Semantic Architecture and Knowledge Evolution of Endoscopic Submucosal Dissection Research: A Computational Bibliometric Analysis

Demir, Huseyin; Akar, Tuncay; Mertoglu, Suleyman; Tekinarslan, Merve

Aim: The overarching intellectual structure, dominant thematic clusters, and longitudinal evolution of the endoscopic submucosal dissection (ESD) literature have yet to be systematically mapped. This study aims to characterize the semantic and thematic architecture of ESD research through computational text analytics and to reveal latent structural patterns and temporal dynamics within this domain. Methods: A corpus of 1,000 peer-reviewed publications was processed through an eight-stage computational pipeline that incorporated word embedding-based semantic analysis, unsupervised clustering, Latent Dirichlet Allocation (LDA) topic modeling, inter-topic network analysis, and temporal trend modeling. The primary outcome was characterization of the semantic architecture of ESD research, whereas secondary outcomes included topic relationships, temporal evolution, and model performance metrics. Results: Semantic analysis revealed two dominant clusters: a technically oriented domain encompassing device innovation and procedural optimization and a clinically oriented domain addressing therapeutic outcomes, organ-specific protocols, and adverse event management. Topic modeling identified nine coherent thematic categories, with limited integration between technical and clinical research trajectories. Temporal analysis revealed a pronounced post-2024 shift toward upper-gastrointestinal applications, particularly esophageal and gastric ESD, with relative stagnation in colorectal investigations. The optimal semantic clustering solution achieved a silhouette score of 0.215, while the nine-topic LDA model demonstrated a coherence score of 0.39. Conclusion: This study provides the first data-driven macro-structural characterization of the ESD literature, revealing a persistent bifurcation between technical innovation and clinical application streams. These findings offer a strategic framework to help clinicians, investigators, and curriculum developers identify priority areas for advancement in therapeutic endoscopy.

Identification of Problems Faced by Individuals with Epilepsy in Health Communication

Bingöl, Nuray; Aktepe Coşar, Dilan; İşcan Ayyıldız, Neşe

Objective: Individuals with epilepsy often face challenges with medication adherence and health monitoring because communication difficulties that hinder self-expression and information exchange, thereby complicating effective disease management. This study aimed to examine the health communication problems experienced by individuals with epilepsy. Methods: This descriptive, cross-sectional study was conducted among 172 individuals with epilepsy who presented to the neurology outpatient clinic in eastern and northern Türkiye between August 2023 and August 2024. Study data were collected using the descriptive data form and the health communication problems scale. The independent groups t-test, one-way ANOVA, and Tukey test were used to evaluate the data. Results: On the health communication problems scale, individuals with epilepsy had mean scores of 2.41 ± 0.53 for the effective communication problems sub-dimension, 2.88 ± 0.80 for the social communication problems sub-dimension, and 2.68 ± 0.72 for the communication barriers sub-dimension. No significant differences were detected in the mean scores for health communication problems among individuals with epilepsy by sex, health expenditure, seizure type, or seizure characteristics (p > 0.05). Conclusion: The study revealed that individuals with epilepsy experienced moderate difficulty with health communication. Those who were married, had higher educational attainment, were unemployed, had lower income levels, and were receiving polytherapy experienced greater communication challenges with healthcare professionals.

Investigation of the Effect of Risk Perception, Socio-demographic and Obstetric Factors on the Level of Decision-making of Pregnant Women via the Internet: Multiple Linear Regression Analysis Model; [Gebelerin İnternet Yoluyla Karar Alma Düzeyi Üzerine Risk Algısının, Sosyo-demografik ve Obstetrik Faktörlerin Etkisinin İncelenmesi: Çoklu Doğrusal Regresyon Analiz Modeli]

Aktaş, Songül; Topçu, Tuğba Yazıcı; Ay, Özge Palancı; Aydın, Ruveyde

Objective: This study examines the effect of risk perception and socio-demographic and obstetric factors on the level of decision-making of pregnant women via the internet. Method: This study employed a cross-sectional and analytical design and was conducted online with 384 pregnant women living in Turkey. Data were collected using descriptive information forms, such as the decision-making scale via the internet on pregnancy and the perception of pregnancy risk scale. Descriptive statistics, including percentages and means, as well as multiple linear regression analysis, were utilized to analyze the data. Results: As the risk perception in pregnancy increased, the level of decision-making via the internet increased (β=0.118, p=0.000). Among pregnant women experiencing pregnancy-related health issues, a significant increase in decision-making via the internet was observed (β=0.092, p=0.046). As the age of women increased (β=-2.623, p=0.013) and income was perceived to be equal to expenses (β=-1.499, p=0.011) or more than expenses (β=-1.953, p=0.023), decision-making via the internet during pregnancy decreased. Unwanted pregnancy has a “reducing” effect of approximately two times on online decision-making (β=-1.919, p=0.026). The number of pregnancies, education and family type were found to have no statistical effect on decision-making (p>0.05). Conclusion: As the risk perception increases in pregnant women, online decision-making also increases. Factors such as some socio-demographic and obstetric factors affect online decision-making of pregnant women.

A Dimension of Child Emergency: Psychiatric Emergency and Nursing Approach; [Çocuk Acilin Bir Boyutu: Psikiyatrik Aciller ve Hemşirelik Yaklaşımı]

    Aksoy, Bahar; Kahriman, İlknur

In recent years, there has been a significant increase in applications to pediatric emergency services. Psychiatric emergencies are one of the frequent applications made to pediatric emergency services. Psychiatric emergencies are the first places that children and families will go to, as a situation that applications urgent intervention. However, in cases that do not require emergency intervention, children and families should apply to psychiatry services, while most of the applications are made to emergency services. Psychiatric emergencies constitute 3-12% of the applications made to emergency services. Therefore, in this review, psychiatric emergencies, risk factors, incidence, identification and evaluation of psychiatric emergencies, specific psychiatric emergencies and nursing approach are discussed in this review.
