The expansion of universal health coverage in Türkiye has intensified pressure on public healthcare providers through high outpatient volumes that strain capacity and financial sustainability. Remote healthcare delivery has emerged as a potential strategy to improve efficiency and access. However, limited evidence exists on how remote outpatient care should be implemented across medical specialties within a universal payer system, or how alternative implementation strategies perform financially. This study evaluates the specialty-specific feasibility and financial impact of remote outpatient care using comprehensive administrative data from a large public training and research hospital in Izmir, supplemented by qualitative input from clinical specialists. The analysis includes 577,984 outpatient visits recorded in 2022 across major medical specialties. Eligibility for remote delivery is identified using observable clinical proxies—imaging requests, laboratory tests, and prescription issuance—to capture visits not requiring physical examination or procedural intervention. Results show that 121,248 visits (21.0%) are suitable for remote delivery, accounting for 16.2% of expenditures ($0.68 million out of $4.20 million). Remote eligibility varies substantially across specialties, with high suitability in dermatology and infectious diseases but limited applicability in device-dependent fields such as pulmonology. Four implementation scenarios are simulated over a 10-year horizon: Regulatory Compliance, Evidence-Based Specialty-Specific, Follow-Up Only, and Hybrid Progressive. The Regulatory Compliance scenario yields the shortest break-even period (3 years) and highest ROI (1185%). The Hybrid Progressive scenario, incorporating phased specialty expansion and AI-supported triage, requires higher upfront costs but generates the largest cumulative net savings ($1.39 million). The findings demonstrate that financially sustainable remote outpatient care can be achieved through carefully designed, specialty-specific implementation strategies.
- MakaleAçık Erişim2026The Effect of Teamwork Attitudes on Job Performance: A Study Among Nurses; [Ekip Performansı Tutumunun İş Performansına Etkisi: Hemşireler Örnekleminde Bir Araştırma]
Köse, Aslı
Aim: To evaluate the impact of nurses’ teamwork attitudes on their job performance. Method: A descriptive, correlational quantitative research design was employed. The sample consisted of 350 nurses who worked at a city hospital in Turkey and voluntarily participated. Data were analyzed using SPSS 25 to assess differences in teamwork attitudes and job performance according to nurses’ educational levels and years of professional experience. Results: The analysis revealed no statistically significant differences in teamwork attitudes or job performance based on educational level. The mean total score for teamwork attitudes was (3.27±0.80), while the mean total score for job performance was 3.25 (SD=0.82). A strong positive correlation was found between teamwork attitudes and job performance (r = 0.764; p < 0.001). A strong positive correlation was found between teamwork attitudes and job performance, with the former accounting for 58.4% of the variance in the latter. Conclusion: This study found that nurses working in a city hospital exhibited moderate attitudes toward teamwork and moderate job performance, and that these two measures were strongly positively correlated.
- MakaleAçık Erişim2025Navigating stress with creativity: The moderating role of conflict-induced creativity on job satisfaction among entrepreneurs
Özsungur, Fahri; Bekar, Fevziye
Background: Entrepreneurs frequently encounter high levels of job stress, which can undermine their job satisfaction. Although the negative consequences of stress are well-known, less attention has been paid to the role of Conflict-Induced Creativity in buffering these effects. Objective: This study investigates how Conflict-Induced Creativity moderates the relationship between job stress and job satisfaction among entrepreneurs, addressing a gap in understanding adaptive mechanisms in high-pressure work environments. Methods: Using a quantitative design, data were collected from 453 entrepreneurs through convenience sampling. Statistical analyses included factor analyses, correlations, regressions, and moderation testing via PROCESS MACRO in SPSS v.22. Results: Validated scales assessed job satisfaction (α = 0.934), job stress (α = 0.919), and Conflict-Induced Creativity (α = 0.832). Job stress negatively predicted job satisfaction (β = −0.126, p < 0.001), while Conflict-Induced Creativity emerged as a strong positive predictor (β = 0.688, p < 0.001). Moderation analysis confirmed that Conflict-Induced Creativity significantly buffered the adverse effects of stress on satisfaction (b = 0.147, 95% CI [0.048, 0.245], p < 0.05). Conclusion: Conflict-Induced Creativity plays a protective role in stressful entrepreneurial contexts. Practically, fostering a work culture that supports Conflict-Induced Creativity can enhance resilience and job satisfaction among entrepreneurs, offering actionable insights for leadership development, training programs, and organizational policy.
- Kitap BölümüYalnız künye2024The effect of digitalization in healthcare on service sustainability
Köse, Asli
This research evaluates how digitalization affects the sustainability of healthcare services. To achieve this, research articles published in the PUBMED and WEB OF SCIENCE databases up to June 30, 2024, were compiled. The PRISMA guidelines were followed when presenting the research findings. In the study evaluating the impact of digitalization on healthcare sustainability, a systematic review method was employed. The systematic review addresses the question of how digitalization affects the sustainability of healthcare services. The research identified two main themes. The first theme focuses on evaluating the effects of digitalization on sustainability within hospitals, healthcare workers, and patient populations. The second theme examines the impact of digitalization on sustainability concerning societal health, health worker well-being, and the overall health system.
- MakaleAçık Erişim2022The effect of the COVID-19 pandemic on the professional perception and image of nursing among nurses working in surgical units: An online study
Sayilan, Aylin Aydin; Kulakac, Nursen
BACKGROUND: Understanding the effects of the COVID-19 pandemic on the perception and image of the profession are key factors in nurses' perceptions regarding their roles and their future. OBJECTIVE: The purpose of this study was to determine the effect of the COVID-19 pandemic on the perception and image of the nursing profession among nurses working in surgical units. METHODS: This cross-sectional, correlational study was conducted with 216 nurses using the random sampling method between 10 and 30 March, 2022. The study data were collected using an information form, the Perception of Nursing Professional Scale (PNPS), and the Scale for the Image of the Nursing Profession. RESULTS: The nurses' mean age was 32.53±9.16 years (min 21, max 57) and their mean professional experience was 16.44±9.80 years (min 1, max 44). The mean PNPS score was 73.12±9.46, and the mean Scale for the Image of the Nursing Profession score was 171.11±11.16. Female nurses had significantly higher positive perceptions of nursing levels than male nurses. Gender, having a history of COVID-19, marginalization during the pandemic, and perception of the nursing profession significantly affected image perceptions of nursing at multiple linear regression analysis. These variables explained 43% of the total variance. CONCLUSION: Nurses working in surgical units during the COVID-19 pandemic exhibited a good level of professionalism and image perception concerning nursing.
