The Effect of Audio-Visual Methods on Preoperative Anxiety in Children: A Systematic Review and Meta-Analysis of Randomized Controlled Trials

    Kulakaç, Nurşen; Ustuner Top, Fadime

Background: Children presenting to hospitals during the preoperative period are often exposed to procedures that cause significant stress. Through preoperative interventions, nurses play an important role in relieving the anxiety of children and their parents. Objective: This study was conducted to determine the effect of audio-visual methods on preoperative anxiety levels in children. Methods: PubMed (including MEDLINE), Cochrane, EBSCOHost, Web of Science, YÖK National Thesis Center, and Google Scholar databases of randomized controlled studies published in the last 10 years were searched for this meta-analysis. 32 studies were included in the meta-analysis, and the total sample size was 2795. Results: Q According to the results of the meta-analysis using the random-effects model, audio-visual methods had a positive effect on reducing anxiety (SMD: -1.312, %95 CI -1,666-(0,958), Z=-7,260, p < 0,001, Q-value = 556,572 I2 = 94.519). The moderator analysis demonstrated that cartoons (SMD:-1.645, p < 0.001), games (SMD:-1.931, p = 0.001), music (SMD:-0.534, p < 0.001), video showings (SMD:-1.363, p = 0.001), and virtual reality (SMD:-1.782, p < 0.001) reduced preoperative anxiety in children. Conclusions: Audio-visual methods are effective in reducing preoperative anxiety in children. The results of this study may provide pediatric perioperative nurses with additional information to help them decrease anxiety for patients and their families.

Prevalence and Determinants of Workplace Violence Against Pediatric Emergency Healthcare Workers and Its Effect on Their Psychological Resilience

Ustuner Top, Fadime; Kulakaç, Nurşen; Cam, Hasan Hüseyin

The study aimed to determine the prevalence and risk factors of workplace violence (WPV) against pediatric emergency healthcare workers and evaluate the relationship between WPV and psychological resilience. This study is cross-sectional and correlational. According to the results, the prevalence of WPV was 69.8%, and its distribution was as follows: verbal abuse (56.9%), bullying/mobbing (37.6%), physical abuse (12.8%), and sexual abuse (2.2%). Being single/separated/divorced/widowed (odds ratio [OR]: 1.85, 95% confidence interval [CI]: 1.03-3.30), being a physician (OR: 4.74, 95% CI: 1.73-12.96), being a staff member (OR: 2.57, 95% CI: 1.10-5.99), routine direct physical contact with patients/clients (OR: 2.77, 95% CI: 1.40-5.48), and lack of encouragement to report WPV (OR: 3.76, 95% CI: 2.01-7.01) were independent predictors of WPV (P <.05), and WPV was found to be associated with low psychological resilience. Arrangements related to violence prevention, preparation, and intervention should be made and maintained in all pediatric emergency departments.

Experiences of Turkish mothers of children with autism: a phenomenological study

Aktaş, Eda; Ustuner Top, Fadime; Uzun, Sevda

The study aimed to explore mothers’ experiences of children with autism using a phenomenological approach. Semi-structured, in-depth interviews were conducted with 18 parents with children attending a special education centre in a northern province of Turkey. Criterion sampling, a form of purposive sampling, was employed to select participants. Interviews continued until data saturation was achieved. All interviews were audio-recorded and transcribed verbatim. Thematic analysis was utilized to analyze the data, and the study adhered to the COREQ checklist for reporting qualitative research. As a result of analyses, three main themes (the process of diagnosis, the effect of the child’s illness on parents and their challenges over time, and considerations regarding the child’s educational journey) and eight sub-themes (feelings and thoughts, reflections on the causes, reactions encountered, emotional effects, social effects, physical effects, perception of social support, and expectations from the educator and the education process) emerged. The study concluded that mothers of children with autism are emotionally, physically, and socially impacted by their child’s condition, facing considerable challenges in coping. In light of these findings, it is recommended that policymakers in our country develop plans that address the difficulties expressed by participants, review and potentially enhance financial and social support for families, and consider increasing special education hours.

Perspectives on the mental health of immigrant children in the pediatric emergency department: a phenomenological study

    Uzun, Sevda; Ustuner Top, Fadime

The concept of immigration is a complex phenomenon that goes far beyond mere relocation and refers to the transition from one social environment to another or from one culture to another. Immigration and its effects on children are regarded as global and interdisciplinary problems. This study aims to reveal the opinions of nurses working in the pediatric emergency department about the mental health of immigrant children. In this qualitative research, a phenomenological research design was used (N = 19).Interviews were conducted using a semi-structured interview form. Colaizzi’s method was used to analyze the data manually. Four categories, eleven themes, and thirty-eight sub-themes emerged from the analysis of the data. The difficulties nurses experience with immigrant children and their relatives in providing care include communication problems, cultural problems, and economic problems. The causes of mental health problems in immigrant children include war, exposure to immigration traumas, cultural assimilation problems, loss/absence of parents, economic reasons/excessive role overload, and exposure to neglect and/or abuse. Education, economic aid, communication, non-stigmatization, and social support were identified as significant sub-themes for practices to protect/promote children’s mental health. The study showed that immigrant children experience physical and mental problems. Nurses struggle to treat immigrant children effectively but have difficulties due to insufficient facilities. It may be suggested to include cultural care models in patient care.
