To evaluate the predictive value of immature granulocytes in intestinal necrosis in incarcerated hernia. The study included patients who were operated on due to incarcerated hernia. The cases were separated into 2 groups as those with or without bowel resection. Age, gender, hernia type, side, white blood cell count, neutrophil count, neutrophil-lymphocyte ratio, red cell distribution width count, immature granulocyte count, platelet count, and platelet lymphocyte ratio were statistically compared between the groups. There were 27 cases (17.3%) in the group with bowel resection and 127 cases (82.7%) in the group without bowel resection. A statistically significant difference was determined between the groups in respect of the hernia type, white blood cell count, neutrophil count, neutrophil-lymphocyte ratio and immature granulocyte count (p<0.05). The cut-off value of the immature granulocyte count was found to be 0.065, with sensitivity of 63% and specificity of 80%. In multivariate analysis, age over 65 years and immature granulocyte count>0.065 were found to be independent predictors of necrosis development. The immature granulocyte value is a fast, easily accessible and reliable marker that can be used to predict intestinal necrosis in cases of incarcerated hernia.

Clinical Preferences of Turkish Urologists in Screening and Diagnosis of Prostate Cancer and Adherence to European Association of Urology Guidelines

Yildiz, Huseyin Alperen; Demirkiran, Engin Denizhan; Madendere, Serdar; Deger, Muslim Dogan

Objective: To compare the clinical preferences of urologists in prostate cancer screening and diagnosis with current guideline recommendations.Materials and Methods: The study is based on an online survey that consists of 21 single response or multiple response questions. By e-mail, 2,305 certified urologists in active practice as well as urology residents in their last year of training were invited to participate in the study. Descriptive statistics were used to analyze practice patterns and demographics. The respondents were divided into groups based on their experience in urology, hospital type and academic title. Results: Our results show that preferences of the majority of urologists conflicts with recommendations of European Association of Urology Prostate Cancer Guidelines regarding prostate biopsy method, management of a patient with high prostate-specific antigen value, use of additional test and performance scoring systems, role of multiparametric prostate magnetic resonance imaging. Conclusion: Urologists act in the direction of their habits or clinical experience rather than current guidelines regardless of their experience. Both clinicians and professional organizations should work on what can be done about the reflection of rapidly changing scientific knowledge on the field and the improvement of the health service provided.

Aim: Pulmonary embolism (PE) is a life-threatening condition that requires early diagnosis and treatment. There is still a need for precise and non-invasive biomarkers that can be used to diagnose PE. This study aimed to determine the diagnostic value of Cardiotrophin-1 (CT-1) in patients with acute PE and ascertain the relationship between serum CT-1 level and disease prognosis and severity. Materials and Methods: In this prospective study, the serum CT-1 levels of 165 patients with suspected acute PE were measured. The patients were divided into two groups, PE (+) and PE (-), based on their computed tomography angiography results. Their serum CT-1 levels were compared. Results: The CT-1 levels of the PE (+) group (median=4.18 pg/mL, range=3.57-9.55 pg/mL) was non-significantly (p=0.08) different from those of the PE (-) group (median=4.38 pg/mL, range=3.57-51.64 pg/mL). Based on the severity of the condition, there were significant differences (p=0.001) in the CT-1 levels of the patients in the intermediate-low risk group (median=4.19 pg/mL, range=3.62-5.32 pg/mL), intermediate-high risk group (median=4.06 pg/mL, range=3.57-9.31 pg/mL) and high risk group (median=5.01 pg/mL, range=3.89-9.55 pg/mL). Based on the clinical course, the CT-1 levels of the patients in the good clinical course group (median=4.15 pg/mL, range=3.57-9.31 pg/ml) were significantly different (p=0.004) from those in the poor clinical course group (median=4.53 pg/mL, range=3.66-9.55 pg/mL). Conclusion: CT-1 level was not observed to be a precise biomarker for the diagnosis of acute PE. However, our study demonstrated that the serum CT-1 levels can be used as a biomarker for ascertaining the severity and prognosis of patients with PE.
