Aged people with chronic diseases dying: An investigation of metaphorical perceptions and cognitive structure; [Sterbende ältere Menschen mit chronischen Erkrankungen: Eine Untersuchung metaphorischer Wahrnehmungen und kognitiver Strukturen]

Demir Çam, Sevgi; Uzun, Sevda; Aydin, Esra; Altindaş, Sueda

Purpose: This study was designed to examine the metaphorical perceptions and cognitive structures of aged individuals with chronic diseases towards death. Methods: The research was conducted as a qualitative study based on metaphor analysis. Data were collected using a semi-structured interview form that investigated the metaphorical perceptions of 15 individuals with chronic diseases towards death and a form that was asked to be explained with drawing technique. In the semi-structured interview form “Death is like … because …”, metaphor expression was included, and in the other part “Describe your thoughts about death with drawings”, the form containing the expression was used. The data were analyzed using the content analysis technique. The study was conducted and reported according to the COREQ checklist. Results: It was determined that the old individuals with chronic diseases who participated in the study produced 4 metaphors with the writing technique and 4 metaphors with the drawing technique. As metaphors, the themes of end of life and a new beginning, extinction and feeling of emptiness, journey of life, salvation and reunion were determined. Conclusion: As a result of the study, it was determined that old individuals with chronic diseases had anxiety about death. In addition, it was determined that old individuals had negative and positive cognitive schemas about death. In this context, old individuals with chronic diseases may need psychosocial support systems.

Effects of psychosocial interventions on sexual function, quality of life, and stress in women with gynecologic cancer: a systematic review and meta-analysis

Aydın, Esra; Özcan, Handan; Karakaş, Sevda; Uzun, Sevda

Objectives: To evaluate the effects of psychosocial interventions on sexual function, quality of life, and stress levels in women with gynecologic cancer. Methods: This systematic review and meta-analysis was conducted in accordance with the PRISMA 2020 guidelines. PubMed, Web of Science, EBSCOhost, Google Scholar, and the YÖK Thesis Center were searched between June and July 2024 without year restriction. Randomized controlled and quasi-experimental studies evaluating psychosocial interventions were included. Data were synthesized using meta-analysis and narrative synthesis. Outcomes were synthesized separately for sexual function, quality of life, and stress. Results: Seven studies were included in the analysis. Psychosocial interventions significantly improved sexual function (SMD: 0.868; 95% CI: 0.021 to 1.715; Z = 2.008; p = 0.045; I2 = 89.97%) and quality of life (SMD: 0.709; 95% CI: 0.060 to 1.358; Z = 2.142; p = 0.032; I2 = 88.61%) in women with gynecologic cancer. However, psychosocial interventions did not demonstrate a significant effect on stress levels (SMD: − 1.126; 95% CI: − 2.649 to 0.397; Z = − 1.449; p = 0.147; I2 = 95.65%). Conclusion: This systematic review and meta-analysis indicates that psychosocial interventions are associated with improvements in sexual function and quality of life among women with gynecologic cancer, whereas no statistically significant reduction in stress was observed. Although substantial between-study heterogeneity was present, the overall evidence supports the integration of psychosocial approaches into supportive oncology nursing care.

Objective: – The aim of this study was to determine the effect of jet lidocaine on pain and anxiety during peripheral venous access. Methods: – A comprehensive literature search was conducted in PubMed, Web of Science, EBSCOhost, Google Scholar, and the YÖK Thesis Center without year limitation. Five randomized controlled trials that met the inclusion criteria were included in the analysis. Data extraction and methodological quality assessment were performed using the Joanna Briggs Institute (JBI) Critical Appraisal Tools, and study selection was conducted in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) 2020 guidelines. The findings were synthesized using meta-analysis and narrative synthesis. Results: – The pooled analysis demonstrated that jet lidocaine significantly reduced anxiety related to peripheral venous access (standardized mean difference: −1.031; 95% confidence interval: −1.653 to −0.409; Z = −3.249; P =.001; I2 = 89.98%). However, its effect on pain was not statistically significant (standardized mean difference: −0.817; 95% confidence interval: −1.817 to 0.182; Z = −1.602; P =.109; I2 = 96.57%). Conclusion: – Jet lidocaine may be an effective option for reducing anxiety during peripheral venous access; however, its effect on pain was not statistically significant. Further high-quality randomized controlled trials with standardized protocols and larger sample sizes are needed to clarify its analgesic effectiveness and clinical outcomes.

Background Thirst is a sensation associated with a dry mouth and the desire or craving to drink water. Surgical patients are among those hospitalized individuals who are at high risk of developing both osmotic and hypovo-lemic thirst. Objectives To develop a new measurement tool for evaluating the thirst-related discomfort of surgical patients and to assess its validity and reliability. Methods The study population consisted of patients admitted to the surgical clinics of a research hospital in Turkey between January and March 2022. The new measurement tool was developed in several stages: a pilot implementation phase with 51 patients and the main implementation phase with 198 patients. Data were col-lected by means of face-to-face interviews with patients. Exploratory factor and confirmatory factor analyses were performed using IBM SPSS Statistics, version 22, and IBM SPSS Amos. Results The final Thirst Discomfort Scale consisted of 12 items in 3 subdimensions. The Cronbach α values of the scale subdimensions ranged from 0.787 to 0.848, and the Cronbach α value for the scale as a whole was 0.886. The scale explained 66.237% of the total variance of the data. Conclusion The Thirst Discomfort Scale consisting of 12 items and 3 subdimensions is a valid and reliable measurement tool for evaluating thirst-related discomfort of surgical patients.
