Background Existing CHA(2)DS(2)-VASc-based scores incompletely capture the prognostic impact of chronic obstructive pulmonary disease (COPD) and of graded renal dysfunction in patients undergoing transcatheter aortic valve implantation (TAVI). We acknowledge that the score's development in a high-risk, high-comorbidity cohort may produce optimistic performance estimates that require attenuation through external validation. We aimed to develop a novel COPD-augmented R(2)CHA(2)DS(2)-VA (COPD-R(2)CHA(2)DS(2)-VA) score that systematically integrates quantitative renal function and COPD, and to internally assess its discriminative performance for one-year all-cause mortality and major adverse cardiovascular events (MACE). Methods In this single-center, retrospective cohort study, 622 consecutive patients undergoing TAVI for severe aortic stenosis (2018-2024) were analyzed. The COPD-R(2)CHA(2)DS(2)-VA score was constructed as a prespecified clinical tool by augmenting the R(2)CHA(2)DS(2)-VA framework with COPD (1 point) and refining renal dysfunction into two graded estimated glomerular filtration rate strata (eGFR 30-59 mL/min/1.73 m & sup2;: 1 point; eGFR < 30: 2 points). Discriminative performance was evaluated using receiver operating characteristic curve analysis with bootstrap internal validation (2,000 resamples) and compared with established CHA(2)DS(2)-VASc-based scores. Multivariable logistic regression assessed the independent prognostic value of the score treated as a continuous variable. Results The COPD-R(2)CHA(2)DS(2)-VA score demonstrated encouraging discriminative ability for one-year all-cause mortality (apparent area under the curve [AUC] 0.946, 95% CI 0.926-0.971). As a prespecified tool with fixed component weights, bootstrap internal validation confirmed negligible optimism (mean optimism - 0.00014), with bias-corrected AUC identical to the apparent value. The addition of COPD significantly improved mortality discrimination over the R(2)CHA(2)DS(2)-VA score (Delta AUC + 0.033, p = 0.0001). For one-year MACE, the apparent AUC was 0.806 (95% CI 0.765-0.852); the addition of COPD did not significantly enhance MACE discrimination (Delta AUC + 0.001, p = 0.853). In multivariable analysis, each one-point increase in the COPD-R(2)CHA(2)DS(2)-VA score was independently associated with 3.52-fold higher odds of one-year mortality (95% CI 2.45-5.06, p < 0.001) and 2.02-fold higher odds of MACE (95% CI 1.67-2.46, p < 0.001). The bias-corrected AUC of the full multivariable model was 0.973 (95% CI 0.962-0.984) for mortality and 0.795 (95% CI 0.752-0.841) for MACE. Conclusions The COPD-R(2)CHA(2)DS(2)-VA score demonstrated encouraging discriminative ability for one-year all-cause mortality after TAVI in this single-center development cohort (apparent AUC 0.946). However, this high discriminative performance is likely influenced by the elevated comorbidity burden in our cohort and the single-center design, and should be interpreted as an upper-bound estimate. The addition of COPD provides a statistically significant but clinically modest incremental improvement for mortality (Delta AUC + 0.033, p = 0.0001), with no significant enhancement for MACE (Delta AUC + 0.001, p = 0.853). These findings are preliminary and hypothesis-generating; urgent external validation in diverse, prospective cohorts is essential before any clinical application can be considered. At present, this score should be regarded as a development-phase, hypothesis-generating tool that complements rather than replaces guideline-recommended instruments such as STS-PROM and EuroSCORE II. The score's clinical utility remains unproven.

