- MakaleAçık Erişim2021A Sociological Criticism of the Global Health Discourse: The Pandemic as a Metaphor of Contemporary Culture
Demir, Sertac Timur
This article argues that a clear correlation exists between disease and the prevailing social functioning. In this respect, every disease can be seen as a metaphor for the age and society in which it emerges. In other words, every period carries a form of disease that bears traces of the dominant lifestyle. This study approaches the epidemic as a kind of sociological projection in the context of studying modern culture and continues the discussion along the lines critically opened by Susan Sontag (2005). lust as Sontag examined disease as a metaphor, this article debates the discourses on masks, social distancing, and hygiene within the scope of COVID-19 along a specific sociological axis around the concepts of speed, control, information, hyper-medicalization, and death as the dominant indicators of contemporary culture. For this purpose, the study seeks answers to the following questions through the leading social theorists of the field such as Bauman, Baudrillard, Turner, Furedi and Chul-Han: How does the global epidemic and the current health discourse embodied in it appear within the sociological context? What thoughts and tendencies emerge when examining the pandemic portrait as a social metaphor? For example, what kind of relationship exists between hygiene and cultural fears; among distance, social isolation, and avoidance; and between masks and the superficial idea of death concealed behind quantification? Undoubtedly, the pandemic is a critical breaking point in the history of modern medicine and has forced not only global health practices but also many value judgments to both paradoxically be accepted and questioned.

Acar, Emrah; Kilicgedik, Alev; Izci, Servet; Inanir, Mehmet; Yilmaz, Mehmet Fatih; Gokce, Mustafa; Izgi, Ibrahim Akin; Kirma, Cevat
Purpose Acute pulmonary embolism is a life-threatening cardiothoracic emergency leading to right ventricle systolic and diastolic dysfunction. In the present study, we investigated the right ventricle diastolic function and its predictive value in patients with acute pulmonary embolism. Material and methods Were prospectively recruited in this study 621 patients diagnosed with acute pulmonary embolism between December 2015 and June 2019. Among them, 173 were excluded, leaving 448 patients for follow-up. Transthoracic echocardiography was performed for the evaluation of the right ventricle systolic and diastolic indices. At 30-day follow-up, the patients were allocated either into the benign or in the complicated clinical course group. Results The group with complicated clinical course had higher value of pulmonary artery systolic pressure (PASP), right/left ventricle diameter ratio, E-wave velocity, E/A ratio, A ' wave velocity, E/E ' ratio, and time interval between E and E ' waves (P < .05), but lower values of tricuspid annular plane systolic excursion, A-wave velocity, isovolumetric relaxation time, deceleration time, E ', and E '/A ' (P < .05). Complicated clinical course was associated with shorter isovolumetric relaxation time (r = .564, P < .001), E/E ' (r = .495, P < .001), and TE-E ' (r = .596, P < .001). Receiver operator characteristic curve analysis showed that TE-E ' had the largest area under curve (TE-E ' 0.82, IVRT 0.77, E/E ' 0.72). A cut-off value of TE-E ' = 70 milliseconds had an 81.1% sensitivity and 71.4% specificity for the prediction of the complicated clinical course. Conclusion The assessment of the right ventricle diastolic function could predict the complicated clinical course in patients with acute pulmonary embolism.

Idil, Eral; Aydin, Ali Ekrem; Bulut, Esra Ates; Isik, Ahmet Turan
Aims: Older adults are at risk for polypharmacy, which has multidimensional safety, clinical and economic implications. Therefore, the optimization of drug therapy is one of the critical components of geriatrics clinical practice. This study is aimed to investigate the effect of drug rationalization on comprehensive geriatric assessment(CGA) parameters. Materials and Methods: The study was a retrospective and longitudinal study examining the effect of change in drug number on functionality and physical performance. A total of 515 patients were included in the study. Detailed medication history, laboratory findings, CGA parameters were recorded in the first admission. Polypharmacy was accepted as concurrent usage of five or more drugs. Medications of each patient were reviewed with the guidance of CGA and Beers Criteria. Results: The mean age of patients was 74.13 +/- 7.29 years, and 68.7% were female. The baseline mean number of drugs was 5.11 +/- 3.34. The polypharmacy group(269 patients) had a higher rate of geriatric syndromes and lower CGA scores in the first admission. After optimization of medications, the mean drug number decreased to 4.76 +/- 2.72. Depression, mobility and nutritional scores improved at the end of six months in the patients whose total number of drugs decreased, while global cognition, activities of daily living scores, and gait speed were preserved. Increasing the number of drugs was associated with lower mobility and functionality. Conclusion: Drug rationalization guided with CGA improves the nutritional, physical, and psychosocial status of geriatric patients. Thus, medication reviews have key importance in the management of older patients.

