Efficacy of Second-Line Treatments After Atezolizumab and Bevacizumab in Advanced Hepatocellular Carcinoma and Related Prognostic Factors: A Multicenter Study by the Turkish Oncology Group (TOG)

Majidova, Nargiz; Yaslikaya, Sendag; Mildanoglu, Maral Martin; Coskun, Alper; Uzundal, Duygu Ercan; Sahin, Taha Koray; Akyildiz, Arif; Akbas, Sinem; Camanli, Ufuk; Sahin, Elif

Background/Aims: The treatment of hepatocellular carcinoma (HCC), which accounts for 90% of all liver cancers, is highly varied. The use of second-line treatments following progression on first-line atezolizumab and bevacizumab (Atez/Bev) for advanced HCC remains controversial. The aim of this study was to analyze the real-world clinical results of second-line treatments in progression after Atez/Bev and to determine the factors affecting prognosis. Materials and Methods: Fifty-eight patients treated with second-line sorafenib, regorafenib, and cabozantinib for progression after first-line Atez/Bev for advanced/metastatic HCC from 20 centers in T & uuml;rkiye between October 2020 and June 2024 were retrospectively analyzed. Responses were evaluated by Response criteria, specifically Response Evaluation Criteria in Solid Tumors (RECIST v1.1) criteria. Median overall survival (OS) and progression-free survival (PFS) were computed with the Kaplan-Meier method. The Cox regression model was utilized to analyze multivariate analyses. Results: About 82.8% of the patients were male and the median age of the whole group was 62 (range, 18-78) years. All patients progressed after first-line Atez/Bev and were given second-line treatment. The most commonly used second-line treatment option was sorafenib (70.7%), followed by regorafenib (12.1%) and cabozantinib (10.3%). Both median PFS (4.1 months) and median OS (7.8 months) were longer in patients treated with sorafenib compared to other treatments. In univariate analyses, Child-Pugh score B, high alpha-fetoprotein (AFP) levels (>200 ng/mL), extrahepatic spread, and Prognostic Nutritional Index (PNI) < 47.6 substantially raised the risk of overall mortality. Multivariate analysis showed that extrahepatic spread (HR (Hazard ratio): 0.41, P = .012), PNI level (HR: 0.24, P = .005), and AFP level (HR:1.97, P = .049) were independent predictors of OS. Conclusion: Although second-line therapies after Atez/Bev show different degrees of efficacy, survival rates are consistent with the literature. Extrahepatic spread, AFP level, and PNI level are the main prognostic factors. In light of this information, personalized treatment strategies may improve outcomes for this challenging patient group.

Nayman, Alaaddin; Guler, Ibrahim; Keskin, Suat; Erdem, Tuba Berra; Borazan, Hale; Kucukapan, Ahmet; Ozbiner, Huseyin; Batur, Abdussamed; Ertekin, Ersen; Feyzioglu, Bahadir

PURPOSE We aimed to demonstrate the success and reliability of a novel puncture, aspiration, injection, and reaspiration (PAIR) technique in liver hydatid cysts. METHODS Percutaneous treatment with ultrasonographic guidance was performed in 493 hepatic hydatid cysts in 374 patients. Patients were treated with a new PAIR technique by single puncture method using a 6F trocar catheter. The results of this novel technique were evaluated with regards to efficacy and safety of the procedure and complication rates. RESULTS Out of 493 cysts, 317 were Gharbi type I (WHO CE 1) and 176 were Gharbi type II (WHO CE 3A). Of all cysts, 13 were referred to surgery because of cystobiliary fistulization. Recurrence was observed in 11 cysts one month later. Therefore, the success rate of the PAIR technique was 97.7% (469/480). Minor complications (fever, urticaria-like reactions, biliary fistula) were seen in 44 treated patients (12%, 44/374); the only major complication was reversible anaphylactic shock which was observed in two patients (0.5%, 2/374). CONCLUSION This novel modified PAIR technique may be superior to catheterization by Seldinger technique due to its efficiency, easier application, lower severe complication rate, and lower cost. Further comparative studies are required to confirm our observations.

    Yilar, Sinan; Yildirim, Omer Selim

Objective: To provide guidance in the selection of the appropriate patient for arthroscopic debridement. Materials and Methods: Ninety patients who were diagnosed with gonarthrosis according to modified American College of Rheumatology (ACR) criteria and who underwent arthroscopic debridement and meniscectomy were evaluated. Functional assessment of patients was made based on preoperative as well as 1 month and 1 year postoperative results of the Lysholm Knee Score and the Visual Analog Scale (VAS). One year postoperatively, the Tapper-Hoover Scoring System was used to calculate long-term results. Analyses were conducted using SPSS 18 software. For this study ethics committee approval was received from the ethics committee of Ataturk University Medical Studies Department Head on 08.10.2010 at the 6th meeting with regards to the document written on 16.06.2010 with number 998. Results: Differences between preoperative and 1-month and 1-year postoperative values in the Western Otorino and McMaster Universities Osteoarthrosis (WOMAC) Index, Lysholm and VAS were found to be statistically significant (p<0.001) (Table 1). According to TapperHoover results, 82.8% of patients benefited from arthroscopic debridement in the long term (Table 2). Among body mass index (BMI) groups, the WOMAC, Lysholm and VAS values of obese patients were worse than those of normal weight and underweight patients, and the difference was statistically significant (p<0.005). Late-term results of patients younger than 55 were superior to those over 55. Conclusion: In the treatment of patients with gonarthrosis, arthroscopic debridement is a good option. Patients who have a low BMI and are younger than 55 years old experience more benefit from arthroscopic debridement.

Memetoglu, Mehmet Erdem; Kurtcan, Serpil; Erbasan, Ozan; Özel, Deniz

PURPOSE We aimed to present the short- to mid-term results of endovenous laser ablation (EVLA) procedures that were used to treat great saphenous vein (GSV) insufficiency. MATERIALS AND METHODS This prospective study was conducted between December 2009 and January 2011. A total of 112 incompetent GSVs were treated using EVLA with a 940 nm wavelength laser. Ninety patients were treated for varicose veins with saphenous reflux, including 36 females (40%) and 54 males (60%). These patients' ages ranged from 17 to 79 years (median, 48 years). After the EVLA, the patients were monitored using duplex ultrasonography and were assessed clinically at 1 week and 1, 3, 6, and 12 months after the surgery. The patients were scheduled for a three-day examination after the EVLA to assess the level of pain that each patient was experiencing in each limb. RESULTS At the end of a one-year follow-up period, the postprocedural duplex scans revealed a total occlusion of the treated GSVs in 88 (97%) patients and a sub-total occlusion in two (2%) patients. The average modified clinical, etiological, anatomical, and pathological score was significantly decreased at 12 months. The following complications were observed in the present study: hypoesthesia (11%), swelling and induration (5%), skin pigmentation (5%), deep vein thrombosis (1%), erythema (1%), and bleeding (1%). The mean visual analog pain score for the entire procedure was 3.14 +/- 1.06. CONCLUSION Our short- and mid-term results of the EVLA procedure were satisfactory, and the results of this study reaffirmed the safety and effectiveness of an EVLA using a 940 nm wavelength for the treatment of GSV insufficiency.