Background Inspiratory muscle training (IMT) has been shown to enhance respiratory efficiency, postural stabilization, and neuromuscular coordination in athletic populations. However, its effects in precision sports such as air pistol shooting remain insufficiently explored. This study investigated the impact of a 4-week IMT program on respiratory muscle strength, explosive lower-limb performance, reaction time, and shooting accuracy in competitive air pistol athletes. Methods Twenty trained male air pistol athletes (age 18-35 years) were randomly assigned to an IMT group (n = 10) or a control group (n = 10). The IMT group performed supervised inspiratory muscle training twice daily (30 breaths/session), six days per week, for four weeks, using a threshold-loading device set initially at 40% of maximal inspiratory pressure (MIP), with progressive overload applied weekly. Both groups maintained their regular shooting training routines. Pre- and post-intervention assessments included countermovement jump (CMJ) and squat jump (SJ) performance (OptoJump), visual and auditory reaction time (Cognitech), and shooting performance evaluated via the SCATT system. A two-way repeated-measures ANOVA (group & times; time) was used for statistical analysis. Results Significant group & times; time interactions were observed for CMJ height (p < 0.001, eta & sup2;p = 0.618), CMJ power (p = 0.007, eta & sup2;p = 0.345), SJ height (p = 0.050, eta & sup2;p = 0.184), and SJ power (p = 0.050, eta & sup2;p = 0.181), indicating meaningful improvements in explosive performance in the IMT group. Shooting accuracy demonstrated a large and statistically significant interaction effect (p < 0.001, eta & sup2;p = 0.532), with substantial improvement observed only in the IMT group. Reaction time variables showed favourable but non-significant trends (p > 0.05). Conclusion A short-term IMT intervention significantly enhanced lower-limb explosive performance and shooting accuracy in competitive air pistol athletes without altering overall training load. These findings suggest that IMT may represent an effective, low-cost adjunct strategy to improve neuromuscular stability and fine-motor precision in precision-sport athletes.

Objective This research aimed to adapt Positive Attitudes Toward and Perceived Importance of Wearable Display Technology (PATPIWDT) as an Effective Learning Tool Among Nursing Students into Turkish and to examine the validity and reliability of its translation. Methods This methodological study included 698 nursing students recruited via convenience sampling. Construct validity was evaluated using Confirmatory Factor Analysis (CFA) with the Maximum Likelihood (ML) estimation method. Internal consistency was assessed using Cronbach's Alpha and McDonald's Omega coefficients. Results The model demonstrated excellent fit according to the confirmatory factor analysis results. The CFA results for the Positive Attitudes subscale indicated a good model fit (CMIN/DF = 4.79, GFI = 0.93, CFI = 0.97, RMSEA = 0.07, SRMR = 0.03). For the Perceived Importance subscale, the single-factor structure showed acceptable fit indices (CMIN/DF = 5.82, GFI = 0.98, SRMR = 0.03). As a result of examining the standardized loadings of the items, it was seen that the item factor loadings were 0.40 and above, indicating sufficient factor loading. Cronbach's alpha coefficients were 0.91 for the positive attitudes subscale and 0.84 for the perceived importance subscale. The respondents also demonstrated an above-average overall score, with a mean of 3.40 (SD = 0.69).Conclusion The Turkish version of the scale provides a dependable measure of nursing students' positive attitudes toward wearable screens and how important they consider this technology for future educational use.

