Purpose Acute pulmonary embolism is a life-threatening cardiothoracic emergency leading to right ventricle systolic and diastolic dysfunction. In the present study, we investigated the right ventricle diastolic function and its predictive value in patients with acute pulmonary embolism. Material and methods Were prospectively recruited in this study 621 patients diagnosed with acute pulmonary embolism between December 2015 and June 2019. Among them, 173 were excluded, leaving 448 patients for follow-up. Transthoracic echocardiography was performed for the evaluation of the right ventricle systolic and diastolic indices. At 30-day follow-up, the patients were allocated either into the benign or in the complicated clinical course group. Results The group with complicated clinical course had higher value of pulmonary artery systolic pressure (PASP), right/left ventricle diameter ratio, E-wave velocity, E/A ratio, A ' wave velocity, E/E ' ratio, and time interval between E and E ' waves (P < .05), but lower values of tricuspid annular plane systolic excursion, A-wave velocity, isovolumetric relaxation time, deceleration time, E ', and E '/A ' (P < .05). Complicated clinical course was associated with shorter isovolumetric relaxation time (r = .564, P < .001), E/E ' (r = .495, P < .001), and TE-E ' (r = .596, P < .001). Receiver operator characteristic curve analysis showed that TE-E ' had the largest area under curve (TE-E ' 0.82, IVRT 0.77, E/E ' 0.72). A cut-off value of TE-E ' = 70 milliseconds had an 81.1% sensitivity and 71.4% specificity for the prediction of the complicated clinical course. Conclusion The assessment of the right ventricle diastolic function could predict the complicated clinical course in patients with acute pulmonary embolism.

Introduction The major salivary glands can be stimulated by chewing gum to increase saliva flow and decrease xerostomia. The aim of this study was to investigate the effect of chewing gum on dry mouth, interdialytic weight gain, and intradialytic symptoms in hemodialysis (HD) patients. Methods This prospective randomized controlled single-blind study was conducted with patients who had been treated for at least 6 months with sessions 3 days a week for 4 hours at two HD units. Patients were randomly allocated to chewing gum group or the control group. In the chewing gum group, gum was chewed for 10 minutes six times a day, and when the patients felt mouth dryness or were thirsty. In the nonchewing gum group, gum was not chewed. The patients were followed-up for 3 months. A total of three saliva samples were taken before starting treatment at the first, 12th, and 36th HD session. Data were collected with the Visual Analogue Scale (VAS), Hemodialysis Patients Fluid Control Scale, Dialysis Symptom Index, and Hospital Anxiety and Depression Scale at baseline, week 4, week 8, and week 12. Findings The study was completed with a total of 44 patients consisting of 22 patients in the each group. The second and third month VAS values (xerostomia) of the patients in the chewing gum groups were statistically significantly lower than those in the control group (P= 0.014,P < 0.001, respectively). The third month salivary flow rate in the chewing gum group was higher than the values in the control group patients (P < 0.001). Discussion It is anticipated that this study will raise nurses' awareness of dry mouth and encourage future studies on interventions to increase the salivary flow rate to prevent or treat dry mouth.
- MakaleYalnız künye2020Hepatic venous Doppler assessment can anticipate simplified pulmonary embolism severity index and right ventricle dysfunction in patients with acute pulmonary embolism
Acar, Emrah; Izci, Servet; Inanir, Mehmet; Yilmaz, Mehmet F.; Izgi, Ibrahim A.; Gokce, Mustafa; Kirma, Cevat
Purpose Acute pulmonary embolism (APE) is a life-threating cardiothoracic thromboembolic emergency in which right ventricle dysfunction (RVD) is a major concern. In the present study, we examined the hepatic veins (HVs) blood flow with pulsed-wave spectral Doppler ultrasonography to determine its relationship with the simplified pulmonary embolism severity index (sPESI) and the patient's RVD status. Methods We divided the 243 patients who met the inclusion criteria into two groups based on both their sPESI scores and their RVD status. Transthoracic echocardiography was performed to evaluate the RVD and the HVs within 1 hour after patient admission. The liver was evaluated using subcostal and intercostal echocardiographic windows in grayscale B-mode, and HVs were assessed using color and spectral Doppler assessment though the same echocardiographic windows. Result A cut-off value of the systolic reverse flow velocity-time integral (SrVTI) = 2.2 cm carried a sensitivity and specificity of 84.29% and 74.89%, respectively, for the prediction of sPESI >= 1. A SrVTI cut-off value of 2.1 cm yielded a sensitivity and specificity of 83.03% and 73.91%, respectively, for the prediction of RVD. Conclusion HV Doppler assessment could be a useful method for anticipating the sPESI and the presence of RVD in patients with APE. In addition, it may provide information regarding the hemodynamic impact of APE.

