OBJECTIVES: It is crucial to promptly identify high-mortality patients in emergency departments and initiate their treatment as soon as possible. Although many parameters have been studied to select patients with high mortality, no comprehensive evaluation exists in previous literature on these parameters in critically ill patients, regardless of patient groups. The aim of this study is to evaluate the Glasgow Coma Scale (GCS), quick Sepsis-related Organ Failure Assessment (qSOFA), blood gas base excess (BE), and blood gas lactate in predicting mortality in critically ill patients admitted to the emergency department. METHODS: This prospective observational cohort study included adult patients with Emergency Severity Index 1-2 (critically ill) admitted to the emergency department. All patients were evaluated by the physician within 10 min, and blood gas samples were taken. The data collection forms recorded the patients' GCS and qSOFA scores at the time of first evaluation by the physician. The qSOFA score assessment was performed in all patients with ESI levels 1 and 2, regardless of whether infective pathology was suspected. Blood gas BE and lactate values were also from laboratory test results. Patients or their relatives were contacted by phone at the end of the 1st month to obtain information about the clinical condition (survival or mortality). RESULTS: A total of 868 patients were included, with 163 deaths observed within 30 days. GCS score, qSOFA score, and lactate value were significant in predicting mortality within 30 days. While the BE value was significant for predicting 30-day mortality for values equal to or below the lower limit of -1.5 (P < 0.001), it was not significant for values equal to or above the upper limit of +3 (P > 0.05). The most successful prediction model for predicting 30-day mortality was found to be qSOFA with a cutoff value of >= 1. CONCLUSION: In emergency departments, each of the GCS, qSOFA scores, BE, and lactate values can be used independently as a practical mortality prediction model in critically ill patients. Among these four models, qSOFA is the most successful practical mortality prediction model in critically ill patients.

Introduction: Anthrax is a zoonosis affecting herbivorous animals. Its agent is Bacillus anthracis and it is gram positive, aerobic or facultative anaerobic, immobile and has a capsule in the polypeptide structure. The incidence is gradually decreasing in the world and in our country. The disease is especially seen in areas where stock breeding is common. In this case series, 3 cases, one of them was mortal, took place after anthrax infected cow was eaten in February 2018 in Trabzon was shared. Case presentations: In the first case, as a result of the animal's meat was eaten, gastrointestinal anthrax and associated sepsis were seen. This patients blood culture was positive and she was died. Second case after contacting the patient who died, and third case after contacting the animal and eating its meat, diagnosed with cutaneous anthrax. Doxycycline treatment was started with cutaneous anthrax patients, these cases were recovered without any complications. Conclusion: We presented this article, to update our information about gastrointestinal anthrax and other anthrax types which are become a current issue again especially due to bioterrorism and also to keep it in mind in the differential diagnosis although it is rarely seen in our stock raising areas.

Objectives The behaviors of individuals during disasters are determined by their level of disaster readiness and their knowledge of disaster threats. The aim of this research was to determine the basic disaster consciousness levels of individuals between the ages of 18 and 23. Methods The study included 291 students from Kelkit Aydin Dogan vocational school at the University of Gumushane. Each student completed a questionnaire with 10 informative questions about disaster consciousness. The data were evaluated using the chi-square test and the t-test. Results Not all 291 students answered each question. Of 285 students, 74.0% had not received any education on disaster preparedness. Of 249 students, 85.9% did not have a disaster preparedness plan. Of 266 students, 88.7% did not have an emergency kit. Out of 10 possible points, participants received 3.00 +/- 1.58 points on average for disaster preparedness. Conclusions The point average for student participants was quite low. Previous disaster training significantly increased the number of points received by participants. Disaster training should be extended to a wider range of individuals.

Objectives The purpose of this retrospective study was to compare the mean platelet volume between the patients with acute deep vein thrombosis (DVT) and control subjects. Methods Data were collected retrospectively from the patients' medical records. The study group consisted of 110 patients with newly diagnosed acute DVT in the lower extremities. An age, gender, and body mass index-matched control group consisted of 98 patients with normal duplex ultrasound findings. We compared the mean platelet volume in patients with acute DVT and control participants. Results There was no significant difference between the study and control groups in mean platelet volume (8.8 +/- 1.6 vs 8.7 +/- 1.3 fl, respectively; p>0.05). Age (p=0.899), body mass index (p=0.43), gender (p=0.466), and platelet count (p=0.886) were statistically similar in both groups. Conclusions There was no significant difference in mean platelet volume between patients with acute DVT and the control group. According to our result, we propose that mean platelet volume should not be considered as an index of acute DVT.
- MakaleAçık Erişim2011A Different Cause of Trauma: Injury by a Mole Gun
Türkmen, Suha; Aktas, Aydin; Karaca, Yunus; Türedi, Suleyman; Gündüz, Abdulkadir
Although injuries caused by the home-made weapons such as mole guns, which are intended to fight off moles, are rare in Turkey they do still happen. They generally involve minor accidents and cause simple injuries to the hand. In this case we present a patient who involves a trauma to the left hand with fragmental fracture of the first metacarpal caused while checking whether a mole gun had fired or not. This injury is an example of an accident related to primitive, home- made weapons, used particularly in rural areas.
