As material science is integrated into medical science as in other fields, the variety, development and usage areas of biomaterials increase. One of the biomaterials in this variety is NiTi alloys. This study was focused on the coating of TiO2 layer by using the method of Micro Ark Oxidation with the aim of the improvement of surface and mechanical properties of Shapedmemory and bio material NiTi alloy. Structural properties of the coatings were analysed by of SEM and XRD. In vitro ability was investigated by soaking the coated NiTi samples in simulated body fluid (SBF) at temperature 37 degrees C for various time periods. After soaking, Ni+2 release was applied to the media where coated and uncoated shape-memory NiTi samples are available, and its control was made by means of ICP-MS device. The results also show that Toxic and carcinogenic effects of Ni+2 release is significantly reduced by the TiO2 coating. Thus, it is observed that the properties that limit the use of NiTi alloys has been substantially fixed by MAO coating. It was found that the bioactive properties of the TiO2 coated NiTi substrate increased by MAO method and at the end of the 72 hour, there was no statistically any cytotoxicity in cell viability compared to the NiTi substrate.

İncele - MakaleYalnız künye2021Scopus SJRQ1Challenges of Setting up a Lower Extremity Reconstruction Practice in a Constrained Environment
Datli, Asli; Karasoy, Ismail; Genc, Yucel; Pilanci, Ozgur
Background Microsurgical lower extremity reconstruction remains challenging, especially when resources are limited such as lack of proper equipment, human resources, administrative support, and located in a remote area far from tertiary care. Nevertheless, reconstructive solutions are required, especially when in urgent trauma situations. In this article, we evaluate ways of overcoming challenges and issues that should be considered in a newly established unit by sharing our initial lower extremity reconstruction experience. Methods We report a local hospital's initial lower extremity reconstruction experience in February 2017 to January 2018. Through a total of seven patients, we tried to enhance the environment, instruments, nurses' contribution, and perspective of the peers and community in terms of factors related to the surgeon, hardware, environment, supporting faculty, reimbursement, and patients. Results Four patients underwent reconstruction with a freestyle propeller flap and three with an anterolateral thigh flap; in one case, a superficial circumflex iliac artery perforator flap was chosen to salvage partial flap necrosis. Increased experience of the surgeon, new equipment, continuing nurse/patient education, and collaborating with other departments allowed us to choose more challenging flaps and be more meticulous while decreasing the operation time and hospital stay. Conclusion To start a lower extremity reconstruction practice in a resource-poor environment, the surgeon needs to evaluate the relevant factors; moreover, he or she should continuously improve them until a working methodology is achieved. Despite all the challenges, the adaptations learned at this center can be applied to other local hospitals around the world to set up a lower extremity reconstruction practice and improve its outcomes.

- MakaleAçık Erişim2021Effect of precautionary and normalization steps on course of COVID-19 pandemic in Turkey
Bostan, Sedat; Agac, Gokhan; Sahinöz, Saime
Aim: The COVID-19 pandemic began in China and has now spread across the whole world, and many countries such as Turkey are still fighting against this pandemic. The aim of this study was to investigate the impact of precautionary and normalization steps taken by the Turkish government on the number of confirmed cases, current cases, and deaths due to the COVID-19 in Turkey. Materials and Methods: The study used exponential regression models to estimate the number of confirmed cases, deaths, and current cases. The study data were collected from the Turkish Ministry of Health, Turkish Ministry of Internal Affairs, and the national public broadcaster of Turkey. Results: The precautions taken during the COVID-19 pandemic have caused the exponentially increasing number of cases to be brought under control and to decrease exponentially. The normalization steps taken paused the exponential decrease in the number of cases and caused the number of cases to remain flat during the summer months and then rise again. If the normalization steps were not implemented and the precautions taken continued in the same way, it was calculated that the numbers of COVID-19 current cases would be reset on June 26, 2020. The total number of deaths would remain at 4423 and the total number of confirmed cases would be 150586. Discussion: As a result, the impact of precautionary and normalization steps on the course of the COVID-19 pandemic was examined. It has been observed that the impact of these steps on the pandemic is exponential. The results provide guidelines for health authorities and administrators to take necessary precautions.