Objective Studies focusing attention on the effects of environmental pollution on the etiology of multiple sclerosis (MS) are on the increase. The aim of this study was to determine MS prevalence in a city home to an iron and steel factory which causes air pollution. Methods The study was designed as a cross-sectional, population-based, descriptive epidemiologic study. Eregli city, which has an iron and steel factory and proven air pollution, was screened. Additionally, Devrek city, which is a rural and clean city, located 40 km away from Eregli was assigned and results were compared. A validated questionnaire was used for screening. McDonald 2010 criteria were used to diagnose cases. Results 32 261 people were screened in Eregli, and 21 963 people were screened in Devrek. In total, 41 patients were diagnosed with clinical definite MS. Crude prevalence was found to be 96.1/100 000 in Eregli and 45.5/100 000 in Devrek. The mean age of patients was 39.8, and the female/male ratio was 1.9. Conclusion The results of this study indicate a more than double MS prevalence rate in the area home to an iron and steel factory when compared to the rural city. This supports the hypothesis that air pollution may be a possible etiological factor in MS.
- MakaleYalnız künye2020Right Ventricular Early Inflow-Outflow Index-A new method for echocardiographic evaluation of right ventricle dysfunction in acute pulmonary embolism

Introduction Acute pulmonary embolism (APE) is a cardiothoracic thromboembolic emergency at risk of life-threatening. Several risk graduation algorithms may be applied to delineate short-term mortality in patients with APE. In this study, we aim to depict the relationship between the right ventricular early inflow-outflow (RVEIO) index, which is a Doppler-based parameter, and the pulmonary embolism severity index(PESI) in acute pulmonary embolism. Methods In the presented study, a total of 160 patients who were diagnosed with APE using pulmonary computed tomography angiography or ventilation/perfusion scintigraphy were comprised. Patients were separated to 2 groups based on the simplified PESI (sPESI): sPESI < 1 (n = 88) and sPESI >= 1 (n = 72). Echocardiographic parameters, including the RVEIO index, were measured. Results There were no significant differences between the groups in age and gender distribution, or the presence of diabetes mellitus, hypertension, smoking, and history of coronary artery disease. There was a positive correlation with the mortality rate and RVEIO index; the mortality was higher in patients with a higher RVEIO index(<0.001). In receiver operating characteristic (ROC) curve analysis using a cutoff level of 14.39, RVEIO index predicted mortality with a sensitivity of 80.4% and specificity of 57.6%(ROC area under curve:0.694; 95%CI, 0.581-0.814; P < .001). RVEIO index was higher in the sPESI >= 1(n:72) than in the patients with sPESI < 1(14.27 +/- 2.13 vs 10.63 +/- 2.09; P < .001). There was a positive correlation between RVEIO index and sPESI score(+0.428; P < .001). Conclusion As well as predicting the degree of tricuspid regurgitation (TR), the RVEIO index is well-correlated with sPESI score and is associated with mortality in patients with APE. This easily measurable parameter may be used to predict short-term mortality in APE patients.

