Providing ethical care for people living with dementia is essential for safeguarding their rights, dignity, and well-being. However, ethical decision-making in dementia care is complex, often involving tensions between competing moral claims such as autonomy, beneficence, nonmaleficence, justice, and non-discrimination. This scoping review aimed to map and describe the ethical principles, related normative considerations, and ethical frameworks discussed in the scientific literature on dementia care, and to identify how these have been conceptualized. Following the methodological guidance of Arksey and O'Malley (2005), Levac et al. (2010), and Peters et al. (2021, 2022), a comprehensive search across five databases (Web of Science, Scopus, CINAHL, Medline/PubMed, and PsycInfo) identified peer-reviewed papers published in English. In total, 388 papers published between 1981 and 2024 were included. The results revealed a wide range of ethical principles and related normative considerations frequently cited in the literature, although they were typically discussed in an isolated way without reference to other principles or broader ethical frameworks. In contrast, ethical frameworks were mentioned far less often, and when they were, they mostly reflected traditional approaches such as principlism. Four gaps were identified: the plurality of ethical frameworks and limited conceptual integration underpinning dementia care research and practice; insufficient attention to ethnic and socioeconomic diversity among people living with dementia; limited consideration of diverse care settings and geographical regions; and the limited inclusion of people living with dementia themselves in the studies reviewed. These findings underscore the need for conceptually integrative, inclusive, and context-sensitive approaches to dementia care ethics that can better inform both ethical reflection and care practices.

Bekircan, Çağrı; Kurnaz, Muammer
Marine Microsporidia represent an increasingly important but historically underestimated group of aquatic intracellular parasites. Although traditionally associated with invertebrates and fishes, recent molecular and environmental studies have revealed extensive hidden diversity across marine ecosystems together with unexpected phylogenetic links between aquatic and human-associated lineages. This review synthesizes five decades of research (1975–2025) concerning the biology, evolution, taxonomy, host–pathogen interactions, ecological diversity, and zoonotic significance of marine Microsporidia. Particular emphasis is placed on members of the Enterocytozoonidae, including Enterocytozoon hepatopenaei, Nucleospora spp., and Desmozoon lepeophtherii, which represent major threats to aquaculture sustainability and may contribute to cross-environmental transmission pathways. Environmental DNA studies further indicate that marine microsporidian diversity is substantially underestimated by morphology-based approaches. However, current evidence for zoonotic transmission remains largely indirect, resting chiefly on phylogenetic relatedness, shared genotypes across hosts and environmental (eDNA) detection; direct, seafood-borne human infection has not yet been demonstrated. Current evidence suggests that aquaculture intensification, climate-driven environmental change, and expanding seafood trade may facilitate the emergence and dissemination of zoonotic microsporidian lineages. Integrating marine Microsporidia into One Health surveillance frameworks is therefore increasingly important for both ecosystem and public health management.

Kurnaz, Muammer
This review presents the first integrated proxy, mechanism and biotic-response framework for the Indian Quaternary, linking the Subcontinent's dense network of palaeoenvironmental archives to the biogeographic trajectory of its endemic herpetofauna. Speleothem δ18O records, Arabian Sea and Bay of Bengal sediment cores, Karewa and peninsular lake sequences, Siwalik and Narmada alluvium, and Kurnool cave deposits together document Indian Summer Monsoon (ISM) variability on orbital, millennial, centennial and historical timescales (the historical band here denoting the past ∼2000 years, resolved by dated speleothem and documentary records). Against this proxy backbone, the fossil and molecular phylogeographic record of Indian amphibians and reptiles is evaluated as a biogenic assemblage here, the body of biological evidence (fossils, subfossils and the phylogeographic signatures of living taxa) that, like pollen or foraminifera, records past environmental conditions that complements geological and climatological archives. Three patterns emerge. At orbital scale, ISM variability aligns with Plio–Pleistocene diversification slowdowns in Western Ghats ranoids and with the Late Pliocene lineage divergence of the Himalayan toad Duttaphrynus himalayanus across the Thakkhola basin. Abrupt Holocene excursions, including the 8.2 ka and 4.2 ka events, are resolved at high precision in speleothem and marine archives but remain untested against coalescent demographic inference in Indian herpetofauna. Late Pleistocene aridity, most severe at the Last Glacial Maximum, coincides with community-level impoverishment in peninsular cave faunas at Kurnool, although the available evidence establishes co-occurrence rather than a demonstrated causal role for aridity relative to co-varying thermal and vegetation change. Three proxy-calibrated research priorities follow: microvertebrate taphonomy in Narmada and Karewa sequences, population genomics tied to speleothem chronology, and species-distribution hindcasting to the LGM, mid-Holocene and 4.2 ka intervals. The synthesis contributes to a biogenic assessment of Quaternary-to-Recent climate impacts on life on the Indian Subcontinent, aligned with UN Sustainable Development Goals 13 (Climate Action) and 15 (Life on Land), and supports future projections of amphibian and reptile response to ongoing change.
