Clinical Preferences of Turkish Urologists in Screening and Diagnosis of Prostate Cancer and Adherence to European Association of Urology Guidelines

Yildiz, Huseyin Alperen; Demirkiran, Engin Denizhan; Madendere, Serdar; Deger, Muslim Dogan

Objective: To compare the clinical preferences of urologists in prostate cancer screening and diagnosis with current guideline recommendations.Materials and Methods: The study is based on an online survey that consists of 21 single response or multiple response questions. By e-mail, 2,305 certified urologists in active practice as well as urology residents in their last year of training were invited to participate in the study. Descriptive statistics were used to analyze practice patterns and demographics. The respondents were divided into groups based on their experience in urology, hospital type and academic title. Results: Our results show that preferences of the majority of urologists conflicts with recommendations of European Association of Urology Prostate Cancer Guidelines regarding prostate biopsy method, management of a patient with high prostate-specific antigen value, use of additional test and performance scoring systems, role of multiparametric prostate magnetic resonance imaging. Conclusion: Urologists act in the direction of their habits or clinical experience rather than current guidelines regardless of their experience. Both clinicians and professional organizations should work on what can be done about the reflection of rapidly changing scientific knowledge on the field and the improvement of the health service provided.

Urologists Are No Longer the Primary Surgeons for Several Urologic Operations: A National Survey Among the Turkish Urologists

Madendere, Serdar; Deger, Muslim Dogan; Demirkiran, Engin Denizhan; Yildiz, Huseyin Alperen

Objective: To investigate the role, interest, and competence of the Turkish urologists for urologic surgeries which are also performed by the other surgeons. Materials and Methods: We conducted a 22-item online survey and invited urologists by e-mail. Questions were asked to evaluate the attitudes, performances, and capabilities of the urologists in surgeries which could be performed by the other surgeons. Results: A total of 305 responses were evaluated. The female incontinence surgery, pediatric urologic surgery, and adrenal surgery are mostly performed by the urologists in their hospitals. Turkish urologists mostly do not perform the following surgeries and interventions themselves and refer patients to the other surgeons for a percutaneous nephrostomy (47.2%), urinary diversions using the intestinal segments (44.9%), kidney transplantation (11.8%), and the transgender surgeries (7.2%). Almost half of the urologists do not perform adrenal surgeries (50.2%). Most urologists agree that they should be the main surgeons for all these surgeries. Conclusion: Urologists are no longer the main surgeons for some surgeries which are included on the urologic curriculum. Many urologists avoid performing the major urologic surgeries. Urology educators should familiarize the new urologists with all the urologic surgeries.

Urologists' Role and Attitude in the Systemic Treatment of Urologic Cancers

Madendere, Serdar; Deger, Muslim Dogan; Demirkiran, Engin Denizhan; Yildiz, Huseyin Alperen

Objective: The goal of this study was to investigate Turkish urologists' role and attitude toward the systemic treatment of urologic cancers. Materials and Methods: An 18-item survey was e-mailed to 2305 certified Turkish urologists. The survey included questions designed to evaluate urologists' knowledge of chemotherapy procedures and their ability to manage complications, as well as how many of them perform systemic therapies and believe urologists should perform these therapies for urologic cancers. Results: We evaluated 305 responses. Most urologists (89.6%) perform hormonotherapy for prostate cancers. Further, the majority refer patients to medical oncologists for chemotherapy. Moreover, 65.9%, 70.8%, and 72.1% of the urologists believe that only medical oncologists should deliver chemotherapy for prostate, bladder, and testicular cancers, respectively. Only 23.9% of urologists believe they are competent to perform chemotherapy, while 16.7% believe they are capable of dealing with chemotherapy complications. Conclusion: Turkish urologists have a lack of performance and interest in administering systemic therapies in urologic cancers. We want to emphasize that urology is more than just surgery. Urologists should be involved in all stages of cancer treatment.

Erectile dysfunction treatment with Phosphodiesterase-5 Inhibitors: Google trends analysis of last 10 years and COVID-19 pandemic

    Deger, Muslim Dogan; Madendere, Serdar

Objectives: We aimed to analyze the trend change of the most popular Phosphodiesterase-5 Inhibitors (PDE5i) over time and geography by using Google Trends (GT) data in 10 years period and COVID-19 pandemic. Materials and methods: GT is able to generate a line-graph, showing how interest has increased or decreased over a period within specific territories. The search values for specific terms are indexed as relative search volume (RSV), which is presented on a scale from 0-100. Avarage annual percentage change (AAPC) and RSV were analyzed to evaluate gain or loss of interest in trends. Search terms were generated for Food and Drug Administration (FDA)-approved PDE5i; tadalafil, sildenafil, vardenafil, avanafil, and their most-used brand names. The data was within worldwide from 1 January 2010, to 31 December 2020, using the global query category. Results: The overall interest in PDE5i has doubled. Sildenafil has become the most trend PDE5i of today with a regular increase (AAPC: 0.016, p < 0.01). Although the search trend of tadalafil remained almost constant until 2014, the rate of increase in the last 6 years raised and tadalafil has become the 2nd most popular PDE5i recently (AAPC: 0.007, p < 0.01). For vardenafil there has been a decreased interest (AAPC: -0.009, p < 0.01). There is no significant change in avanafil trend (AAPC: 0.000, p: 0.5). All PDE5i interest on GT decreased notably from February to June 2020. But after June, search trends reached the level before the COVID-19 period in a month. Conclusions: These findings show us, with its increasing prevalence, erectile dysfunction (ED) has become a major public health problem. People from different geographies search the Internet for ED treatment options. Patients should be informed that ED may be the first sign of many comorbid diseases, and patients with ED should be referred to a health institution for diagnosis and treatment.
