Surrogate Trauma in Nurses Providing Pediatric Palliative Care: A Phenomenological Study

Oğuzhan, Hatice; Akay, Gamze; Uzun, Sevda; Koç, Elif Simay; Öncer, Aysun

The aim of this study was to deeply examine the experiences of proxy grief among nurses providing pediatric palliative care. This qualitative phenomenological study was conducted with 16 nurses working in the pediatric ward of a hospital located in the northern region of the country. Data were analyzed using Colaizzi’s phenomenological analysis method. The analysis revealed 3 themes (challenges experienced while caring for children in palliative care; secondary trauma and coping processes; proxy trauma and professional resilience) and 9 sub-themes (emotional strain during care, establishing an empathetic bond, emotional exhaustion, traumatic experiences, seeking psychological support, religious coping methods, developing a professional approach, resilience and acceptance, and motivation to continue in the profession). The study found that nurses providing pediatric palliative care experienced significant psychosocial stress. Pediatric nurses were particularly challenged emotionally when caring for palliative patients. In this context, it is important to develop stress-coping skills to help nurses manage vicarious trauma. Strengthening nurses psychologically and spiritually will positively impact their professional lives, which in turn will enhance the quality of care they provide.

Psychometric evaluation and cultural adaptation of the Turkish version of the needs assessment tool: progressive disease-heart failure (NAT: PD-HF)

Kiliç, Betülay; Palaz, Ilknur; Gürel, Tugba Yardimci

Heart failure (HF) is a progressive, life-limiting condition that often leads to complex physical, emotional, and social needs. This study aimed to translate, culturally adapt, and validate the Turkish version of the Needs Assessment Tool: Progressive Disease-Heart Failure (NAT: PD-HF) to facilitate the timely identification of palliative care needs. A total of 98 patients with chronic HF, 91 caregivers, and 25 healthcare professionals participated in this single-centre validation study. The tool underwent forward-backward translation and pilot testing. Internal consistency was good (Cronbach’s α = 0.812), and inter-rater and test-retest reliabilities were high (PABAK: 0.908–1.000). Most patients (84.7%) found the tool understandable, and 67.7% felt it enhanced care. Healthcare professionals from cardiology, intensive care, and palliative care rated it as clinically relevant and feasible. The Turkish NAT: PD-HF is a valid, reliable, and culturally appropriate tool for identifying the palliative care needs of patients with HF and their caregivers. This tool can be integrated into routine clinical practice to facilitate early identification of palliative care needs.

The impact of surgical intervention on religious coping, psychological well-being, and pain levels in older adult patients: A quasi-experimental study

    Daştan, Buket; Kulakaç, Nurşen

Aim: This study aimed to determine the impact of surgical intervention on religious coping, psychological well-being, and pain levels in older adult patients. Method: The data of the study, which used a one-group pretest-posttest quasi-experimental design without a control group, was collected between July 1, 2023, - January 30, 2024. Data collection involved the use of a Personal Information Form, the Religious Coping Scale, the Psychological Well-Being Scale, and the Visual Analog Scale. Results: The results revealed a significant increase in psychological well-being and positive religious coping after surgical intervention compared to presurgical levels (p < 0.001). Multiple linear regression analysis revealed that male gender, younger age, enhanced psychological well-being, and positive religious coping were identified as factors contributing to a reduction in postsurgical pain levels. Conclusion: The study's results indicate that surgical intervention serves as a significant variable influencing psychological well-being and religious coping among older adult patients.

Ulutaşdemİr, Nilgün; Ay, Halil; Göçmen, Adalet; Uzun, Sevda; Kulakaç, Nurşen

Objective: This article aimed to investigate the needs of caregivers of patients with palliative neurological problems and contribute to the solution proposals. Material-method: Phenomenological research design, a qualitative research design, was used in the study. The article was conducted with 16 relatives of patients with palliative neurological problems. The interviews were conducted using face-to-face in-depth semi-structured interview form. Colaizzi’s phenomenological analysis method used for the analyse. Results: In the analysis of the data, three categories, eight main themes, and seventeen sub-themes emerged. As a result of the interviews with the participants, the category of the effect of neurological palliative disease on the quality of life of individuals was gathered under three main themes, namely mental, physical, and social effects, and in the process of coping with the disease requiring neurological palliative care, two main themes were identified as emotion-focused coping and problem-focused coping. Conclusion: It was concluded in our study that having a patient in palliative care has psychological, social, and physical effects on the relatives of the patients. The burden of caregiving mostly affects the relatives of patients negatively. Relatives of patients mostly need time for themselves, moral support, and companion support.