- MakaleAçık Erişim2026The effect of hawthorn vinegar, black mulberry syrup, and green tea on oral mucositis and oral health-related quality of life in individuals with diabetes: a single-blind, randomized controlled trial
Demiraǧ, Hatice; Akeren, Zahide
Objective – This study was designed as a single-blind randomized controlled trial to investigate the effects of hawthorn vinegar, black mulberry syrup, and green tea on oral mucositis and quality of life related to oral health in individuals with diabetes. Methods – Total of 120 participants were divided into four groups (hawthorn vinegar, black mulberry syrup, green tea, control; n = 30 each) and the study was completed. All participants completed the “Patient Information Form, ” “Oral Assessment Guide (OAG/ADR), ” and “Oral Health-Related Quality of Life Scale (OHQoL-UK)” before the study, on the 7th day, and after the study. In the experimental groups (hawthorn vinegar group, black mulberry syrup group, green tea group). In addition to routine oral mucositis treatment, patients with DM gargled for 1 min with diluted hawthorn vinegar [1 tablespoon (10 cc) mixed with 40 cc of water], pure black mulberry syrup, or green tea solution three times a day for 14 days, 15–20 min before meals. Results – Patients in the hawthorn vinegar group showed a decrease in oral mucositis and an increase in oral health-related quality of life at the interim measurement (day 7) and final test (day 14) measurements (p < 0.05). A negative correlation was found between the severity of oral mucositis and quality of life (p < 0.05). Conclusion – Although there was no difference between hawthorn vinegar and black mulberry syrup, they were found to be more effective than green tea in reducing oral mucositis and improving oral health-related quality of life, and all three complementary treatments were found to be effective. Aim: This study aimed to evaluate the effects of three different music-based interventions on anxiety and fatigue levels in patients undergoing hemodialysis, compared to a concurrent control group receiving standard care. Methods: This randomized controlled trial involved 64 patients receiving maintenance hemodialysis. They were randomly assigned to four equal groups (n = 16 each): live music, live ney sound therapy (using a traditional reed flute known for its soothing, breath-like tones), a music-repeating robotic parrot therapy (an interactive robotic parrot that repeats music or sounds provided by the patient), or a control group (no intervention). The experimental groups received 30-min sessions 3 days per week for 2 months. The “Beck Anxiety Scale” and “Fatigue Severity Scale” were used to measure anxiety and fatigue, respectively. Assessments were performed before the intervention, immediately after the 2-month intervention period, and at 2 months after the end of the intervention period. Results: Before the intervention, there were no significant differences in anxiety or fatigue scores across the four groups (p > 0.05). After the interventions, all three groups exposed to music therapy showed significant reductions in both anxiety and fatigue scores compared to the control group (p < 0.001). Conclusion: All three therapies were effective in managing anxiety and fatigue symptoms in hemodialysis patients. Among them, music-repeating robotic parrot therapy demonstrated the most superior and longest-lasting effects in reducing both anxiety and fatigue levels. Trial Registration: ClinicalTrials.gov identifier: NCT07238374.
- MakaleYalnız künye2026The Effect of Parrot Therapy on During Venipuncture Pain in Chronic Patients
Demirağ, Hatice; Tuç, Adile
Objective There are no randomized controlled trial (RCT) in the current literature comparing live, robotic, and visual animal-based therapies. This single-blind randomized controlled trial was conducted to comparatively evaluate the effectiveness of live parrot, robotic parrot, and parrot picture therapies on pain during venipuncture in adults with chronic illness. Methods The study included 240 patients randomized into four groups. Groups interacted with live parrots, robotic parrots, or parrot pictures during venipuncture. The control group received no intervention. Pain levels were assessed before and after the procedure using the "Visual Analog Scale (VAS)." Analysis was performed using Chi-Square test, One-way ANOVA and paired samples t-test. Results There was no difference in pre-procedure pain scores between the groups (p ' .05). After the procedure, the VAS score for the live parrot group was 2.01 ± 1.35, for the robotic parrot group 2.03 ± 1.46, for the parrot picture group 2.08 ± 1.62, and for the control group 4.91 ± 1.88. Again, after the procedure, pain scores in all experimental groups were found to be significantly lower than in the control group (p ' .001). Live parrot therapy was found to be the most effective pet therapy, followed by robotic parrot and parrot picture therapy. The reduction in pain in the live parrot group was statistically more significant than in the picture group. Conclusion The research findings revealed that pet therapy applied during venipuncture, especially live animal interaction, may be a potentially effective, safe, and non-pharmacological method for reducing pain. Robotic animals, on the other hand, can be considered a promising alternative when the use of live animals is not possible. It is recommended that these distraction strategies be considered and integration into routine nursing practices be developed as part of patient-centered care.
