Demirag, Hatice; Kulakaç, Nurşen; Hintistan, Sevilay; Çilingir, Dilek
Objective: This study aims to determine the effects of cachexia, causing major problems in the world and Turkey, on self-care agency and quality of life in cancer patients. Methods: The population of this cross-sectional and relationship-seeking study consisted of cancer patients in Turkey from April 1 to April 20, 2021. Using the snowball sampling method, 174 patients were sampled. 'Patient Information Form,' 'The European Organization for Research and Treatment of Cancer C30 Cancer Quality of Life Scale,' and 'Exercise of Self-Care Agency Scale' were used as data collection tools. Results: In the study, 52 patients (29.9%) were found to have cachexia. Function, general well-being, symptom (except insomnia), and self-care agency, which are subdimensions of the quality-of-life scale, were found to be significantly lower in patients with cachexia than patients without cachexia (P < 0.001). It was determined that there was a significant negative correlation between the cachexia status of the patients and the five basic functions in the functional scale (physical, role, emotional, cognitive, and social function), general well-being, and self-care agency, and there was a significant positive correlation between the cachexia status of the patients and the symptom scale (P < 0.001). According to the results of multiple linear regression analysis, it was found that the factor that significantly affected the cachexia status of the patients was their self-care agency (P < 0.001). Conclusions: It was determined that cachexia caused significantly lower self-care agency and quality of life in cancer patients. Furthermore, quality of life was related to self-care agency.

Aydın Sayılan, Aylin; Demir Doğan, Melike
Introduction: Research has shown that social support has a significant impact on quality of life. The purpose of this cross-sectional study was to determine the relationship between perceived social support, perception of disease and quality of life, and the factors affecting these, in patients with cancer in Turkey. Methods: The study data were collected from two hospitals. Data were collected using the face-to-face interview method from patients diagnosed with cancer between March and August 2018. No sampling was performed, and the entire population was studied. Results: The mean age of the patients in the study was 61.37 ± 14.89 years. Married patients had better social support than single individuals (t = 2.129; p = 0.035). Significant negative correlation was found between the illness perception scale and total quality of life scale score (r = −0.163; p = 0.026), and physical well-being (r = −0.224; p = 0.002) and social well-being (r = −0.199; p = 0.006) subscales. Conclusion: Patients’ social support and quality of life were at moderate levels. Married participants had better social support than single participants, and operated participants had better social support than unoperated participants. As illness perception decreased, quality of life, physical well-being and social well-being increased.

- MakaleAçık Erişim2020Scopus SJRQ3Determining the quality of life and the sleep quality in patients with benign prostate hyperplasia
Kulakaç, Nurşen; Sayılan, Aylin Aydın
Research question: How is the quality of life and quality of sleep affected in patients with benign prostatic hyperplasia?. Research problem: The aim of this study was to evaluate the quality of life and quality of sleep in patients with benign prostatic hyperplasia (BPH) and to identify the relationship between them. Nocturia is one of the main symptoms associated to BPH, causing significant sleep disturbances, including sleep interruptions. Such sleep interruptions may significantly impair quality of life. Literature review: The assessment of nocturia and its impact on quality of life and sleep quality in patients living with benign prostatic hyperplasia (BPH) has been poor in terms of clinical research; moreover, there is a lack of specific methods to assess this impact. Methodology: The sample of this descriptive study consisted of all patients with the diagnosis of BPH who were admitted to the urology outpatient clinic of a state hospital in Turkey. The data were collected from 212 patients with BPH who accepted to participate in the study in January-June 2018. All patients gave consent prior to inclusion in the study. The Pittsburgh Sleep Quality Index and the WHOQOL-BREF (World Health Organization Quality of Life Scale - Brief Form) were used as the principle data collection tools. Percentage, t test, correlation and one-way variance analysis test were used to evaluate the data. Results: The mean age of the patients was 67.50 ± 8.89 years and 63.2% of them were first diagnosed with benign prostatic hyperplasia 2.13 years ago. The lowest and the highest average scores that the patients obtained from the WHOQOL-BREF were their social and environmental domains scores, respectively. In all, 20.2% of the patient were found to have poor sleep quality (global score ≥ 5). The results showed that there was a positive correlation between the mean scores of the sleep quality of the patients and their quality of life physical domain subscale. It was also found that there was a significant negative correlation between night urination and the sleep quality in BPH patients (P <.05). Conclusions: It was determined that low quality of life in BPH patients is associated with low sleep quality. Personal characteristics, frequent night urination and working status are important determinants of the relationship between quality of life and sleep quality.

