a result, patient privacy violations that have been acknowledged by the system and the patients, is destroying human val- ues. Lack of patient information reduces treatment effectiveness. Health worker's seizure-prone work are increasing their exhaustion and triggers insecurity in medical care. Health administrators and educators should be very interested in these issues.

    Orijinal araştırma

    Bostan, Sedat; Çolakc, Tuğba

AABBSS TTRRAACCTT The aim of this study is to assess the attitudes, thoughts and views of media workers in relation to the patient rights implementations that have been practiced since 2014 in hospitals in Turkey. As the patient rights is a relatively emerging and social concept in Turkey, it is covered in various ways by the news media. As the coverage ways have important influence over the for- mation of social perception, individual attitudes of media workers gain importance. Thus we made a survey on media workers in Trabzon city center with the aim to assess their personal views and attitudes. The obtained data has been analyzed by SPSS and yielded findings suggesting that although media workers were seeing patient rights as a rising value, they needed sufficient information on the topic. It was also found that a difference appeared between the attitudes of those media workers who had read or written about patient rights and those who hadn't. It is awaited that a positive coverage could support patient rights practices to get more influential in the health care process which in turn would foster more convenient conditions for patients and patients' relatives for appealing to patient communication units. Efforts for informing media workers would be influential on a media cover- age that could foster a peaceful relationship between health care workers and patients.