- MakaleAçık Erişim2026Core–Periphery Organization and Spatial Heterogeneity in Pseudopus apodus (Anguidae) Across Its Western Palearctic Range
Şahin, Mehmet Kürşat; Topal, Azra; Kurnaz, Muammer
Understanding the internal spatial structure of widely distributed species is fundamental for biogeographic theory and conservation practice, yet such structure is often masked by extent-based range metrics. We investigated the spatial organisation of Pseudopus apodus (Pallas, 1775), the largest limbless lizard of the Western Palearctic, using 3967 occurrence records spanning 1843–2025. Spatial point pattern analysis revealed a pronounced deviation from complete spatial randomness (Clark–Evans R = 0.105, p < 0.001), with strong fine-scale clustering. Kernel density estimation identified a clear core–periphery organisation: high-density core areas accounted for 30% of records but occupied only 22% of the total extent of occurrence (EOO). The discrepancy between EOO (~8.9 million km2) and area of occupancy (AOO; ~8944 km2) spanned three orders of magnitude, emphasising that only a small fraction of the species’ geographic envelope is actively occupied. Spatial heterogeneity was high (coefficient of variation ≈ 0.99), and core and peripheral occurrences were significantly segregated along both latitudinal and longitudinal gradients. The proportion of core records showed a weak positive temporal trend most plausibly attributable to sampling effort, particularly the recent expansion of citizen-science contributions, rather than to ecological processes. These findings demonstrate that P. apodus exhibits a compact spatial core embedded within a broad, sparsely occupied periphery, underscoring the limitations of EOO-based metrics in the conservation assessment of widely distributed reptiles. We emphasise that this structure is characterised using the density of occurrence records and therefore describes the observed spatial organisation of the available data rather than directly measured population density; ecological interpretations are accordingly framed as hypotheses requiring independent validation.
- MakaleYalnız künye2026Spatial heterogeneity of microscopically detected Nosema spp. infection in honeybees: Climate-associated patterns in the Eastern Black Sea Region of Türkiye
Tosun, Onur; Bekircan, Çağrı; Kurnaz, Muammer
Nosemosis has emerged as the predominant microsporidian infection of honeybees (Apis mellifera L.) in many regions, including Türkiye, yet the climatic drivers of its epidemiology remain insufficiently quantified. This study assessed the effects of temperature and relative humidity on nosemosis prevalence across seven provinces in the climatically diverse Eastern Black Sea region. A dataset of 2688 colony-level observations was analyzed using generalized linear models, mixed-effects models, beta regression, generalized additive models, and spatial autocorrelation. Both temperature and humidity were consistently significant predictors (p < .001). Quadratic temperature terms and nonlinear GAM smooths were consistent with a thermal performance pattern, with infection peaking at intermediate ranges. Humidity exerted threshold-like effects, with disproportionately higher prevalence at upper quantiles. Province-level random effects accounted for ∼18% of variance, underscoring spatial heterogeneity, while elevation was not significant once climatic variables were included. Exploratory spatial analysis (Global Moran's I = −0.476, p = 0.043) tentatively suggested dispersion rather than clustering; however, given the very small number of spatial units (n = 7), this result should be regarded as hypothesis-generating only. The beta mixed-effects model achieved moderate predictive accuracy under cross-validation (RMSE ≈ 0.109). Collectively, these findings indicate that nosemosis prevalence in the region is strongly associated with climatic variability, exhibits nonlinear and threshold-like responses, and shows pronounced spatial heterogeneity. We emphasize that these are statistical associations and that unmeasured factors (e.g., colony strength, nutrition, pesticide exposure, management practices, and co-infections) likely contribute to the unexplained variance. By integrating hierarchical, nonlinear, and spatial approaches, this study establishes a climate-associated epidemiological assessment of nosemosis risk in A. mellifera and highlights the importance of localized climatic suitability under ongoing climate change.
- MakaleYalnız künye2026Determination of the Effects of Virtual Reality-Based Videos on Symptoms, Vital Signs and Comfort in Hemodialysis Patients
Coşar, Dilan Aktepe; Bingöl, Nuray
Introduction: This pioneering study aimed to examine the effects of virtual reality (VR)-based videos on symptoms, vital signs, and comfort levels in hemodialysis patients, offering an innovative approach to improving their quality of life. Methods: This randomized controlled pretest-posttest study included 54 hemodialysis patients treated at “X” Hospital between June 2024 and May 2025. Twenty-five patients were assigned to the intervention group, 25 to the control group, and 4 to a pilot group. Data were collected at baseline and after 8 weeks. Statistical analyses included t-tests, repeated measures ANOVA, Mann–Whitney U, and Friedman tests. Results: In the intervention group, Dialysis Symptom Index (DSI) scores decreased significantly (p < 0.05), and comfort levels increased significantly (p < 0.05) across all subscales of the Hemodialysis Comfort Scale Version II. Vital signs remained within normal ranges before and after the intervention. Conclusion: This study revealed that the use of virtual reality headsets significantly influenced dialysis symptom index scores and comfort levels. The intervention effectively reduced dialysis symptoms and significantly increased patient comfort. No significant differences were observed in vital signs.
