Akay, Gamze; Koç, Elif Simay; Oğuzhan, Hatice

The aim of this study is to investigate the relationship between post-traumatic stress, quality of life, and loneliness levels in adolescents following the earthquakes centered in Kahramanmaraş. The research, designed as descriptive and correlational, was conducted with 701 children aged 13–18 who experienced the earthquake in Kahramanmaraş. Data were collected through a Demographic Information Form, the Child Post-Traumatic Stress Reaction Index, the Pediatric Quality of Life Inventory, and the UCLA Loneliness Scale-Short Form. Ethical principles were adhered to throughout the study. Female adolescents were found to have higher levels of post-traumatic stress (t = 3.508, p < 0.001) and lower quality of life (t = 4.223, p < 0.001). The adolescents exhibited severe levels of post-traumatic stress (55.25 ± 13.80). It was determined that the mother’s education level (F = 4.423, p = 0.002), the father’s occupation (F = 2.859, p = 0.036), and perceived income level (F = 5.912, p = 0.003) significantly influenced adolescents’ loneliness levels. The father’s occupation (F = 3.403, p = 0.017) was found to have a significant effect on post-traumatic stress levels, and perceived income level (F = 4.753, p = 0.009) had a significant effect on quality of life. A moderate negative correlation was found between post-traumatic stress levels and quality of life (r = −0.474, p < 0.001), and a weak positive correlation was observed between post-traumatic stress and loneliness levels (r = 0.215, p < 0.001). A weak negative correlation was also found between quality of life and loneliness levels (r = −0.284, p < 0.001). A moderate negative correlation was found between adolescents’ post-traumatic stress levels and quality of life, while a weak positive correlation was observed between post-traumatic stress levels and loneliness. It is recommended to provide psychological support and guidance services aimed at reducing post-traumatic stress and loneliness levels in adolescents.

The associations between metacognition problems, childhood trauma and internalizing symptoms in healthcare workers working directly with patients infected with COVID-19

Sen Demirdogen, Ezgi; Algedik, Pinar; Warikoo, Nishchint; Bahadir, Erdi; Akbiyik, Meral; Büyüktarakçi, Seda; Kadak, Muhammed Tayyip; Yavuz, Mesut

Healthcare workers (HCWs) providing medical support while facing one of the highest levels of adverse and potentially fatal outcomes due to COVID-19 are put in a vulnerable position leading to the development of mental health problems. The development of any prevention and intervention programs to reduce this risk is possible with better understanding and knowledge of possible vulnerability factors. The aim of the present study is to investigate psychological effect of working directly with patients infected with COVID-19 (WD) and possible individual vulnerability factors for the development of psychological problems in HCWs. The data used in this cross-sectional study were collected using online self-reported questionnaires from 290 HCWs aged 21-61 years old. The mean score of the scales of 145 HCWs-WD and 145 HCWs not WD (HCWs-NWD) were compared by independent sample t test. Associations between childhood traumas, metacognitive dysfunctional beliefs and internalizing symptoms were analyzed using structural equation modelling (SEM). The depression, anxiety, stress symptoms levels, somatization and sleep problem levels were found to be higher in HCWs-WD compared to HCWs-NWD. SEM revealed that childhood trauma levels was associated with the increased risk of internalizing problems, and metacognitive dysfunctional beliefs had a partial mediator role between childhood traumas and internalizing symptoms in HCWs-WD. Improving metacognitive abilities may hence need to be considered in prevention programs for the HCWs. The findings can also be used to set up further research on the specific interventions on the HCWs who are at a risk as their profession entails them being in such traumatic situations.

Evaluation of acute kidney injury (AKI) with RIFLE, AKIN, CK, and KDIGO in critically ill trauma patients

    Ülger, F.; Küçuk, M. Pehlivanlar; Kucuk, A. O.; Ilkaya, N. K.; Murat, N.; Bilgiç, B.; Abanoz, H.

The aim of our study was to evaluate the effects of AKI development on mortality with four different classification systems (RIFLE, AKIN, CK, KDIGO) in critically ill trauma patients followed in the intensive care unit. A retrospective review of 2034 patients in our intensive care unit was conducted between July 2010 and August 2013. A total of 198 patients with primary trauma were included in the study to evaluate the development of AKI. When the presence of AKI was investigated according to the four criteria (RIFLE, AKIN, CK, and KDIGO), the highest incidence of AKI was found according to the KDIGO classification (74.2%), followed by AKIN (72.2%), RIFLE (69.7%), and CK (59.1%). It was observed that more AKI developed according to KDIGO in patients with multiple trauma and thoracic trauma (p = 0.031, p = 0.029). Sixty-two (31%) of the 198 trauma patients monitored in the intensive care unit died; mortality was frequently found high in AKI stage 2 and 3 patients. According to the CK classification, there was a significant increase in mortality in patients with AKI on the first day (p = 0.045). AKI classifications by RIFLE, AKIN, CK, and KDIGO were independently associated with the risk of in-hospital death. In this study, the presence of AKI was found to be an independent risk factor in the development of in-hospital mortality according to all classification systems (RIFLE, AKIN, CK, and KDIGO) in critically traumatic patients followed in ICU, and the compatibility between RIFLE, AKIN, and KDIGO was the highest among the classification systems.