Background It has been shown that neopterin levels in various body fluids can help predict the diagnosis of different malignancies. Although many studies have investigated the roles of serum neopterin concentrations, tryptophan metabolism, and the dietary inflammation index (DII) in various cancers, the connection between these factors and prostate cancer (PC) remains uncertain. The aim of this study was to investigate the predictive and prognostic roles of serum neopterin, and kynurenine levels, along with the kynurenine-to-tryptophan ratio (KTR) and DII in prostate cancer. Methods This study was conducted with 57 newly diagnosed prostate cancer patients, 55 benign prostate hyperplasia patients, and 56 healthy male individuals in the control group, all aged 40 years and older. Participants had specific anthropometric measurements taken, and three-day food records were kept. DII was calculated based on data for thirty-four available nutrients by using food records. Serum levels of neopterin, kynurenine, tryptophan, tumor necrosis factor alpha (TNF-alpha), and interferon-gamma (IFN-gamma) were measured using an enzyme-linked immunosorbent assay (ELISA) method. Results Serum neopterin (11.79 +/- 6.09), kynurenine (1655.48 +/- 1122.04), TNF-alpha (221.59 +/- 232.06) and IFN-gamma (137.68 +/- 107.11) levels were higher in the malignant group than in both the control (2.72 +/- 0.76, 352.40 +/- 69.33, 223.94 +/- 226.67, and 23.03 +/- 7.28, respectively) and benign groups (3.17 +/- 0.74, 377.94 +/- 109.05, 179.61 +/- 224.36, and 25.63 +/- 7.50, respectively) (p < 0.05). Another result of our study is that in the malignant group, serum neopterin levels exhibited significant positive correlations with serum kynurenine (r = 0.80, p < 0.0001), IFN-gamma (r = 0.86, p < 0.0001) and TNF-alpha levels (r = -0.27, p = 0.0453) and DII (r = 0.30, p = 0.0229), while they exhibited weak correlations with serum PSA, and other diet parameters. Additionally, kynurenine was the only parameter showing significant differences in mean values concerning surgical margins (p < 0.05). Conclusions The findings of this study support the hypothesis that serum neopterin and specific tryptophan catabolites (e.g., kynurenine), could serve as predictive and prognostic biomarkers for prostate cancer diagnosis. However, our results do not support the hypothesis that DII and certain dietary factors play a direct role in the development or diagnosis of prostate cancer.
- MakaleAçık Erişim2025Could GSTP1, PTEN and NKX3.1 gene expression be novel markers in the relationship between prostate cancer and epigenetics?
Koçak, Tevfik; Acar-Tek, Nilufer; Yesil, Suleyman; Ergün, Mehmet Ali; Icer, Yusuf
Background: Although numerous treatment modalities exist for Prostate Cancer (PCa) with a complex pathophysiology, a significant worldwide health concern for males, early identification remains the paramount factor influencing disease prognosis. Proposed new biomarkers, including GSTP1, PTEN, and NKX3.1, may provide predictive and prognostic functions, hence enhancing the efficacy of PSA in diagnosing prostate cancer. Epigenetic variables associated with nutrition influence the blood gene expression levels of the new biomarkers GSTP1, PTEN, and NKX3.1. This study aimed to elucidate the correlation between nutrition-related epigenetic variables and the expression of the tumor suppressor genes GSTP1, PTEN, and NKX3.1 in benign prostate and prostate cancer patients. Methods: This study was conducted with 41 newly diagnosed prostate cancer and 42 benign prostate patients, all aged 40 years and older. Participants had specific anthropometric measurements taken, and seven-day food records were kept. Serum levels of novel biochemical markers GSTP1, PTEN, and NKX3.1 were measured using an Enzyme-Linked Immunosorbent Assay (ELISA) method. Expression analyses of tumor-protective GSTP1, PTEN, and NKX3.1 genes were performed on prostate tissues obtained from fresh prostate biopsies taken from the groups and were associated with nutritional status. Results: A significant relationship was found between the serum levels of GSTP1, PTEN, and NKX3.1, which are important in the formation and treatment of prostate cancer, and Gleason grade, tumor stage, and surgical margin status (p < 0.05). However, significant relationships were found between many macro and micronutrients obtained from the dietary pattern and effective in epigenetic pathways and GSTP1, PTEN, and NKX3.1 gene expressions (p < 0.05). Conclusions: The findings of this investigation corroborated the notion that prospective novel markers, including GSTP1, PTEN, and NKX3.1, may serve predictive and prognostic functions in prostate cancer, hence enhancing the efficacy of PSA in its diagnosis. Dietary trends may influence the optimization of many epigenetic processes. Targeted therapeutic nutritional interventions designed to enhance gene expression will facilitate the formulation of prostate cancer prevention methods.

