- MakaleAçık Erişim2025Cardiovascular Findings and Effects of Caffeine on Experimental Hypothyroidism
Yuksel, Duygu; Ozmen, Ozlem
Background: Thyroid hormone deficiencies can disrupt organ functions, significantly impacting the cardiovascular system. Recently, the effects of thyroid hormones on the heart have garnered increased attention. However, most studies are conducted on humans using clinical data, while cellular-level and experimental studies remain limited. Objective: This study aimed to investigate the cardiovascular implications of hypothyroidism and evaluate the impact of caffeine on cardiac health in rats induced with hypothyroidism using propylthiouracil (PTU). Methods: The study involved 60 rats divided into six groups. Group 1 served as the untreated control. Group 2 received PTU for two months to induce hypothyroidism. Group 3 received PTU for one month, followed by caffeine for one month. Group 4 received caffeine for two months. Group 5 received both PTU and caffeine simultaneously for two months. Group 6 received PTU for one month, followed by one month under normal conditions. Results: During necropsy, normal thyroid glands were observed in Groups 1, 4, and 6, enlarged thyroids in Group 2, and smaller thyroids in Groups 3 and 5. Microscopic examination revealed varying thyroid histologies: Group 2 showed significant epithelial cell proliferation and absent colloid, while Groups 3, 5, and 6 had altered yet colloid-containing acini. Macroscopic inspection of hearts appeared normal across all groups. However, histopathological examination revealed significant hyperemia and microhemorrhages in Group 2, contrasting with normal findings in other groups. Immunohistochemical analysis indicated reduced cardiac troponin expression in Group 2, while other groups maintained prominent expression. Additionally, Group 2 displayed increased serum TSH levels and decreased T3 and T4 levels. Conclusion: The findings suggest that administering caffeine alongside or after PTU treatment in rats with experimentally induced hypothyroidism may ameliorate thyroid and cardiac irregularities. This study indicates caffeine's potential in mitigating the adverse effects of hypothyroidism on thyroid and heart health.

Introduction: The number of people infected with human immunodeficiency virus (HIV) in our country is increasing day by day. In this study, epidemiological, clinical, laboratory features, treatment regimens and side effects of HIV/AIDS cases followed in our clinic were evaluated. Materials and Methods: Fifty-four patients who were followed up with HIV infection in our clinic between 2000-2018 were examined retrospectively. Results: Of the 54 patients, 18 (33.3%) were females and 36 (66.7%) were males. Mean age was 42.0 +/- 15.2 (18-70) years. Mean duration of the disease was median 3 (0.5-28) and the duration of clinical follow-up was 3.3 +/- 2.7 (0.5-10) years. A total of 44 (88%) patients had risky sexual contact, and nine women were infected by her husband. The first complaint of the patients for admission to our clinic was weakness (22.2%) and fever (9.3%). Patients were diagnosed most frequently during routine examinations (48%). Mean CD4 count of the patients at the time of admission was 317.2 +/- 247.7 (7-918) cell/mm(3), and HIV-RNA was median 61000 (0-30000000) copy/mL. The most frequent treatment was tenofovir disoproxil fumarate-emtricitabine and lopinavir/ritonavir (TDF-FTC and LPV/r) in 24.4% of the patients. Treatment was changed in 14 (25.9%) patients. In five patients, noncompliance to treatment and in five patients, side effects were encountered, and treatment was changed. Hyperlipidemia was the most common side effect (52.9%) in patients receiving treatment. Prophylaxis was given to 50% of the patients in terms of opportunistic infections at the beginning of treatment. One patient had gonococcal urethritis and one patient had HPV infection in the cervix. Four patients discontinued treatment and 46 patients continued treatment. One female patient was exitus. Conclusion: The number of HIV/AIDS patients, a chronic disease, is increasing also in our country. Early diagnosis of these patients should be made, and the disease should be controlled by providing access to treatment and suppression of viral load.

