Stroke is the leading cause of disability worldwide. It is known that atrial fibrillation and left atrial enlargement contribute ischemic stroke, and mean platelet volume (MPV) increases in patients with ischemic stroke and atrial fibrillation. We aimed to determine whether higher MPV is associated with ischemic stroke in patients with sinus rhythm. We evaluated 74 patients in sinus rhythm and with ischemic stroke (Group 1) and 90 age-matched and sex-matched healthy individuals as control group (Group 2). After physical and echocardiographic examination, 24-48 h Holter monitoring and complete blood counts were studied. There were no statistically significant differences in age, sex rates, and comorbidities between groups. Left atrial diameter was higher in Group 1 than Group 2 (P=0.001), but both were in normal range. MPV was significantly higher in Group 1 (P<0.001) and was an independent determinant [odds ratio (OR): 1.840; P<0.001; 95% confidence interval (CI) 1.330-2.545] of ischemic stroke with left atrial (OR: 1.138; P=0.006; 95% CI 1.037-1.248). In conclusion, higher MPV is associated with acute ischemic stroke in patients with sinus rhythm and without heart failure or left atrial enlargement. MPV and left atrial diameter are independent predictors of ischemic stroke in this patient population. Copyright (C) 2016 Wolters Kluwer Health, Inc. All rights reserved.

Background - Mean platelet volume (MPV) is an indicator of platelet activation. The pathophysiology of the primary and secondary Raynaud's Phenomenon (RP) have not been completely established. The aim of this study was to investigate the relationship between MPV and RP. Materials and Methods - Our study was a prospective randomized study carried out from January 2011 to March 2012. The study group consisted of 39 patients: 27 (70%) patients having primary, 12 (30%) patients having secondary RP. An age-, gender-, and body mass index-matched control group consisted of 40 healthy participants. We compared the MPV in patients with RP and control participants statistically. Results - MPV of RP group was 8.79 +/- 1.37 femtoliter (fL) while MPV of control group was 8.39 +/- 1.36 fL. Comparison of MPV of RP group and control group showed no difference (p = 0.274). The mean of MPV was significantly higher among patients with secondary RP (9.76 +/- 1.68 fl) when compared with patients with primary RP (8.37 +/- 0.96 fl) (p = 0.018). Conclusion - The results of our study suggest that MPV may be used as a marker in secondary RP.

Relationship between platelet indices and international normalized ratio in patients with non-valvular atrial fibrillation

Atrial fibrillation (AF) is the most common sustained arrhythmia and associated with adverse outcomes and increased risk for thromboembolic events. Warfarin is still the most extensively prescribed oral anticoagulant in AF to prevent ischemic complications. We aimed to determine the differences at platelet indices with warfarin usage layered by International Normalized Ratio (INR). A total of 250 patients with permanent non-valvular AF (mean age 70.2 +/- 9.1; 153 female) were divided into two groups. Group 1 included 125 patients whose INR is between 2.0 and 3.0 (called as effective'') and Group 2 included 125 patients whose INR is <2.0 (called as ineffective''). Also 123 age- and sex-matched individuals in sinus rhythm enrolled as control group (Group 3). After physical and echocardiographic examination, complete blood counts and INR were studied. There was no statistically significant difference in age, sex, co-morbidities and medications, also hemoglobin, white blood cell and platelet counts among the groups. The CHA(2)DS(2)-VASc scores were similar between Groups 1 and 2. The mean platelet volume (MPV), platelet distribution width (PDW) and plateletcrit (PCT) were significantly higher in Group 2 than Groups 1 and 3 and similar between Groups 1 and 3. MPV was positively correlated with PDW and PCT and also inversely correlated with INR value and platelet count. On multivariate logistic regression analysis, MPV, PDW and PCT were independent predictors of ineffectiveness of INR. The results of this study showed that MPV, PDW and PCT are increased in patients with non-valvular AF without effective warfarin treatment. Warfarin usage adjusted by INR is associated with lower values of these platelet indices, even lower as the values of subjects in sinus rhythm. MPV, PDW and PCT are independent predictors of INR ineffectiveness and seem to be useful parameters for monitoring the effectiveness of warfarin treatment.

Is Increased Mean Platelet Volume a Risk Factor in Patients with Acute Deep Vein Thrombosis?

Kalkan, Asim; Memetoglu, Mehmet Erdem; Bilir, Ozlem; Ersunan, Gokhan; Kutlu, Rasim; Tutar, Nuri

Objectives The purpose of this retrospective study was to compare the mean platelet volume between the patients with acute deep vein thrombosis (DVT) and control subjects. Methods Data were collected retrospectively from the patients' medical records. The study group consisted of 110 patients with newly diagnosed acute DVT in the lower extremities. An age, gender, and body mass index-matched control group consisted of 98 patients with normal duplex ultrasound findings. We compared the mean platelet volume in patients with acute DVT and control participants. Results There was no significant difference between the study and control groups in mean platelet volume (8.8 +/- 1.6 vs 8.7 +/- 1.3 fl, respectively; p>0.05). Age (p=0.899), body mass index (p=0.43), gender (p=0.466), and platelet count (p=0.886) were statistically similar in both groups. Conclusions There was no significant difference in mean platelet volume between patients with acute DVT and the control group. According to our result, we propose that mean platelet volume should not be considered as an index of acute DVT.

Effect of enhanced external counterpulsation treatment on mean platelet volume in patients affected by ischemic chronic heart failure

Ertas, Gokhan; Kozdag, Guliz; Emre, Ender; Akay, Yasar; Ural, Dilek; Hebert, Kathy

Mean platelet volume (MPV) is increased in chronic heart failure (CHF) and is an independent predictor of mortality in CHF patients. It is not known whether enhanced external counterpulsation (EECP) therapy leads to decreased MPV values or not. The purpose of this study was to examine the effects of EECP on platelet count and MPV values and to assess the influence of MPV on the risk of death and recurrent ischemic events in ischemic CHF patients. A total of 68 ischemic heart failure patients with CHF symptoms and refractory angina pectoris were included in the study, 47 consecutive patients (39 males and eight females) aged 44-82 years. Although follow-up period started after completion of EECP in treated patients, control group follow-up started at the end of 7-week treatment without EECP. All patients were monitored for a mean duration of 13 +/- 8 months (range, 1-36 months). The primary endpoints of the study were effects of EECP treatment on platelets after treatment period (7 weeks) and the recurrence of ischemic events. Secondary endpoint was cardiovascular death during the follow-up period. We observed a significant increase in platelet count and decrease in MPV levels (P=0.044 and P=0.004, respectively) in the control group. There were no significant differences in platelet count and MPV levels in the EECP group (P>0.05). After the treatment period, New York Heart Association functional classification (2.60 +/- 0.75 vs. 1.72 +/- 0.68, P<0.001) and Canadian Cardiovascular Society functional classification of angina (2.50 +/- 0.90 vs. 1.60 +/- 0.74, P<0.001) improved in patients with EECP treatment. In our study, we found that EECP therapy had a neutral effect on MPV values and platelet count Blood Coagul Fibrinolysis 23:127-131 (C) 2012 Wolters Kluwer Health vertical bar Lippincott Williams & Wilkins.