Objectives/HypothesisTo determine the effect of radioactive iodine (RAI) treatment on the nasal mucosa and nasal functions. Study DesignProspective clinical study. MethodsThis study included 41 patients (31 female and 10 male) who were treated with RAI for papillary thyroid carcinoma. A visual analogue scale (VAS) was used for subjective evaluation of nasal symptoms. The saccharine test, nasal Schirmer test, and acoustic rhinometry were used for objective evaluation of nasal functions. All tests were administered at baseline (before RAI treatment), and then 1 month and 1 year post-RAI treatment. Only 21 of the patients could be evaluated after 1 year of treatment and were included in the study's analysis. ResultsVAS obstruction and dryness scores 1 month and 1 year post-RAI were significantly higher than the baseline scores (P < .05 and P < .05, respectively). Mean cross-sectional area values 1 month and 1 year post-RAI did not differ significantly from baseline values (P > .05 and P > .05, respectively). Schirmer test results 1 month and 1 year post-RAI treatment were significantly lower than at baseline (P < .05 and P < .05, respectively). Saccharine test results 1 month and 1 year post-RAI were significantly higher than at baseline (P < .05). ConclusionsRAI treatment can adversely affect the nasal mucosa. Nasal dryness and obstruction can occur immediately after RAI treatment. Additional research is warranted to further elucidate the effects of RAI treatment on nasal function. Level of Evidence4. ObjectivesWe aimed to test the left atrial (LA) mechanics and contraction synchrony by 2D strain imaging, in patients with Wolff-Parkinson-White (WPW) syndrome, before and after radiofrequency catheter ablation (RFCA). MethodsStudy population consisted of 25 patients with WPW scheduled for RFCA and 30 healthy controls. The peak LA strain at the end of the ventricular systole (LAs strain) and the LA strain with LA contraction (LAa Strain) were obtained. To assess LA dyssynchrony, septal versus lateral wall time-to-peak strain measurements were measured. ResultsThere was no difference between the patients with WPW and control subjects with regard to peak LAs and LAa strain. Patients with WPW demonstrated higher global time-to-peak LAs and LAa strain values compared with the control group. Peak LAs strain and LAa strain values, measured before and after the RF ablation of the accessory pathway, were comparable (34.33.92 vs. 34.6 +/- 3.2, P=0.816, 14.7 +/- 2.8 vs. 15.3 +/- 2.3, P=0.052, respectively). Global time-to-peak LAs and LAa strain measurements were significantly shorter after the RFCA compared with the values obtained before the RFCA. However, septo-lateral times to peak LA strain differences were found to be comparable in both WPW versus control and pre- versus postablation groups. ConclusionLA mechanical function assessed by 2D strain imaging was comparable between patients with WPW and control subjects. Patients with WPW had more prominent LA dyssynchrony during atrial pump phase as compared with the controls, a condition which could not improve after successful elimination of the accessory pathway by RFCA.
- MakaleYalnız künye2015Effect of applications of organic solid cattle manure on Hungarian vetch and barley intercropping mixtures grown on soils of different depths
Yolcu, H.
Sloping fields on soils of shallow depth to tillage are commonly left uncultivated in many parts of the world. This study was conducted to compare the effects on morphological traits, dry-matter (DM) yield, legume ratio (LR), crude protein content (CP), crude protein yield (CP yield) and mineral concentrations (N, P, K, S, Ca, Cu, Fe, Mg, Mn, Na, B and Zn) of Hungarian vetch (Vicia pannonica Crantz.) and barley (Hordeum vulgare L.) grown in intercropping mixtures in response to three rates of organic solid cattle manure application (M0: 0, M1: 10, M2: 20 t ha-1). Experimentation was conducted on soils of two different soil depths [shallow (8-12 cm; low-medium erosion risk) and normal soil depth (18-22 cm; no erosion risk)] on a sloping field in the 2006-2007 and 2007-2008 growing seasons at Gumushane, Turkey. Herbage harvested on the shallow depth area had 22-73% less DM yield, 14-72% less CP yield, 6-9% greater CP content and generally higher minerals contents than herbage from the normal soil depth area. Cattle manure applications increased DM yield by about 23%, increased CP content and CP yield, and also increased the contents of most minerals in herbage of the intercropping mixtures, relative to the control, averaged over the two soil depths. It is suggested that, for areas with shallow soil depths that are prone to erosion, plant cover should be used for forage production, and that fertilization with solid cattle manure at 20 t ha-1 can support production of quality forage of acceptable DM yield.