Basaran, Y. K.; Gürel, M. S.; Erdemir, A. T.; Turan, E.; Yurt, N.; Bagci, I. S.
Background/objectiveReflectance confocal microscopy (RCM) is a noninvasive, objective imaging technique that provides in vivo, high-resolution skin imaging. We sought to assess epidermal and dermal changes associated with the psoriasis and its treatment with RCM before the treatment and at weeks 4 and 8 of the treatment. Material and methodsThis is an investigator-blinded, internal-controlled, follow-up study. A total of 25 patients with plaque psoriasis were included in the study. The RCM evaluation criteria were defined on the basis of the histopathological diagnostic criteria for psoriasis. The clinical severity of the psoriasis was evaluated using the Psoriasis Area Severity Index (PASI). ResultsThe RCM findings which were correlated with the PASI can be used to follow up the patient's response to treatment have been identified as follows: the acanthosis, the number of spongiotic sites, the number of regular/irregular honeycomb-like sites, the number of epidermal inflammatory cells, the number of focal microabscesses, the total epidermal thickness, the number of nonedge dermal papillae, the length of the papillary dermis, the number of dermal inflammatory cells, and the vascularization in the papillary dermis (P<0.05). ConclusionThis is the first study with a large group of patients to perform a noninvasive assessment with RCM of the response of psoriasis to different treatments: phototherapy, systemic and topical treatment. Micrometric and morphometric changes occurring in the psoriatic tissue during the 8-week treatment period were identified by in vivo RCM in a noninvasive manner. RCM is capable of monitoring of treatment response in psoriasis.

Background: This study aims to evaluate the characteristics of patients with traumatic rupture of diaphragm (TDR), the diagnostic and therapeutic approaches used in these cases and and the outcomes of the patients. Methods: Forty-one patients (31 males, 10 females; mean age 41.9 years; range 7 to 71 years) with thoracoabdominal trauma and diagnosis of TDR who were treated in our clinic between January 2000 and January 2010 were analyzed retrospectively. The patients were statistically compared in terms of age, gender, the time of diagnosis (early or late), the diagnostic procedures that were performed, whether the diagnosis of rupture was made during or after the operation, the localization of the rupture, the diameter of the rupture, accompanying injuries, the surgery performed, mortality and morbidity and the trauma injury severity score. Results: The etiology for the diaphragmatic rupture was blunt trauma in 43.9% and penetrating trauma in 56.10% of the patients. 85.36% of the patients were diagnosed in early stage and 14.64% in late stage. Herniation dignosis was made radiologically in 118 (43.9%) patients and diaphragmatic rupture was detected during the operation in 23 patients (56.1%). Twenty-nine (70.73%) patients had thoracotomy, seven (17.02%) patients had laparatomy and five (12.19%) patients had thoracotomy plus laparatomy. The overall mortality rate was 14.63% (n=6). Conclusion: Traumatic diaphragmatic ruptures may have a fatal course depending on the strangulation of the herniating abdominal viscera. Traumatic diaphgram rupture should be suspected in all multitraumatic patients and these cases shuld be seriously evaluated for the definitive diagnosis and treatment plan.

Çobanoglu, Ufuk; Melek, Mehmet; Kara, Volkan; Mergan, Duygu
Aim: The aim of this study was to review cases with isolated hemothorax after thoracic trauma, to assess the diagnostic and treatment methods, and to discuss the determining factors of morbidity and mortality. Material and Method: A total of 57 patients were examined retrospectively. All patients underwent tube thoracostomy and underwater seal as the initial treatment approach. Emergency thoracotomy was performed on cases with continuing drainage from the chest tube, expanding hemothorax on the posteroanterior (PA) chest x-ray, and with hemodynamic instability. Result: Hemothorax occurred as a result of penetrating trauma in 31 (54.38%) and due to blunt trauma in 26 (45.61%) cases. Of the cases, 49 (85.96%) underwent tube thoracostomy drainage. All the 8 cases (14.03%) that underwent emergency thoracotomy had penetrating trauma. Mortality occurred in one patient (1.75%) who had penetrating trauma and who underwent emergency thoracotomy. The morbidity rate in patients with blunt trauma was significantly higher than those with penetrating trauma (p<0.0001). Discussion: Accurate diagnosis and appropriate surgical intervention in cases with traumatic hemothorax is essential for reducing the morbidity and mortality. Chest tube insertion and underwater seal application should be the initial treatment modality and successful in most cases. Emergency thoracotomy is life-saving in indicated patients. The need for thoracotomy is higher in isolated hemothorax due to penetrating chest trauma.