Oguz, Ahmet Burak; Polat, Onur; Eneyli, Muge Gunalp; Gulunay, Behnan; Eksioglu, Merve; Gurler, Serdar

Study objective: Our aim was to determine the efficiency of ultrasound (US) scanning in patients with wrist trauma admitted to the emergency department and to compare US diagnostic usagewith other radiological imagingmethods. Methods: Patients who presented to the emergency department with wrist injury and who met the inclusion criteria and exclusion criteria were eligible. For all patients, US evaluation of thewhole wristwas performed by an emergency physician before other radiological imagingmethods (radiographies, computed tomography (CT) and magnetic resonance (MR) imaging). All of the patients included in the study underwent US, radiography, CT, and MR. Results: During the study, 122 patients were admitted with a wrist injury. After filtering for the exclusion criteria, 80 patients were included in the study. The sensitivity of US scanning in detecting fractures was 95.31% (95% confidence interval [CI]: 87.1-98.39), the specificity was 93.75% (95% CI: 71.67-98.89), and the positive predictive value was 98.39% (95% CI: 91.72-99.85), and the negative predictive value was 83.33% (95% CI: 72.98-90.41). The sensitivity of US scanning in detecting tendon and ligamentous structural injury was 66.67% (95% CI: 41.71-84.82), the specificity was 100% (95% CI: 94.42-100), the positive predictive valuewas 100% (95% CI: 94.29-99.89), and the negative predictive was 92.86% (95% CI: 84.25-97.14). Conclusion: US scanning is an effective method that can be applied in the emergency department to adult patients to diagnose distal forearm and carpal bones fractures. In soft tissue injuries, US and MR examinations produce similar results. (C) 2017 Elsevier Inc. All rights reserved.

Traumatic diaphragmatic ruptures: diagnostic and therapeutic approaches

Çobanoglu, Ufuk; Kara, Volkan; Yalçinkaya, Irfan; Er, Metin; Isik, Ahmet Feridun; Sayir, Fuat; Mergan, Duygu

Background: This study aims to evaluate the characteristics of patients with traumatic rupture of diaphragm (TDR), the diagnostic and therapeutic approaches used in these cases and and the outcomes of the patients. Methods: Forty-one patients (31 males, 10 females; mean age 41.9 years; range 7 to 71 years) with thoracoabdominal trauma and diagnosis of TDR who were treated in our clinic between January 2000 and January 2010 were analyzed retrospectively. The patients were statistically compared in terms of age, gender, the time of diagnosis (early or late), the diagnostic procedures that were performed, whether the diagnosis of rupture was made during or after the operation, the localization of the rupture, the diameter of the rupture, accompanying injuries, the surgery performed, mortality and morbidity and the trauma injury severity score. Results: The etiology for the diaphragmatic rupture was blunt trauma in 43.9% and penetrating trauma in 56.10% of the patients. 85.36% of the patients were diagnosed in early stage and 14.64% in late stage. Herniation dignosis was made radiologically in 118 (43.9%) patients and diaphragmatic rupture was detected during the operation in 23 patients (56.1%). Twenty-nine (70.73%) patients had thoracotomy, seven (17.02%) patients had laparatomy and five (12.19%) patients had thoracotomy plus laparatomy. The overall mortality rate was 14.63% (n=6). Conclusion: Traumatic diaphragmatic ruptures may have a fatal course depending on the strangulation of the herniating abdominal viscera. Traumatic diaphgram rupture should be suspected in all multitraumatic patients and these cases shuld be seriously evaluated for the definitive diagnosis and treatment plan.

Isolated Hemothorax Following Thoracic Trauma: Analysis of 57 Cases

Çobanoglu, Ufuk; Melek, Mehmet; Kara, Volkan; Mergan, Duygu

Aim: The aim of this study was to review cases with isolated hemothorax after thoracic trauma, to assess the diagnostic and treatment methods, and to discuss the determining factors of morbidity and mortality. Material and Method: A total of 57 patients were examined retrospectively. All patients underwent tube thoracostomy and underwater seal as the initial treatment approach. Emergency thoracotomy was performed on cases with continuing drainage from the chest tube, expanding hemothorax on the posteroanterior (PA) chest x-ray, and with hemodynamic instability. Result: Hemothorax occurred as a result of penetrating trauma in 31 (54.38%) and due to blunt trauma in 26 (45.61%) cases. Of the cases, 49 (85.96%) underwent tube thoracostomy drainage. All the 8 cases (14.03%) that underwent emergency thoracotomy had penetrating trauma. Mortality occurred in one patient (1.75%) who had penetrating trauma and who underwent emergency thoracotomy. The morbidity rate in patients with blunt trauma was significantly higher than those with penetrating trauma (p<0.0001). Discussion: Accurate diagnosis and appropriate surgical intervention in cases with traumatic hemothorax is essential for reducing the morbidity and mortality. Chest tube insertion and underwater seal application should be the initial treatment modality and successful in most cases. Emergency thoracotomy is life-saving in indicated patients. The need for thoracotomy is higher in isolated hemothorax due to penetrating chest trauma.