- MakaleYalnız künye2015Antifungal effect of Origanum onites essential oil as an alternative to formalin in the artificial incubation of narrow-clawed crayfish (Astacus leptodactylus Eschscholtz, 1823)
Koca, Seval Bahadir; Cevikbas, Mustafa
As alternative to formalin, the antifungal effect of a plant product [Origanum onites L. (Lamiaceae) oil] was investigated for use in the artificial incubation of narrow-clawed crayfish (Astacus leptodactylus Eschscholtz) eggs. For this purpose, this study was conducted as two experiments. In experiment I, the eggs were artificially incubated for 40 days. In experiment II, juveniles were cultured to determine effects of O. onites oil on juveniles for 30 days. The experimental groups were as follows: formalin (3500 ppm for 15 min), O. onites oil (300 ppm for 15 min, 700 ppm for 2 min and 1000 ppm as a dip treatment 15 split-second) and a control (no treatment). In the experiment I, the highest hatching rate (86%) and survival rate of stage II juveniles (80%) were observed in 1000 ppm dip group. These results were similar to that of formalin group (85% and 79%) respectively. The control group exhibited the lowest hatching rate (49%) and stage II rate (42%) compared with the 1000 ppm dip group and 3500 ppm formalin treatments. However, other concentrations (300 and 700 ppm) of O. onites showed toxic effects on the eggs and there was no hatching. In the experiment II, the survival rate and growth performance of the crayfish juveniles were similar in all groups. This study indicated that the 1000 ppm O. onites dip treatment could be a good alternative to formalin for improved egg hatchability in the artificial incubation of crayfish eggs.
- MakaleAçık Erişim2014Multiwalled CNT-pHEMA composite conduit for peripheral nerve repair
Arslantunali, D.; Budak, G.; Hasirci, V.
A nerve conduit is designed to improve peripheral nerve regeneration by providing guidance to the nerve cells. Conductivity of such guides is reported to enhance this process. In the current study, a nerve guide was constructed from poly(2-hydroxyethyl methacrylate) (pHEMA), which was loaded with multiwalled carbon nanotubes (mwCNT) to introduce conductivity. PHEMA hydrogels were designed to have a porous structure to facilitate the transportation of the compounds needed for cell nutrition and growth and also for waste removal. We showed that when loaded with relatively high concentrations of mwCNTs (6%, w/w in hydrogels), the pHEMA guide was more conductive and more hydrophobic than pristine pHEMA hydrogel. The mechanical properties of the composites were better when they carried mwCNT. Elastic modulus of 6% mwCNT loaded pHEMA was twofold higher (0.32 ± 0.06 MPa) and similar to that of the soft tissues. Electrical conductivity was significantly improved (11.4-fold) from 7 × 10-3 Ω-1.cm-1 (pHEMA) to 8.0 × 10-2 Ω-1.cm-1 (6% mwCNT loaded pHEMA). On application of electrical potential, the SHSY5Y neuroblastoma cells seeded on mwCNTs carrying pHEMA maintained their viability, whereas those on pure pHEMA could not, indicating that mwCNT helped conduct electricity and make them more suitable as nerve conduits.

Sezer, Umran Aydemir; Arslantunali, Damla; Aksoy, Eda Ayse; Hasirci, Vasif; Hasirci, Nesrin
In this study, novel poly(epsilon-caprolactone) (PCL) composite scaffolds were prepared for bone tissue engineering applications, where gentamicin-loaded -tricalcium phosphate (-TCP)/gelatin microspheres were added to PCL. The effects of the amount of -TCP/gelatin microspheres added to the PCL scaffold on various properties, such as the gentamicin release rate, biodegradability, morphology, mechanical strength, and pore size distribution, were investigated. A higher amount of filler caused a reduction in the mechanical properties and an increase in the pore size and led to a faster release of gentamicin. Human osteosarcoma cells (Saos-2) were seeded on the prepared composite scaffolds, and the viability of cells having alkaline phosphatase (ALP) activity was observed for all of the scaffolds after 3 weeks of incubation. Cell proliferation and differentiation enhanced the mechanical strength of the scaffolds. Promising results were obtained for the development of bone cells on the prepared biocompatible, biodegradable, and antimicrobial composite scaffolds. (c) 2013 Wiley Periodicals, Inc. J. Appl. Polym. Forage DMY and SY increased over the first 4 yr, but the yields decreased due to the aging of the crested wheatgrass stand in the subsequent year. There were significant differences for lines in all variables, except for SWY and CPC. Particularly, lines AC1, AC23, AC29, AC37, AC39, AC65, AC66, AC107, and AC121 in DMY; AC1, AC37, AC83, AC107, AC113, AC121, and AC125 in SY; and AC23, AC39, and AC113 in CPY showed significant improvement compared with the original parent material. The results of DMY, SY, and CPY show that selection studies were effective. Selected lines will be useful for breeding studies and livestock production.
