The purpose of the study is to determine work-related tension, using the methods of over- coming the stress and the affecting factors in emergency staff MMaatteerriiaall aanndd MMeetthhooddss:: The universe of the study in descriptive type is consisted of health team members in institutions rendering emergency health service belong to public sector in Turkey. Turkey was divided into 4 clusters (east, west, south and north) by taking the geographical settlement areas into consideration and a province was selected from each cluster in random. The number of peo- ple within the scope of the research is 769 and 560 people was reached (response rate is 72.82%). Approval of ethi- cal committee and written permissions were acquired from the related institutions before the research. There are questions including the demographic characteristics of the participants and their opinions on the job in first section of the form in which data were gathered, and “Work-Related Tension Scale -WRTS” in the second section, and “Overcoming the Stress Scale -OSS” in the third section. The data were recorded in statistical program. The aver- ages were given with standard deviations and p<0.05 was accepted as meaningful. Frequency analysis was carried out in the evaluation of the data and, Mann Whitney U test, Kruskall Wallis test and Spearman Correlation Analy- sis were implemented. RReessuullttss:: Average of age of the participants was 29.71±7.92 years. WRTS point average was 49.80±5.92 and was found high in young people, having low education level, in emergency medical technicians (EMT), the ones working on contract, having negative point of view against the occupation (p<0.05). OSS point av- erage of the participants was 56.56±11.23. CCoonncclluussiioonn:: It was detected that WRTS point average in the participants was over scale point average and OSS point average was under scale point average. In health personnel rendering emergency service; service and training studies that will increase the level of using overcoming the stress methods, decrease the work-related tension and improve affecting factors should be given priority

Breast Conserving Surgery and Modified Radical Mastectomy in Turkish Women with Breast Cancer: A Prospective Analysis of Quality of Life

Kement, Metin; Gezen, Cem; Asik, Ayla; Karaöz, Alev; Ustaalioglu, Basak Oven; Bilici, Ahmet; Öncel, Mustafa

Objective: The aim of this study is to compare the postoperative quality of life of the patients who underwent breast conserving surgery followed by radiotherapy (BCT) and the patients who underwent modified radical mastectomy (MRM) in our clinic. Material and Methods: The patients who were operated on with diagnosis of breast cancer in our clinic between January 2006 and January 2008 were included in the study. Those who received neoadjuvant therapy and who required radiotherapy after modified radical mastectomy and those in ASA classes 3 and 4 were excluded. The patients who were treated with modified radical mastectomy were included in the first group (Group MRM) and the patients who were treated with breast conserving therapy and radiotherapy thereafter were included in the second group (Group BCT). Quality of life of the patients were assessed using Medical Outcome Syudy-Short Form 36 (MOS SF-36) form six months after their treatment (surgery, radiotherapy, chemotherapy) was completed. Results: There were no significant differences between two groups in terms of age, evaluated socioeconomic parameters (education, income, marital status) and clinical parameters (stage, complications, adjuvant chemotherapy). Quality of life of the patients who underwent BCT (n=27) was worse than that of general Turkish population in six of eight (except vitality and social function) SF-36 scale and worse than mean values of normal individuals in NSABP BCPT trial in seven of eight (except vitality) scales. Besides, while physical component score was worse than that of general population (45.3 vs 52.6), mental component score (MCS) was relatively similar to general population (53.4 vs 51.7). When quality of life of the patients who underwent MRM (n=23) and BCT were compared, mean values of BCT group were found to be better in all scales and the difference was found to be statistically significant in six out of eight (except vitality and social function) scales (p<0.05). Conclusion: It can be stated that quality of life of the patient who underwent BCT due to breast cancer is impaired in terms of physical status, however did not change in terms of mental status. It can also be stated that quality of life of the patients who underwent breast conserving therapy were better than the ones who underwent modified radical mastectomy in terms of both physical and mental status.

The relationship between the peroxisome proliferator-activated receptor gamma 2 gene polymorphism, lipids and adipokines in patients with major depression; [Majör depresyonlu hastalarda peroksizom çoǧaltici-aktive edici reseptör gama 2 gen polimorfizmi, lipidler ve adipokinlerle ilişkisi]

Objective: Peroxisome proliferator-activated receptor gamma (PPARγ), lipids, lipoproteins, and adipokines have recently been shown to be associated with psychiatric diseases. Our major aim is to investigate the contribution of the PPARγ gene polymorphism, adipokines, lipids, and lipoproteins to the development of major depression. Material and Methods: The frequency of Pro12Ala in exon 2 and C478T in exon 6 of the PPARγ gene, lipids and adipokines in major depression (n = 78) and control subjects (n= 64) were analyzed. Genotypes of PPARy gene polymorphisms were examined. Serum leptin, adiponectin, and resistin were studied by enzyme-linked immunosorbent assay (ELISA). Serum apo A1, apo B, and Lp(a) levels were determined by immunonephelometry. Serum total cholesterol, triglyceride, HDL-cholesterol and LDL-cholesterol levels were analyzed by enzymatic methods. Results: The genotypes of exon 2 and exon 6 distribution did not differ between the control subjects and patients with major depression. Frequencies of genotypes of Pro12Ala, and Ala12Ala in exon 2 in overweight and obese patients with major depression were higher than those of overweight and obese controls. C478T polymorphism was highest in overweight and obese patients with major depression. Pro12Ala and Ala12Ala genotypes in exon 2 of PPARγ gene in patients were found to be associated with triglyceride and HDL-cholesterol. There were significant differences regarding glucose, total cholesterol, LDL-cholesterol, apo B, Lp(a), adiponectin and resistin levels between patient and control subjects. Conclusion: PPARγ exon gene polymorphisms, alterations in lipid profile and adipokines may be associated with the development of major depression.