İncele - MakaleAçık Erişim2021Scopus SJRQ3Erectile dysfunction treatment with Phosphodiesterase-5 Inhibitors: Google trends analysis of last 10 years and COVID-19 pandemic
Deger, Muslim Dogan; Madendere, Serdar
Objectives: We aimed to analyze the trend change of the most popular Phosphodiesterase-5 Inhibitors (PDE5i) over time and geography by using Google Trends (GT) data in 10 years period and COVID-19 pandemic. Materials and methods: GT is able to generate a line-graph, showing how interest has increased or decreased over a period within specific territories. The search values for specific terms are indexed as relative search volume (RSV), which is presented on a scale from 0-100. Avarage annual percentage change (AAPC) and RSV were analyzed to evaluate gain or loss of interest in trends. Search terms were generated for Food and Drug Administration (FDA)-approved PDE5i; tadalafil, sildenafil, vardenafil, avanafil, and their most-used brand names. The data was within worldwide from 1 January 2010, to 31 December 2020, using the global query category. Results: The overall interest in PDE5i has doubled. Sildenafil has become the most trend PDE5i of today with a regular increase (AAPC: 0.016, p < 0.01). Although the search trend of tadalafil remained almost constant until 2014, the rate of increase in the last 6 years raised and tadalafil has become the 2nd most popular PDE5i recently (AAPC: 0.007, p < 0.01). For vardenafil there has been a decreased interest (AAPC: -0.009, p < 0.01). There is no significant change in avanafil trend (AAPC: 0.000, p: 0.5). All PDE5i interest on GT decreased notably from February to June 2020. But after June, search trends reached the level before the COVID-19 period in a month. Conclusions: These findings show us, with its increasing prevalence, erectile dysfunction (ED) has become a major public health problem. People from different geographies search the Internet for ED treatment options. Patients should be informed that ED may be the first sign of many comorbid diseases, and patients with ED should be referred to a health institution for diagnosis and treatment.

- MakaleAçık Erişim2021Relationship between androgen levels and neutrophil/lymphocyte ratio in patients diagnosed with hirsutism
Sayilan, Samet; Mert, Meral; Kulakac, Nursen
Aim: Hirsutism is a disease involving increased terminal hair growth in women with a male-type distribution pattern in androgen-sensitive areas such as the chin, upper lip, chest, abdomen, back, and thighs. Our review of the available literature revealed no previous studies concerning the role of the peripheral blood neutrophil/lymphocyte ratio (NLR), an easily calculated and noninvasive marker, in hirsutism. The purpose of this research was to determine the relationship between androgen levels and NLR in female patients diagnosed with hirsutism. Material and Methods: The files of 392 patients presenting to the Bakirkoy Dr. Sadi Konuk Education and Research Hospital due to hirsutism between 2018 and 2020 were included in this cross-sectional, retrospective study. Patients' hirsutism and menstrual cycles, age at menarche, number of pregnancies in the case of married women, number of living children, and history of infertility were investigated. Hyperandrogenemia findings, such as hair loss and acne, were also recorded. Patients' biochemical and complete blood count parameters were evaluated. Results: The mean age of the participants was 26.27 +/- 6.37 years (min-max: 16-48), and their androgen levels were statistically significantly associated with high-density lipoprotein, hemoglobin, hematocrit, 17-hydroxyprogesterone, and HbA1c levels (p<0.05). However, no significant relationship was found between patients' androgen levels and NLR values (p>0.05). Discussion: No significant relationship was determined in this study between androgen levels and NLR values in women diagnosed with hirsutism. Due to the limited numbers of previous similar studies, further research with larger patient numbers is now needed.

This study aimed to analyze preferred leg (PLs) and non-preferred leg (NPLs) isokinetic knee strength, lateral asymmetry ratios and to examine their correlation with static balance. Sixteen female taekwondo (TKD) athletes voluntarily participated in the study. Knee isokinetic extension (Ex) and flexion (Flx) strength were measured in concentric / concentric (Con / Con) contractions at angular velocities of 60 degrees/s, 180 degrees/s, and 240 degrees/s. Ipsilateral hamstring / quadriceps (H/Q) and bilateral (H/H and Q/Q) ratios were calculated. Static balances were determined by taking the center of body pressure in X (COPX) and Y (COPY) axis. Paired sample t-test and Pearson correlation tests were used in statistical analysis. When the isokinetic knee strength was examined, it was found that 60 degrees/s angular velocity parameter was significantly different in Flx phase while 180 degrees/s and 240 degrees/s angular velocities were found to be significantly different in Ex phase in favor of PLs. There was no significance in bilateral and ipsilateral strength ratios of all angular velocities. There was a significant correlation between 240 degrees/s HQ and COPX in terms of ipsilateral asymmetry ratios whereas a significant correlation was found between 60 degrees/s HH and COPX in terms of bilateral asymmetry ratios. It was found that there were differences in strength between PL and NPLs, but no asymmetry was observed. There was no high level of correlation between lateral asymmetry ratios and balance in female TKD practitioners.