- MakaleYalnız künye2013Coronary Flow Reserve and Myocardial Performance Index in Newly Diagnosed Diabetic Patients
Kalkan, Gulhan Yuksel; Gür, Mustafa; Sahin, Durmus Yildiray; Baykan, Ahmet Oytun; Elbasan, Zafer; Kuloglu, Osman; Kivrak, Ali; Türkoglu, Caner; Arik, Osman Ziya; Çayli, Murat
BackgroundPatients with diabetes mellitus (DM) are at high risk of left ventricular (LV) dysfunction. A reduction of coronary flow reserve (CFR) has been demonstrated in diabetic patients without coronary artery stenosis. In this study, we investigated the association between LV myocardial diastolic and systolic functions and the CFR in newly diagnosed diabetic patients in the absence of coronary artery disease. MethodWe studied 88 patients (mean age 4910years) with newly diagnosed DM and 40 healthy control subjects (mean age 509years). All subjects underwent transthoracic echocardiography. The myocardial performance index (MPI) was determined by using pulsed-wave Doppler. The CFR was calculated as the hyperemic to resting coronary diastolic peak velocities ratio. ResultsPatients with diabetes had significantly longer mitral E-wave deceleration time and higher MPI value than control group (P=0.023 and P<0.001, respectively). The mean CFR was lower in diabetic patients compared with controls (P=0.001). The MPI was significantly correlated with CFR (r=-0.371, P<0.001), LV ejection fraction (r=-0.274, P=0.018) E/A ratio (r=-0.244, P=0.024), and glycosylated hemoglobin (HbA1c) (r=0.476, P<0.001). Multivariate regression analysis showed that MPI was independently associated with CFR (=-0.292, P<0.0001) and HbA1c level (=0.372, P<0.0001). ConclusionsThe MPI was independently associated with CFR in newly diagnosed diabetic patients. The decrease in CFR may cause LV systolic and diastolic dysfunction in asymptomatic diabetic patients.
- MakaleYalnız künye2012The effect of adding sour cherry pulp into yoghurt on the physicochemical properties, phenolic content and antioxidant activity during storage
Şengül, Memnune; Erkaya, Tuba; Şengül, Mustafa; Yildiz, Hilal
This study investigated the effect of adding sour cherry pulp into yoghurt on its physicochemical properties, phenolic content, antioxidant activity and sensory characteristics. Sour cherry pulp was added at 0%, 8%, 12% and 16% into the yoghurt, and measures were checked through 14days cold storage. The increasing sour cherry pulp concentration in yoghurt resulted in increasing pH and whey separation, whereas the values of the other parameters, total solid, fat, protein, ash, titratable acidity and viscosity decreased. During storage, total phenolic contents and antioxidant activity in yoghurts ranged from 20 to 81μg gallic acid equivalent per mg of sample and from 48% to 86%, respectively.
- MakaleYalnız künye2011Determination of Lactic Microflora of Kefir Grains and Kefir Beverage by Using Culture-Dependent and Culture-Independent Methods
Kesmen, Zulal; Kacmaz, Nazife
In this study, we investigated the bacterial compositions of kefir grains and kefir beverages collected from different regions of Turkey by using culture-independent and culture-dependent methods. In the culture-independent detection, 10 different species of bacteria were detected in total by using the polymerase chain reaction-denaturing gradient gel electrophoresis (PCR-DGGE) analysis of the 16S rRNA gene V3 region. Among these species, Lactobacillus kefiranofaciens was the most dominant one in the kefir grains, while Lactococcus lactis was found to be significantly prevalent in the kefir beverages. In the culture-dependent detection, the primary differentiation and grouping of the isolates from kefir beverages and kefir grains were performed using repetitive sequence-based PCR (rep-PCR) fingerprinting, and the results were validated by 16S rDNA full-length sequencing. According to the results of culture-dependent methods, the most frequently isolated species were L. lactis, Leuconostoc mesenteroides, and Lactobacillus kefiri, respectively. Only 3 species, which are L. lactis, Lactobacillus acidophilus, and Streptococcus thermophilus, were detected with both culture-dependent and culture-independent methods. This study showed that the combination of both methods is necessary for a detailed and reliable investigation of microbial communities in kefir grains and kefir beverages. Practical Application Due to their artisan- and region-dependent microflora, kefir products can be a source of interesting lactic acid bacteria, either new taxa or strains with specific functional properties, which might be used for the development of new starter cultures and innovative food products. Therefore, an increasing demand exists for new strains that show desirable effects on the product characteristics Artisan dairy products are a candidate source of such microorganisms. For this reason, in this study, the bacterial compositions of kefir grains and kefir beverages obtained from different regions of Turkey were studied using culture-dependent and culture-independent molecular methods.