- MakaleYalnız künye2012Scopus SJRQ2The role of group education on quality of life in patients with a stoma
Altuntas, Y.E.; Kement, M.; Gezen, C.; Eker, H.H.; Aydin, H.; Sahin, F.; Okkabaz, N.; Oncel, M.
Stoma education has been traditionally given in a one-to-one setting. Since 2007, daily group education programmes were organised for stoma patients and their relatives by our stoma therapy unit. The programmes included lectures on stoma and stoma care, and social activities in which patients shared their experiences with each other. Patients were also encouraged to expand interaction with each other and organise future social events. A total of 72 patients [44 (61.1%) male with a mean (±SD) age of 56.8 ± 13.6 years] with an ileostomy (n= 51, 70.8%), a colostomy (n= 18, 25.0%) or a urostomy (n= 3, 4.2%) were included in the study. Patients were asked to answer a survey (SF-36) face-to-face before the initiation of the programme, which was repeated 3 months later via telephone call. The comparison of pre-education and post-education SF-36 scores revealed a statistically significant improvement in all 8-scale profiles, but not in vitality scale, and both psychometrically-based and mental health summary measures. Analyses disclosed that married patients and those who were living at rural districts seem to have the most improvement in life quality particularly in bodily pain, general health and role-emotional scales and mental health summary measure. In our opinion, group educations may be beneficial for stoma patients, and stoma therapy units may consider organising similar activities.

Kement, Metin; Gezen, Cem; Asik, Ayla; Karaöz, Alev; Ustaalioglu, Basak Oven; Bilici, Ahmet; Öncel, Mustafa
Objective: The aim of this study is to compare the postoperative quality of life of the patients who underwent breast conserving surgery followed by radiotherapy (BCT) and the patients who underwent modified radical mastectomy (MRM) in our clinic. Material and Methods: The patients who were operated on with diagnosis of breast cancer in our clinic between January 2006 and January 2008 were included in the study. Those who received neoadjuvant therapy and who required radiotherapy after modified radical mastectomy and those in ASA classes 3 and 4 were excluded. The patients who were treated with modified radical mastectomy were included in the first group (Group MRM) and the patients who were treated with breast conserving therapy and radiotherapy thereafter were included in the second group (Group BCT). Quality of life of the patients were assessed using Medical Outcome Syudy-Short Form 36 (MOS SF-36) form six months after their treatment (surgery, radiotherapy, chemotherapy) was completed. Results: There were no significant differences between two groups in terms of age, evaluated socioeconomic parameters (education, income, marital status) and clinical parameters (stage, complications, adjuvant chemotherapy). Quality of life of the patients who underwent BCT (n=27) was worse than that of general Turkish population in six of eight (except vitality and social function) SF-36 scale and worse than mean values of normal individuals in NSABP BCPT trial in seven of eight (except vitality) scales. Besides, while physical component score was worse than that of general population (45.3 vs 52.6), mental component score (MCS) was relatively similar to general population (53.4 vs 51.7). When quality of life of the patients who underwent MRM (n=23) and BCT were compared, mean values of BCT group were found to be better in all scales and the difference was found to be statistically significant in six out of eight (except vitality and social function) scales (p<0.05). Conclusion: It can be stated that quality of life of the patient who underwent BCT due to breast cancer is impaired in terms of physical status, however did not change in terms of mental status. It can also be stated that quality of life of the patients who underwent breast conserving therapy were better than the ones who underwent modified radical mastectomy in terms of both physical and mental status.

Kement, M.; Karabulut, M.; Gezen, F.C.; Demirbas, S.; Vural, S.; Oncel, M.
Aim: This study aims to evaluate the risk factors for incontinence after lateral internal sphincterotomy (LIS) and assess quality of life in different levels of incontinence. Methods: All consecutive patients (n = 253) with chronic anal fissure who underwent LIS between 2003 and 2006 were retrospectively reviewed. All patients were questioned for possible anal incontinence according to the Wexner Incontinence Score (WIS). Demographics, vaginal delivery history, additional procedures and surgeon's experience were evaluated as risk factors. Endoanal ultrasound (EUS) was performed in incontinent patients to assess the thickness of the remaining internal sphincter and to evaluate any injury in the external sphincter. Quality of life was questioned with SF-36. Results: Twenty-eight (11.7%) patients suffered from incontinence (mean WIS = 3.6 ± 2.5). The search for a risk factor was unsuccessful when continent and incontinent groups were compared. In subgroup analyses, patients were found to be suffering from mild (WIS <5, n = 19) or severe (WIS >5, n = 9) incontinence. Vaginal delivery history was found more often in the severely incontinent subgroup than in the continent group (p < 0.05). Also, vaginal delivery history and the additional procedures were more frequently observed in the severely incontinent subgroup than in the mildly incontinent subgroup. EUS did not find any external sphincter injury in these cases. WIS had negative correlations with the physical and mental component scores of SF-36. Conclusion: In our opinion, the threat for incontinence is unpredictable; however, vaginal delivery history may increase the risk of severe incontinence.