- MakaleYalnız künye2021Scopus SJRQ2Osteoclastic activity in chronic otitis media with cholesteatoma-related bone destruction
Özgur, A.; Yemis, T.; Basbulut, E.; Turgut, N. F.; Özdemir, D.; Akgul, G.; Mehel, D. M.; Celebi, M.
Background Cholesteatoma-related bone destruction is the cause of many complications due to chronic otitis media. This study aimed to evaluate osteoclastic activity in cholesteatoma-related bone destruction using tartrate-resistant acid phosphatase 5b, an enzyme specific to osteoclastic activity. Method Seventy-two patients diagnosed with chronic otitis media were included in this study and were divided into two groups: with and without bone destruction. The blood serum and tissue tartrate-resistant acid phosphatase 5b levels from both groups were compared. Results There were no significant differences in the level of serum enzymes between both groups. However, in tissue samples, tartrate-resistant acid phosphatase 5b levels were significantly lower in the bone destruction group than the group without bone destruction. Conclusion This study determined that the level of tartrate-resistant acid phosphatase 5b, a specific enzyme for osteoclastic activity in cholesteatoma-related bone destruction, is locally decreased. This data suggests that osteoclastic activity may decrease in cholesteatoma-related bone destruction. However, further experimental and clinical studies are required to clarify this highly complex mechanism.

Purpose To evaluate the potential toxicity of operation microscopes with halogen and light emitting diode (LED) light source on the rabbit eyes. Materials and methods Thirty-two eyes of 16 male New Zealand pigmented rabbits were involved in the study. The rabbits were divided into two groups according to the type of light source applied. Only one eye of each rabbit was exposed to illumination light, unexposed fellow eyes served as the control group. Experimental groups included group 1 exposed to halogen light for 2 h and evaluated 1 day and 1 week after the illumination, group 2 exposed to LED light for two hours and evaluated 1 day and 1 week after the illumination. On the first and seventh days after exposing the light, we evaluated the rabbit corneas using in vivo confocal microscopy (IVCM). At the end of the seventh day, the Hematoxylin-eosin staining and TUNEL staining were performed to investigate the presence of apoptosis in the retina and retina pigment epithelium. Results Early IVCM findings revealed corneal epithelial cell ovalization and indistinct intercellular borders in the halogen light group. We also observed more increase in the keratocyte density index (23.7% vs 14.1%, p = 0.001, respectively) and the Bowman reflectivity index (12.4% vs 4.1%, p = 0.001, respectively) at first day of the light exposure in halogen light group compared to LED light group. However, late IVCM indicated that these findings disappeared one week later. No apoptosis was observed in the corneal and retinal layers in early and late examination groups. Conclusion The present experimental study demonstrated that both halogen and LED lights, which were commonly used for microscopic eye surgery, had no sustained adverse effect on the cornea and retina of the rabbits; however, halogen light had a temporary adverse effect on corneal epithelium and stroma, which resolved within 1 week.

Objective: The goal of this study was to investigate Turkish urologists' role and attitude toward the systemic treatment of urologic cancers. Materials and Methods: An 18-item survey was e-mailed to 2305 certified Turkish urologists. The survey included questions designed to evaluate urologists' knowledge of chemotherapy procedures and their ability to manage complications, as well as how many of them perform systemic therapies and believe urologists should perform these therapies for urologic cancers. Results: We evaluated 305 responses. Most urologists (89.6%) perform hormonotherapy for prostate cancers. Further, the majority refer patients to medical oncologists for chemotherapy. Moreover, 65.9%, 70.8%, and 72.1% of the urologists believe that only medical oncologists should deliver chemotherapy for prostate, bladder, and testicular cancers, respectively. Only 23.9% of urologists believe they are competent to perform chemotherapy, while 16.7% believe they are capable of dealing with chemotherapy complications. Conclusion: Turkish urologists have a lack of performance and interest in administering systemic therapies in urologic cancers. We want to emphasize that urology is more than just surgery. Urologists should be involved in all stages of cancer treatment.

