Comparison of automated subcortical segmentation methods: VolBrain vs. 3D Slicer in brain volumetry

Acer, Niyazi; Palanci, Ozgur; Ipekten, Funda; Erdogan, Furkan Mert; Yarar, Burhan; Ozyasar, Ali Faruk

Objective:Accurate volume measurements of subcortical brain structures are essential in neuroimaging studies. This study aimed to compare two automated segmentation algorithms, 3D Slicer and VolBrain, for subcortical brain volumetry. Method:Subcortical volume measurements obtained using 3D Slicer and VolBrain were compared using the Neurofeedback Skull-stripped (NFBS) dataset. Statistical analyses included the coefficient of determination (R2) and Bland-Altman (B-A) plots. Volumetric measurements from both tools were evaluated across multiple subcortical brain regions. Results:Strong agreement was observed for the caudate nucleus, thalamus, and hippocampus, while notable discrepancies were identified in the amygdala, globus pallidus, and nucleus accumbens. Both constant and proportional systematic errors were detected in certain regions, with greater variability observed in 3D Slicer measurements. Intraclass Correlation Coefficient (ICC) analysis demonstrated weak agreement for the accumbens and amygdala, and moderate agreement for the hippocampus and thalamus, highlighting region-specific differences in performance Conclusion:Both VolBrain and 3D Slicer are effective tools for subcortical brain volumetry. VolBrain provides a fully automated workflow, whereas 3D Slicer allows greater flexibility through detailed corrections. Method selection should be guided by research objectives, regions of interest, and image quality. Further validation across diverse datasets is warranted to improve clinical applicability, particularly in regions showing lower agreement.

Rationally decreasing the number of drugs seems to be a useful therapeutic approach in older adults: 6-month follow-up study

Idil, Eral; Aydin, Ali Ekrem; Bulut, Esra Ates; Isik, Ahmet Turan

Aims: Older adults are at risk for polypharmacy, which has multidimensional safety, clinical and economic implications. Therefore, the optimization of drug therapy is one of the critical components of geriatrics clinical practice. This study is aimed to investigate the effect of drug rationalization on comprehensive geriatric assessment(CGA) parameters. Materials and Methods: The study was a retrospective and longitudinal study examining the effect of change in drug number on functionality and physical performance. A total of 515 patients were included in the study. Detailed medication history, laboratory findings, CGA parameters were recorded in the first admission. Polypharmacy was accepted as concurrent usage of five or more drugs. Medications of each patient were reviewed with the guidance of CGA and Beers Criteria. Results: The mean age of patients was 74.13 +/- 7.29 years, and 68.7% were female. The baseline mean number of drugs was 5.11 +/- 3.34. The polypharmacy group(269 patients) had a higher rate of geriatric syndromes and lower CGA scores in the first admission. After optimization of medications, the mean drug number decreased to 4.76 +/- 2.72. Depression, mobility and nutritional scores improved at the end of six months in the patients whose total number of drugs decreased, while global cognition, activities of daily living scores, and gait speed were preserved. Increasing the number of drugs was associated with lower mobility and functionality. Conclusion: Drug rationalization guided with CGA improves the nutritional, physical, and psychosocial status of geriatric patients. Thus, medication reviews have key importance in the management of older patients.

Regulators of mitochondrial complex I activity: A review of literature and evaluation in postmortem prefrontal cortex from patients with bipolar disorder

Duong, Angela; Che, Yi; Ceylan, Deniz; Pinguelo, Arsene; Andreazza, Ana C.; Young, L. Trevor; Berk, Michael

Phenomenologically, bipolar disorder (BD) is characterized by biphasic increases and decreases in energy. As this is a state-related phenomenon, identifying regulators responsible for this phasic dysregulation has the potential to uncover key elements in the pathophysiology of BD. Given the evidence suggesting mitochondrial complex I dysfunction in BD, we aimed to identify the main regulators of complex I in BD by reviewing the literature and using the published microarray data to examine their gene expression profiles. We also validated protein expression levels of the main complex I regulators by immunohistochemistry. Upon reviewing the literature, we found PARK-7, STAT-3, SIRT-3 and IMP-2 play an important role in regulating complex I activity. Published microarray studies however revealed no significant direction of regulation of STAT-3, SIRT-3, and IMP-2, but a trend towards downregulation of PARK 7 was observed in BD. Immunocontent of DJ-1 (PARK-7-encoded protein) were not elevated in post mortem prefrontal cortex from patients with BD. We also found a trend towards upregulation of DJ-1 expression with age. Our results suggest that DJ-1 is not significantly altered in BD subjects, however further studies are needed to examine DJ-1 expression levels in a cohort of older patients with BD.