Wheat (Triticum aestivum L.) is one of the most significant crops and the backbone of food security worldwide. However, low wheat production remains a substantial concern in global agricultural systems. It can be attributed to several factors, including adverse climatic conditions, plant disease and poor soil quality. Recent efforts have explored bioinoculant applications as a promising approach to enhance wheat yield, trying to mitigate constraints essential for future wheat production and global food security. This study tested talc powder, wheat biochar, sugarcane bagasse biochar, and farmyard manure as carriers with two endophytic bacterial strains, Burkholderia phytofirmans PsJN and Bacillus spp. MN54 was applied to three wheat varieties (Ujala-16, Zincol-16, and Fathejang-16). The data was recorded at the seedling and maturity growth stages of plants. A pot experiment revealed significant improvements in plant growth following bioinoculant application compared to controls. Notably, the combination of sugarcane bagasse biochar with Bacillus sp. MN54 exhibited the most pronounced effects, promoting internodal length, spike length, tiller number per plant, grain yield per plant, and spikelets per spike. Additionally, talc powder with Bacillus sp. MN54 increased peduncle length, tiller number per plant, and spike length in Fathejang-16. These findings offer valuable insights into optimizing bioinoculant formulations for improved agricultural practices, adapting to climate change, and contributing to ensuring global food security.

Purpose: Visual feedback and music while warm up may help elicit peak performance, but its effect during the Wingate anaerobic test (WAnT) in futsal players is unexplored. Therefore, the aim of this study was to examine the effects of music and visual stimulus on WAnT performance in futsal players. Methods: The study included 17 young male futsal players (age, 21.53 +/- 1.32 years; height, 177.59 +/- 5.75 cm; 73.71 +/- 9.31 kg; BMI 23.342 +/- .47). The WAnT was administered using three distinct protocols: with music (M), without music (NM), and with music and visual feedback (MV) following a 10-min warm-up at 72-h intervals. After WAnT, the following parameters were evaluated: (a) peak power output: PPO (W), (b) relative peak power output: RPPO (W/kg), (c) mean power output: MPO (W), (d) relative mean power output: RMPO (W/kg), (e) fatigue index: FI (%), and (f) minimum power output MIPO (W). Results: MV indicated that futsal players' WAnT values, including PPO, RPPO, MPO, RMPO, and MIPO improved more than with other protocols (p < 0.05). However, there was no significant difference between protocols for FI (%) WAnT values (p > 0.05). Conclusions: Listening to music and watching visual feedback while warm up before performing WAnT performance suggest to coaches and futsal players.

Background: Propolis is a natural, resinous hive product that has several pharmacological activities. Its composition varies depending on the vegetation, climate, season and environmental conditions of the area from where it was collected. Surface enhanced laser desorption ionization time of flight mass spectrometry (SELDI-TOF MS) is a proteomic approach which has been used in cancer proteomics studies. Prostate cancer is one of the most commonly diagnosed cancers in men. It has shown that nutritional supplements rich in polyphenolic compounds such as propolis play a significant role in prostate cancer chemoprevention. The aim of this study is to evaluate if protein expression profile in PC-3 prostate cancer cell lines could be differentiated when incubated with dimethyl sulfoxide and water extracts of Turkish propolis. Results: The antioxidant potentials of dimethyl sulfoxide and water extracts of propolis were found in correlation with the amount of total phenolic compounds of them. Dimethyl sulfoxide and water extracts of propolis of 20 μg/mL reduced the cell viability to 24.5% and 17.7%, respectively. Statistically significant discriminatory peaks between control PC-3 cells and dimethyl sulfoxide extract of propolis-treated PC-3 cells were found to be the proteomic features at m/z 5143, 8703, 12661, 20184 and 32794, detected by CM10 ProteinChip, and the peak at m/z 3772, detected by Q10 ProteinChip. Between control PC-3 cells and water extract of propolis-treated PC-3 cells, statistically significant discriminatory peaks were found to be the proteomic features at m/z 15846, 16052 and 24658, detected by CM10 ProteinChip and the peaks at m/z 10348, 10899 and 11603, detected by Q10 ProteinChip. Conclusions: It was concluded that dimethyl sulfoxide and water extracts of Turkish propolis may have anti-proliferative activity through differentiating protein expression profile in PC-3 prostate cancer cell lines along with their antioxidant capacity.