- Derleme MakaleAçık Erişim2021Scopus SJRQ4The New Disease of the Modern Age: What Is the Fear of Missing Out FOMO? A Review
Erdogan, Ali; Yilmaz, Yavuz; Hocaoglu, Cicek
Social networks have become a very important part of human life in recent years. This situation has led to new definitions such as the Fear of Missing Out (FOMO). FOMO can be defined as the concern that others can have a good experience when the person is absent and therefore the person wants to stay on social networks constantly to avoid missing anything. It is especially common in young people. Problematic internet use, psychosocial factors, social network use and anxiety are blamed in its etiology. Family conflicts, loneliness, depression, and low self-efficacy can be said as risk factors. People constantly want to spend time on social networks, daily life activities are blocked and the person's productivity decreases. It has no place in diagnostic classifications. For FOMO, the fear of missing out scale developed by Przybylski can be used. It is reported that FOMO has a significant positive relationship with nomophobia. There is no specific treatment method. There is a need for extensive research on FOMO.

- MakaleYalnız künye2021Scopus SJRQ1Challenges of Setting up a Lower Extremity Reconstruction Practice in a Constrained Environment
Datli, Asli; Karasoy, Ismail; Genc, Yucel; Pilanci, Ozgur
Background Microsurgical lower extremity reconstruction remains challenging, especially when resources are limited such as lack of proper equipment, human resources, administrative support, and located in a remote area far from tertiary care. Nevertheless, reconstructive solutions are required, especially when in urgent trauma situations. In this article, we evaluate ways of overcoming challenges and issues that should be considered in a newly established unit by sharing our initial lower extremity reconstruction experience. Methods We report a local hospital's initial lower extremity reconstruction experience in February 2017 to January 2018. Through a total of seven patients, we tried to enhance the environment, instruments, nurses' contribution, and perspective of the peers and community in terms of factors related to the surgeon, hardware, environment, supporting faculty, reimbursement, and patients. Results Four patients underwent reconstruction with a freestyle propeller flap and three with an anterolateral thigh flap; in one case, a superficial circumflex iliac artery perforator flap was chosen to salvage partial flap necrosis. Increased experience of the surgeon, new equipment, continuing nurse/patient education, and collaborating with other departments allowed us to choose more challenging flaps and be more meticulous while decreasing the operation time and hospital stay. Conclusion To start a lower extremity reconstruction practice in a resource-poor environment, the surgeon needs to evaluate the relevant factors; moreover, he or she should continuously improve them until a working methodology is achieved. Despite all the challenges, the adaptations learned at this center can be applied to other local hospitals around the world to set up a lower extremity reconstruction practice and improve its outcomes.

- MakaleAçık Erişim2021Effect of precautionary and normalization steps on course of COVID-19 pandemic in Turkey
Bostan, Sedat; Agac, Gokhan; Sahinöz, Saime
Aim: The COVID-19 pandemic began in China and has now spread across the whole world, and many countries such as Turkey are still fighting against this pandemic. The aim of this study was to investigate the impact of precautionary and normalization steps taken by the Turkish government on the number of confirmed cases, current cases, and deaths due to the COVID-19 in Turkey. Materials and Methods: The study used exponential regression models to estimate the number of confirmed cases, deaths, and current cases. The study data were collected from the Turkish Ministry of Health, Turkish Ministry of Internal Affairs, and the national public broadcaster of Turkey. Results: The precautions taken during the COVID-19 pandemic have caused the exponentially increasing number of cases to be brought under control and to decrease exponentially. The normalization steps taken paused the exponential decrease in the number of cases and caused the number of cases to remain flat during the summer months and then rise again. If the normalization steps were not implemented and the precautions taken continued in the same way, it was calculated that the numbers of COVID-19 current cases would be reset on June 26, 2020. The total number of deaths would remain at 4423 and the total number of confirmed cases would be 150586. Discussion: As a result, the impact of precautionary and normalization steps on the course of the COVID-19 pandemic was examined. It has been observed that the impact of these steps on the pandemic is exponential. The results provide guidelines for health authorities and administrators to take necessary precautions.
