Use of Piezoelectric Devices in Closed Structural Rhinoplasty

    Kaderi, Sina; Ekinci, Can

Background Closed rhinoplasty techniques are gaining popularity once again as more and more patients demand them. The use of piezoelectric devices (PEDs) is also another popular practice in rhinoplasty surgeries recently to reduce postoperative edema and bruising. Objectives The aim of this study was to propose a new technique in which all osteotomies in closed rhinoplasty surgeries can be performed using PEDs. Methods A comparison was made between 274 patients who underwent closed piezo (CloPi) rhinoplasty and 120 patients who underwent closed rhinoplasty using conventional osteotomy. In the CloPi technique, all osteotomies, including lateral and transverse osteotomies, were performed with PEDs using straight and 90 degrees curved tips. Preoperative and postoperative results obtained with the Rhinoplasty Outcome Evaluation (ROE) questionnaire were compared to evaluate aesthetic and functional results. Results In the CloPi group, the mean age was 30.8 years (range, 18-60 years) and the mean follow-up period was 19.6 months, whereas in the control group, the mean age was 33.5 years (range, 18-61 years) and the mean follow-up period was 29.3 months. Significantly better results were obtained in the ROE questionnaire compared with preoperative values in both groups (P < .001). When comparing the postoperative-preoperative change in ROE scores between the 2 groups, the improvement observed in the CloPi cohort was significantly greater for all items (P < .001), except for Question 2-assessing the ability to breathe-where no significant difference was detected (P = .231). Conclusions CloPi rhinoplasty is an easy and reliable technique in which all osteotomies can be performed with PEDs. Successful surgical results can be achieved with wide surgical dissection.Level of Evidence 3 (Therapeutic)

Multiple Mantle Melting and Metasomatism during the Initial Stages of Subduction: Multi-Geochemical and Os-Isotope Evidence from Well-Preserved Kızıldağ Ophiolite, southern Türkiye

Uysal, Ibrahim; Sen, Ahmet D.; Su, Ben-Xun; Ersoy, Yalcin; Demir, Yilmaz; Pan, Qi-Qi; Hu, Wen-Jun; Bagci, Utku; Godard, Marguerite

The processes governing ophiolite formation and evolution remain incompletely understood. The K & imath;z & imath;lda & gbreve; ophiolite, a well-preserved remnant of the Neo-Tethyan oceanic lithosphere in the Anatolian region, southern T & uuml;rkiye, offers critical insights into subduction-related processes and geodynamics. This study presents new element data for mineral and whole-rock samples, combined with Re-Os isotopic compositions of mantle peridotites and associated crustal rocks and extrusives. Petrographic observations identify two distinct groups of peridotites: (1) those with very low modal clinopyroxene, interpreted as highly depleted harzburgites and preserving primary mineral assemblages, and (2) depleted harzburgites containing secondary clinopyroxene with elevated trace element contents, inferred to have crystallized from percolating melts and thus reflecting metasomatic modification. Spinel and orthopyroxene compositions indicate 17-25% degrees of partial melting, with evidence for melting in both garnet and spinel stability fields. However, mineral and whole-rock compositions also record subsequent cryptic and modal metasomatism. Crustal rocks and extrusives exhibit geochemical signatures transitional between fore-arc basalts and boninites, consistent with subduction initiation settings. Variations in Os isotope ratios (187Os/188Os) in crustal rocks further reveal melt-rock interactions and the incorporation of radiogenic Os from subducted oceanic crust. The integration of petrographic, geochemical, and isotopic data supports a four-stage tectonic model for subduction initiation and mantle wedge evolution, consistent with global models proposed for supra-subduction zone (SSZ) ophiolites, particularly those of the Tethyan domain. These findings provide new constraints on mantle melting, metasomatism, and subduction initiation processes and contribute to a broader understanding of SSZ mantle evolution in both regional and global contexts.

Behcet's disease with aneurysm of internal iliac artery and percutaneous treatment

    Memetoglu, Mehmet Erdem; Kalkan, Asim

Behcet's disease (BD), described in 1937 by Hulusi Behcet, is a multisystemic inflammatory disorder. In this paper, we report a large pseudoaneurysm of the internal iliac artery in a patient who had BD, and was admitted to emergency service complaining of leg pain and the percutaneous treatment of the pathology which followed.

Radiological characterisation of Artvin and Ardahan provinces of Turkey

    Kucukomeroglu, B.; Yesilbag, Y.O.; Kurnaz, A.; Celik, N.; Cevik, U.; Celebi, N.

Indoor radon concentration measurements were carried out and corresponding annual effective doses due to exposure to indoor radon were determined in Artvin and Ardahan provinces located in the eastern part of Turkey. The measurements were performed for four seasons in order to determine the seasonal fluctuations mostly observed in indoor environments. Indoor radon concentration values were observed to range from 21 to 321 Bq m-3 for the Artvin province and from 53 to 736 Bq m-3 for the Ardahan province. It was observed that minimum indoor radon concentration values were obtained in summer, while the highest ones were observed in winter. Indoor radon concentration values of the current study were compared with those of other provinces in Turkey. As elevated indoor radon concentrations are mostly correlated with high 238U activity concentrations in soil, a total of 57 and 33 soil samples were collected from the Artvin and Ardahan provinces, respectively, to determine 238U activity concentration as well as the concentration of 232TH and 40K-naturally occurring radionuclides. It was also observed that soil samples collected from the study areas contained 137Cs as an artificial radionuclide.

Combination technique of tumescent anesthesia during endovenous laser therapy of saphenous vein insufficiency

Memetoglu, Mehmet Erdem; Kurtcan, Serpil; Kalkan, Asim; Özel, Deniz

Objective: This study aimed at evaluating the efficiency of applying tumescent anesthesia before an ablation procedure and continuously during endovenous laser treatment (EVLT) (combined tumescent technique) of saphenous vein insufficiency to reduce the patients pain and discomfort. Methods: Twenty-five patients with 31 duplex-confirmed great saphenous vein insufficiency underwent endovenous laser (940 nm) varicose vein treatment between December 2009 and April 2010. The patients were randomized in two groups. In group A (15 patients) tumescent anesthesia was used before the ablation procedure and in group B (10 patients) tumescent anesthesia was used before and continuously during the procedure. Patients were scheduled for a three-day examination after EVLT to assess the level of pain experienced. Results: According to the statistic analysis we observed that the mean level of pain score for the patients given classical tumescent anesthesia showed a higher level (P=0.003) compared with the patients given tumescent anesthesia in the combined procedure. Conclusion: Combination technique of administering tumescent anesthesia before ablation and continuously during the EVLT procedure may be an alternative way to reduce the patient's peroperative pain and discomfort.
