The evolutionary path of health expenditure research: unveiling dynamic insights through topic modeling

Demır, Hüseyın; Karaer, Merve; Uruş, Ali Fuat

The increasing burden of healthcare services on national economies has made understanding the dynamics of health expenditure a crucial issue. Therefore, there is a need to analyze the complex and interdependent patterns of these dynamics. The aim of the study is to identify the key themes in the literature related to health expenditure and examine the relationships between them. A total of 3,449 scientific publications were obtained from the Web of Science database, and topic modeling was conducted using the Latent Dirichlet Allocation model. As a result, the themes related to health expenditure were identified as health economics and policy, healthcare services and research, patient care and health outcomes, and healthcare cost and utilization. The findings emphasize the importance of adopting cost-effective practices, improving service processes, and integrating health outcomes into healthcare service processes for effective management of health expenditure. Additionally, the study shows that topic modeling provide valuable insights for policymakers in the healthcare sector and that machine learning approaches can lead to more scalable and efficient solutions in healthcare domain.

The Burden of Covid-19 Cases on the Payer Institution: The Case of Aydın Provincial Ambulance Service

Demır, Hüseyın; Karaer, Merve; Kaplan, Selma

The study aims to assess the economic burden of Covid-19 cases on the payer institution in the context of ambulance services and to investigate the factors affecting costs. Data were obtained from Aydın Provincial Ambulance Service. Cumulative frequency, cumulative percentage and mean (standard deviation) statistics were used for descriptive findings. Logistic regression model was used to examine the factors affecting costs. Covid-19 cases posed a significant economic burden. The cost per case was approximately 466.40 TL and this amount was higher with additional diagnoses. The pandemics created a higher economic burden in 2020. Accordingly, Covid-19 cases accounted for 14.92% of the number of cases and 14.75% of the costs, with an average cost per case of 447.12 TL. In the second year (2021), the economic burden of the pandemic decreased significantly. Accordingly, the share of Covid-19 cases was 7.69%, while the cost share was 7.12%. The average cost in that year was 503.6 TL. Age, triage, reason for call, urban & rural and region variables had a significant effect on costs. There was a statistically significant relationship between advanced age and high costs per case. Covid-19 cases caused a significant economic burden on the payer. It is considered that the implementation of principles and practices related to risk and crisis management for provincial ambulance services will contribute to reducing costs. It is thought that the findings will be useful for other decision makers and policy makers, especially the payer institution.

A Challenge for Systemic Transformation towards Circular Healthcare Economy: Single-Use or Not?

    Demır, Hüseyın; Karaer, Merve

The study aims to discuss the challenge of systemic transformation of healthcare economy in the context of disinfection and sterilization (DAS) process. The database of the Web of Science (WoS) has been used to obtain data. In R environment, a variety of analyzes have been conducted through the biblioshiny. Following the descriptive findings, trend words, trend word weights and related visuals have been obtained. Multiple correspondence analysis was used to evaluate the development course of trending words by year, and the ggplot2 package has been used to visualization. An attempt has been made to draw attention to the difficulty of the systemic transformation of the healthcare economy by supporting the circular approach concepts featured in the publications with knowledge and experience from the field. The number of publications and citations in the field has shown an increasing trend. Prominent studies have been conducted in the areas of infection control, dialysis, anesthesia, and analgesia. Trend words have shown that life cycle assessment, reuse, sterilization, reprocessing, etc. concepts have a high level of centricity and concentration. Similarly, multiple correspondence analysis findings have shown that a heavy reliance has been placed on DAS processes in recent studies. The results clearly show that the systemic transformation from a linear healthcare economy to a circular one will create challenges for hospital administrations. It is therefore assessed that a hybrid approach to the continued use of disposable products instead of a purely circular approach will be beneficial to the sustainability of healthcare.

New Algorithm for the Lid-driven Cavity Flow Problem with Boussinesq-Stokes Suspension

    Süngü, İnci Çilingir; Demır, Hüseyın

In the present investigation, a streamfunction-vorticity form for Boussinesq-Stokes liquids (with suspended particles) is suitably usedto examine the problem of 2-D unsteady incompressible flow in a square cavity with moving top and bottom wall. A new algorithm isused for this form in order to compute the numerical solutions for high Reynolds numbers up to Re=2500. This algorithm is conductedas a combination of the multi-time-stepping temporal differential transform and the spatial finite difference methods. Convergenceof the time-series solution is ensured by multi-time-stepping method. The classical benchmark results of the Newtonian liquid arerecovered as a limiting case and the decelerating influence of the suspended particle on the Newtonian liquids’ flow field is clearlyelaborated.