Bilge, Aysegul; Karasoy, Ismail; Neziroglu, Elif; Güner, Yesim
Although novel coronavirus-2019 (COVID-19) primarily affects the respiratory system, it can affect multiple organ systems, leading to serious complications, such as acute respiratory distress syndrome (ARDS) and multiple organ failure. Nearly 20 to 55% of patients with COVID-19 experience coagulation disorders that cause high mortality in line with the severity of the clinical picture. Thromboembolism can be observed in both venous and arterial systems. The vast majority of thromboembolic events are associated with the venous system and are often observed as pulmonary embolism. Arterial thromboembolisms often involve the arteries in the lower extremities, followed by those in the upper extremities. Herein, we report a rare case of COVID-19 pneumonia whose left arm was amputated at the forearm level after arterial thromboembolism in the left upper extremity. This case report is valuable, as it is the first reported case of upper extremity arterial thromboembolism in Turkey, as well as the only case in the literature in which the patient underwent four surgical interventions and is still alive.

Ayazoglu Demir, Elif; Demir, Selim; Turan, Ibrahim
Ethyl pyruvate (EP) is a simple aliphatic ester derived from pyruvic acid which is an endogenous metabolite. Although various studies have investigated the antioxidant and anti-inflammatory properties of EP, there has been only limited research into the cytotoxic effect of EP on cancer cells. The aim of this study was to determine the cytotoxic effects of EP on cells representing common cancer types. EP was purchased commercially and intermediate stock solutions were prepared with phosphate buffer saline. The cytotoxic effect of EP on human melanoma (VMM917), cervix (HeLa), breast (MCF-7), lung (A549), liver (HepG2), colon (WiDr) cancer and normal fibroblast (BJ) cells was determined using the MTT assay. Cisplatin was used as a positive control in cytotoxicity experiments. The results showed that EP exhibits selective cytotoxic effect on VMM917 (10.1-fold) and HeLa (3.04-fold) cells compared to BJ cells. This study shows for the first time that EP has a highly selective cytotoxic effect, especially on melanoma and cervix cancer cells. The mechanism of this effect needs to be elucidated by more extensive studies.

İncele - MakaleAçık Erişim2021Scopus SJRQ3Does evaluation of tumor volume or/both origination site better guide to optimal surgery for inverted papilloma?
Sahin, Muammer Melih; Yilmaz, Metin; Zorlu, Mehmet Ekrem; Göcek, Mehmet; Düzlü, Mehmet; Sayar, Erolcan; Ceylan, Alper
Introduction: Sinonasal inverted papilloma is noted for its high rate of recurrence. Staging systems aid to reduce recurrence and avoid excessive surgeries by guiding the selection of the optimal surgical approach. Objective: To evaluate the effectiveness of different endoscopic approaches in inverted papilloma by assessing tumor origin site and tumor volume. Methods: Krouse classification system that is based on tumor volume was used for staging; furthermore, tumor origin sites were grouped as lateral nasal wall, medial wall and other walls of maxillary sinus. The main treatment method for all patients was endoscopic sinus surgery. Endoscopic extended middle meatal antrostomy, endoscopic Caldwell-Luc and endoscopic medial maxillectomy were the additional surgery types performed in different combinations. Results: Fifty-five patients (42 male) with a mean 54.9 +/- 14.4 years of age were included. 37 patients were diagnosed with advanced stage inverted papilloma (67.2 %). Recurrence was observed in 12 patients (21.8 %). In early stage lateral nasal wall origination, no recurrence was observed in the simple tumor resection group (0/10). In early stage medial wall origination, no recurrence was observed in the extended middle meatal antrostomy group (0/8). In advanced stage medial wall origination, the recurrence rates of extended middle meatal antrostomy, extended middle meatal antrostomy + endoscopic Caldwell- Luc and endoscopic medial maxillectomy were 100.0 %, 53.8 % and 13.6 %, respectively (p =0.002). In advanced stage other walls of maxillary sinus origination, recurrence rates of extended middle meatal antrostomy + endoscopic Caldwell-Luc and endoscopic medial maxillectomy were 20 % and 16.6 %, respectively (p =0.887). Conclusion: Tumor origin site, tumor stage and surgery types show an impact on recurrence. Despite the fact that tumor origin site singly could lead to appropriate selection of the surgery type in most cases, tumor stage carries substantial importance in selection of surgery type for sinonasal-inverted papilloma. An operation plan regarding both tumor volume and tumor origin site may aid surgeons in selecting optimal endoscopic surgical method to avoid recurrence or excessive surgeries. Erol, Ismail; Ar, Ilker Murat; Ozdemir, Ali Ihsan; Peker, Iskender; Asgary, Ali; Medeni, Ihsan Tolga; Medeni, Tunc
Purpose The objective of this study is to assess quantitatively how feasible blockchain is for various industries, such as logistics and supply chain, health, energy, finance, automotive, pharmaceutical and agriculture and food using a comprehensive list of indicators. Design/methodology/approach A decision aid was applied to the problem of identifying the feasibility of blockchain in Turkish industries. To this end, first, a set of indicators was identified. Then, the fuzzy AHP and fuzzy TOPSIS were utilized to assess the feasibility comparatively using the data gathered from a group of experts. Finally, a scenario analysis was conducted to ensure the consistency of our evaluation. Findings The findings of this study suggest that comparatively, logistics and supply chain, finance and health industries are the most feasible industries for blockchain. This study further suggests that blockchain is the least feasible for the automotive industry compared to the rest of the identified industries. Research limitations/implications It is cumbersome to find out the respondents who have sufficient knowledge of both blockchain and the identified industries. Even if we took the utmost care in identifying the right respondents, we limited our search to the biggest industrial hubs of Turkey. Practical implications The findings of this research may help various decision-makers employed in governments, conglomerates, software and consulting firms and national research institutions make more informed decisions and allocate their resources more effectively. Originality/value To this date, the current studies have solely investigated possible research opportunities in blockchain and demonstrated several blockchain applications in stand-alone cases. To the best of our knowledge, however, no single study exists that evaluates the feasibility of blockchain comparatively and holistically among a group of industries using various indicators.