- MakaleYalnız künye2011Effects of random material and geometrical properties on structural safety of steel-concrete composite systems
Çavdar, Özlem; Bayraktar, Alemdar; Çavdar, Ahmet
A stochastic finite element-based algorithm for probabilistic analysis of structural systems made of composite sections with random material and geometrical properties under earthquake forces is proposed in this paper. Uncertainties in the structural parameters can be taken into account in this algorithm. For the perturbation-based stochastic finite element method, only the first two moments of random variables need to be known, and is numerically much more efficient and feasible than simulation techniques. The efficiency and accuracy of the proposed algorithm are validated by comparison with results of Monte Carlo simulation method. A summary of stiffness matrix formulation and perturbation-based stochastic finite element dynamic analysis formulation of structural systems made of composite sections is given. These are followed by suitable numerical examples, which indicate that employment of such a dynamic stochastic finite element method leads to significant economical, efficient and accurate solutions for the dynamic analysis of composite structures with stochastic parameters under earthquake forces.

Gokhan, Nezihe; Sozmen, Mahmut; Ozba, Burhan; Gungor, Emine
Objective: To describe the clinical and pathomorphological features of meibomian carcinoma (MC) diagnosed in a Simmental cow. Material and methods: A 7-year-old Simmental cow was admitted to the Veterinary Surgery Clinics of Faculty of Veterinary Medicine, University of Kafkas, Kars, Turkey. The signalment and anatomical distribution were summarized and focused on pathomorphological description. Results: Macroscopically, a papillary mass, 5 × 3.5 × 7 cm in size, arising from the left lower eyelid with local spread to the upper eyelid and covering the entire globe was observed. The mass was ulcerated and hemorrhagic and applied pressure to the globe. Following sedation and local anesthesia, the mass and globe were excised. Histopathological examination revealed an MC. Conclusions: The diagnosis of MC is infrequent in veterinary literature. Complete surgical excision could be curative and histopathology is crucial for the diagnosis of MC occurring in animals.
- MakaleAçık Erişim2009Water quality modeling and dissolved oxygen balance in streams: A point source Streeter-Phelps application in the case of the Harsit stream
Nas, S. Serkan; Nas, Evin
The Harsit Stream flowing into the Black Sea is an important watershed in the northeastern part of Turkey. The monthly averages of dissolved oxygen, carbonaceous biochemical oxygen demand, temperature, pH, total suspended solids, total phosphorus and total nitrogen were obtained for the water samples taken from the Tekke, Akcakale and Haciemin sites of the Harsit Stream. Water quality parameters were analyzed and evaluated according to the WHO standards and the Turkish Water Pollution Control Regulation. Initially, the variations in the dissolved oxygen profile were determined from dissolved oxygen, temperature and carbonaceous biochemical oxygen demand measurements by using the point source Streeter-Phelps equation. Critical dissolved oxygen deficit values were calculated for the monthly average from January-December 2005, which are below the limit value (4 mg L-1) excluding April (7.26 mg L-1) and May (5.98 mg L-1) in the Harsit Stream, resulting in it having second class water quality in terms of dissolved oxygen concentration, temperature, pH and biochemical oxygen demand. It is also seen that the minimum dissolved oxygen limit value (8 mg L-1), which is a desired quantity for drinking water, is under the limit value for all months in the case of the water mixture. In particular, during the summer months, the consumption speed of oxygen is greater than the production speed of oxygen with increasing temperature and decreasing discharge, and negative values (anoxia) of dissolved oxygen sag are seen. These situations call for significant precautions and treatment criteria.
