Surrogate Trauma in Nurses Providing Pediatric Palliative Care: A Phenomenological Study

Oğuzhan, Hatice; Akay, Gamze; Uzun, Sevda; Koç, Elif Simay; Öncer, Aysun

The aim of this study was to deeply examine the experiences of proxy grief among nurses providing pediatric palliative care. This qualitative phenomenological study was conducted with 16 nurses working in the pediatric ward of a hospital located in the northern region of the country. Data were analyzed using Colaizzi’s phenomenological analysis method. The analysis revealed 3 themes (challenges experienced while caring for children in palliative care; secondary trauma and coping processes; proxy trauma and professional resilience) and 9 sub-themes (emotional strain during care, establishing an empathetic bond, emotional exhaustion, traumatic experiences, seeking psychological support, religious coping methods, developing a professional approach, resilience and acceptance, and motivation to continue in the profession). The study found that nurses providing pediatric palliative care experienced significant psychosocial stress. Pediatric nurses were particularly challenged emotionally when caring for palliative patients. In this context, it is important to develop stress-coping skills to help nurses manage vicarious trauma. Strengthening nurses psychologically and spiritually will positively impact their professional lives, which in turn will enhance the quality of care they provide.

Fully automated 3D volumetric analysis of masticatory muscles in a pediatric population: A CT-based normative morphometric study

The masticatory muscles play a fundamental role in craniofacial growth and development, yet normative volumetric data encompassing all four muscles across the full pediatric age range remain poorly characterized. The aim of this study was to establish age- and sex-specific normative reference values for the volumetric development of masticatory muscles in a normal pediatric population (0–17 years) using a fully automated 3D segmentation method. This retrospective, cross-sectional study included 258 normal pediatric individuals stratified into four age groups based on biomechanical milestones (0–5, 6–9, 10–13, and 14–17 years). Bilateral volumes of the masticatory muscles were automatically calculated using TotalSegmentator in the 3D Slicer environment. Two-way analysis of variance and Bonferroni post-hoc tests were applied. All muscles showed significant volume increases with age (p < 0.001), with the masseter demonstrating the greatest increase (199%). No sex differences were observed in 0–5 years; significant male-favoring differentiation began from 6 to 9 years. The temporalis muscle exhibited a biphasic sex differentiation pattern, with no significant difference at 10–13 years despite significance at 6–9 and 14–17 years. The highest sex-related effect sizes were observed in the pterygoid muscles (medial pterygoid ηp² = 0.176; lateral pterygoid ηp² = 0.160). Bilateral asymmetry analyses revealed left-sided favoring in the masseter, a lateralization trend in the temporalis (exploratory), right-sided favoring in the medial pterygoid, and sex-specific lateralization in the lateral pterygoid. These findings provide a large-scale CT-based normative reference for pediatric masticatory muscles, demonstrating muscle-specific volumetric patterns and variable sex differentiation, with normative ranges supporting clinical decision-making in dentistry, craniofacial surgery, and pediatric radiology.

OBJECTIVE: This study was carried out to evaluate the injuries in pediatric earthquake victims due to the 2023 Turkey-Syria earthquakes with computed tomography and determine the anatomotopographic distribution of injuries. METHODS: The material of this retrospective study consisted of the computed tomography findings of 257 pediatric cases injured in the 2023 Turkey-Syria earthquakes, and those were divided into subgroups based on their age group, i.e., 0-4, 5-9, 10-14, and 15-18 years, and the type of injury, i.e., head, maxillofacial, thoracic, abdominal, pelvic, and spinal injuries. RESULTS: Earthquake-related injuries had been detected in 102 (39.6%) patients. Of the 29 patients with multiple injuries, 17, 10, and 2 had injuries in two, three, and four topographic regions, respectively. The most common injury was a head injury, which was detected in 48 (18.7%) cases, followed by thoracic injury, spinal injury, pelvic fracture, abdominal injury, and maxillofacial fracture, which were detected in 40 (15.6%), 22 (8.5%), 19 (7.4%), 10 (3.9%), and 6 (2.3%) patients, respectively. The cranial bone fractures and intracranial injuries were significantly more frequent in the 0-4 years age group compared with other age groups (p=0.028 and p=0.024, respectively). The rib fractures with spinal and pleural injuries were significantly more common in the 15-18 years age group compared with others (p=0.016, p=0.004, and p=0.002, respectively). CONCLUSION: The head injury was the most common earthquake-related injury in pediatric cases. Herein, it was more common in younger children compared with other age groups, whereas rib, spine, and pleural injuries were more common in older children.