Composite coatings were deposited on AZ91 magnesium alloys by micro arc oxidation (MAO) and the MAO-hydrothermal treatment (HT). The MAO process of Mg alloys was performed in the silicate/phosphate-based electrolyte with graphite particles size of 5-10 and 75 µm and then the obtained samples were submitted to hydrothermal treatment at 150°C in the graphite-ethanol mixture. The structural and morphological properties of composite coatings were analized X-ray diffraction (XRD) and scanning electron microscopy (SEM). The wear resistance was examined with ball-on-disc tribometer. The addition of graphite to the MAO electrolyte solution increased the coating thicknesses and compactness and the surface morphologies acquired relatively a homogeneous appearance. The coating surfaces were enriched in graphite by HT and the wear test results showed that the wear resistance of the graphite-doped MAO-HT composite coatings was significantly improved.

- MakaleYalnız künye2021Scopus SJRQ2Innovative and polygonal trend analyses applications for rainfall data in Vietnam
Şan, Murat; Akçay, Fatma; Linh, Nguyen Thi Thuy; Kankal, Murat; Pham, Quoc Bao
It is a known fact that the size, frequency, and spatial variability of hydrometeorological variables will irregularly increase under the impact of climate change. Among the hydrometeorological variables, rainfall is one of the most important. Trend analysis is one of the most effective methods of observing the effects of climate change on rainfall. Recently, new graphical methods have been proposed as an alternative to classical trend analysis methods. Innovative Polygon Trend Analysis (IPTA), which evolved from Innovative Trend Analysis (ITA), is currently one of the proposed methods and it does not contain any assumptions. The aim of this study is to compare IPTA, ITA with the Significance Test and Mann-Kendall (MK) methods. To achieve this, the monthly total rainfall trends of 15 stations in the Vu Gia-Thu Bon River Basin (VGTBRB) of Vietnam have been examined for the period 1979–2016. The analyses show that rainfall tends to increase (decrease) in March (June) at nearly all stations. IPTA and ITA with the Significance Test are more sensitive than MK in determining the trends. While trends were detected in approximately 90% of all months in IPTA and ITA with the Significance Test, this rate was only 23% in the MK test. Although the arithmetic mean graphs in the 1-year hydrometeorological cycle are considerably regular at almost all stations, their standard deviations are relatively irregular. The most critical month for trend transitions between consecutive months for all the stations is October, which has an average trend slope of −1.35 and a trend slope ranging from −3.98 to −0.21, which shows a decreasing trend.
