Objective: Coronary artery ectasia (CAE) is increasingly recognized as an active inflammatory vascular disorder rather than a benign anatomical variant. The pan-immune-inflammation value (PIV) is a novel biomarker integrating neutrophil, monocyte, platelet, and lymphocyte counts, providing a comprehensive measure of systemic inflammation. This study aimed to evaluate the association between PIV and CAE and to compare their diagnostic performance with that of conventional inflammatory indices. Method: In this retrospective case-control study, 17,538 patients who underwent elective coronary angiography between 2018 and 2024 were screened. A total of 228 patients with isolated CAE and 296 age-, sex-, and Body Mass Index (BMI)-matched controls with normal coronary arteries were included. Hematologic and biochemical parameters were analyzed, and inflammatory indices were calculated. Logistic regression and receiver operating characteristic (ROC) analyses were performed to identify independent predictors and assess diagnostic performance. Results: Patients with CAE had significantly higher PIV levels compared to controls (801.6 [504.4-1301.8] vs. 491.8 [302.4-872.7], P < 0.001). In multivariable logistic regression, log-transformed PIV remained independently associated with CAE (odds ratio [OR]: 1.987, 95% confidence interval [CI]: 1.057-3.737, P = 0.033), along with hypertension, triglycerides, high-density lipoprotein (HDL) cholesterol, and serum creatinine. PIV demonstrated the highest discriminative ability among all inflammatory indices (area under the curve [AUC]: 0.674, 95% CI: 0.623-0.722), and correlated strongly with the Systemic Immune-Inflammation Index (SII), neutrophil-to-lymphocyte ratio (NLR), platelet-to-lymphocyte ratio (PLR), and Systemic Inflammation Response Index (SIRI) (P = 0.75-0.94). Conclusion: Elevated PIV levels are independently associated with CAE, reflecting the pivotal role of systemic inflammation in its pathogenesis. Given its simplicity and availability, PIV may serve as a practical adjunctive marker for identifying patients at risk of CAE, warranting validation in larger prospective studies.

Soygenc, Elcin Izmir; Onur, Ozge Sahmelikoglu
Objective: Growing evidence suggests that unipolar mania (UM) may represent a distinct clinical condition rather than a subtype of bipolar disorder (BD). The present study aimed to compare the sociodemographic characteristics, clinical features, and psychosocial functioning of euthymic patients with UM or bipolar I disorder (BD-I) and healthy control subjects. Method: This cross-sectional study included euthymic patients with UM (n=58), BD-I (n=58), and healthy control subjects (n=58). Diagnoses were established according to the Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition (DSM-5) criteria, and euthymia was confirmed using the Young Mania Rating Scale, Beck Depression Inventory, and Positive and Negative Syndrome Scale. Psychosocial functioning was assessed using the Functioning Assessment Short Test (FAST) in all groups and the Bipolar Disorder Functioning Scale (BDFS) in the UM and BD-I groups. Sociodemographic and clinical data were collected using a structured form. Group comparisons were conducted using appropriate parametric and nonparametric statistical analyses. Results: Both patient groups demonstrated significantly impaired psychosocial functioning compared with healthy controls across all FAST subdomains (p<0.05). On the BDFS, patients with UM showed better emotional functioning and less social withdrawal than patients with BD-I (p<0.05); however, total functioning scores did not differ significantly between groups (p>0.05). Clinically, UM was characterized by manic-predominant onset, higher rates of mood-congruent psychotic symptoms, longer episode duration, and more frequent hospitalizations. Suicidal behavior and total number of mood episodes were higher among patients with BD-I (p<0.05). In the final logistic regression model, mood-congruent psychotic symptoms during the first episode were independently associated with UM relative to BD-I, whereas a history of suicide attempts was associated with lower odds of UM (p<0.05). Conclusion: Although both groups exhibited functional impairment, patients with UM demonstrated relatively better emotional and social functioning than those with BD-I. These findings suggest that UM may be associated with a distinct functional profile, with potential implications for prognosis and treatment planning. Incorporating functional outcomes into the clinical assessment of UM may help optimize therapeutic strategies and improve long-term psychosocial outcomes.

Tuncer, Gulsum Zekiye; Tuncer, Metin
Background: The rapid adaptation of international nursing students to university life enables them to actively continue their education without disruption in the following years. Identifying the challenges faced by these students, who are future nursing professionals, is essential fortheir adaptation and professional development. Aim: This study aimed to explore the perspectives of international nursing students regarding their first-year experiences at university. Methods: This study employed a descriptive qualitative research design. The Consolidated Criteria for Reporting Qualitative Research (COREQ) was used for reporting. The sample consisted of 13 international nursing students enrolled in the Faculty of Nursing at a university. Data were collected through face-to-face, individual, in-depth interviews using semi-structured interview questions. Content analysis was used to analyze the data. Results: The mean age of the nursing students was 23.3 years. Among the participants, seven were women, 11 had been residing in Turkiye for approximately one year, 12 were from Iran, and one was from Iraq. Three key themes emerged in this study: Facilitators of Adaptation, Challenges, and Coping. The students identified several facilitators of adaptation, including similarities between the Turkish economy and culture and their home countries' economies and cultures, proficiency in the Turkish language, studying at a nursing school aligned with their career goals, and receiving social support from their relatives. However, they also faced challenges such as language barriers, cultural differences, discrimination, social isolation, homesickness, and academic and clinical difficulties. Conclusion: International nursing students may encounter language barriers, cultural differences, and education-specific challenges during their first year of nursing school. In this context, developing support mechanisms to guide these students in their academic and social lives will facilitate their ability to cope with these difficulties.
- MakaleYalnız künye2023The Relationship between Urbanization, Energy Consumption and Carbon Dioxide Emissions: Panel Ardl Analysis for Newly Industrialized Countries
Akyol, Mehmet
An increase in the world population, technological advancement, and economic concerns bring along an increase in urbanization and energy consumption. Increasing urbanization speed and energy consumption cause environmental problems. The main hypothesis of this study is based on the idea that urbanization rate and energy consumption increase carbon dioxide emissions in developing countries. To test this hypethesis, the study ana-lyzed the relationship between urbanization, energy consump-tion, and carbon dioxide emission specific to newly industrialized countries. Panel ARDL method was utilized in this study that scrutinized the years between 1990 and 2018. According to the long-term results of the analysis, urbanization negatively affects carbon dioxide emission in the country group aforementioned; energy consumption and economic growth that is control vari-able have a positive effect on carbon-dioxide emission. Regarding short-term results, only the energy consumption positively af-fects carbon dioxide emission while other variables are statisti-cally insignificant. The results of the analysis reveal the necessity of policies towards smart city applications in reducing carbon dioxide emissions in newly industrialized countries. In addition, turning to renewable sources in energy consumption plays a key role in reducing carbon dioxide emissions.

Sert, Seda; Arici, Ceyhun; Mergen, Burak; Ekmekci, Ozlem Balci
Objectives: Investigation of the surgical correction effect on tear film functions and tear oxidative stress levels in patients with blepharoptosis and pseudoptosis. Methods: Sixty patients with blepharoptosis or pseudoptosis due to dermatochalasis and 30 healthy controls were included in the study. Forty eyes underwent upper blepharoplasty and 20 eyes underwent levator surgery. The tear oxidative stress levels (8-hydroxydeoxyguanosine [8-OHdG] and 4-hydroxy-2-nonenal [4-HNE]) were determined by enzyme-linked immunosorbent assay and tear film functions were evaluated pre-operatively and at the post-operative 1st and 6th months. Results: 8-OHdG and 4-HNE levels in tears were found higher in patients with dermatochalasis (86.3 +/- 38.2 ng/mL; 29.8 +/- 11.4 ng/mL, respectively) and blepharoptosis (95.3 +/- 43.8 ng/mL; 40.8 +/- 3.8 ng/mL, respectively) compared to healthy controls (52.9 +/- 14.0 ng/mL; 27.8 +/- 6.6 ng/mL, respectively). Both levels decreased 1 month after blepharoplasty surgery. The 8-OHdG level in tears of patients who underwent levator surgery increased 1 month after the surgery (p=0.008). No change was detected in tear function tests findings between visits in any patient group. Conclusion: Dermatochalasis and blepharoptosis may lead to an increase in the tear oxidative stress levels. Contrary to a decrease in these levels after blepharoplasty, they may increase in the early period after levator surgery followed by a return to normal levels at the 6th-month visit.

Objective: Enhanced external counterpulsation (EECP) is a noninvasive treatment that is proven safe and effective in patients with coronary artery disease (CAD) and heart failure (HF). The aims of this study are to investigate the clinical effects of EECP therapy in patients with symptomatic CAD and chronic HF, and to find out an answer to the question: Does EECP therapy have any effect on the prognostic markers of HF? Methods: This study was designed as a prospective cohort study. A total of 68 consecutive patients with symptomatic CAD and chronic HF referred to EECP therapy were enrolled in this study between November 2007 and December 2010; 47 patients (39 males and 8 females, 65 +/- 7, years), have undergone EECP treatment, and 21 patients (20 males and 1 female, 62 +/- 10 years), who did not want to participate in the EECP program comprised the control group. Statistical analysis was performed using t tests for dependent and independent samples, Mann-Whitney U test, Chi-square and Fischer exact tests. Results: EECP therapy resulted in significant improvement in post-intervention New York Heart Association functional class (p<0.001), left ventricular ejection fraction (p<0.001), B-type natriuretic peptide levels (p<0.003), uric acid levels (p<0.05), free-T3/free-T4 ratio (p<0.034) and mitral annular E (p<0.05) velocity, compared with baseline, a finding not evident in the control group. Conclusion: EECP treatment significantly improved clinical and some biochemical parameters, which are mostly prognostic markers in patients with symptomatic CAD and chronic HE (Anadolu Kardiyol Derg 2012; 12:214-21)
